AbstractThe appropriateness of accessibility models for evaluating efficiency and equity in the distribution of general practitioners in New Zealand cities is examined, using the example of Christchurch. Such models, commonly used in town planning and increasingly in health services research abroad, represent an objective technique for aiding decision‐makers in assessing planning options. There are considerable variations in accessibility to primary health care in Christchurch with the greatest shortage being experienced in the eastern suburbs. SummaryBy use of data on smoking induced death in Americans it has been estimated that current levels of cigarette smoking in Australia caused some 10,434 premature deaths in 1978. These deaths represented a total loss of about 156,730 person years of life, of which about 70,005 were lost before the age of 70 years. By way of comparison, in the same year, road crashes caused some 3,728 premature deaths with loss of about 152,211 person years of life in total, or about 123,062 before the age of 70 years. These results illustrate one method of setting priorities for action in preventive medicine.

AbstractA survey of the medicines stored in 102 randomly selected households revealed a total of 1,181 medicines, with a range of 0–67 medicines per household. The number of medicines per household was positively correlated with household size, and households with children tended to have more medicines than childless households. The most commonly occurring medicines were skin preparations and analgesics. The median storage time was 3–6 months, and only 40 percent of the medicines were said to be in current use. Twenty‐seven percent of the medicines found were available on prescription only, and these medicines had been stored for less time and were more likely to be in current use than other medicines. The kitchen was the most popular place for storage of medicines (42 percent) followed by the bathroom (36 percent); only 5 percent of all medicines were kept in a locked cupboard. SummaryA controlled intervention trial was set up in 1976 to evaluate a program to lower risk factors for coronary heart disease. The 3,401 persons who attended a voluntary screening program at their place of work, became the subjects. All subjects in the treatment group were given their results; those with a high blood pressure reading were referred to their doctor, while those with increases in other risk factor levels were offered educational group programs and literature. For control subjects, the only intervention was the sending of. screening results, with advice to consult a doctor, for the 10 per cent at highest risk.Repeat screening after one year showed that similar changes in risk factor levels had occurred in both the treatment and the control groups, although changes in blood pressure favoured the control group. This program of screening and intervention had little effect on the level of coronary disease risk factors.

The theme of this ‘commentary’ will be limitations on policy and planning. Recognition of limitations has now been the angst of the times for ten or fifteen years, as the titles of popularly influential writing, say ‘Limits to Growth’ or ‘Small is Beautiful’ and even ‘Limits to Medicine’, illustrate. However, the books I shall draw upon for this review article emphasise, by and large, limitations of a different character from the apocalyptic ‘Limits to Growth’ school or the Utopian obverse of the same coin.
