AbstractRecent attention has focused on the difficulties of providing equal employment opportunities for women while ensuring reproductive health in the workplace. This paper examines the potential hazards faced by pregnant firefighters and recommends a policy that seeks a reasonable accommodation between employment and fetal and maternal health. The potential hazards faced by firefighters include physical exertion, hyperthermia, and exposure to carbon monoxide and other toxic gases. The ideal policy option for improving reproductive health is to make the workplace safe for all employees. Where this cannot be accomplished and a unique reproductive hazard exists to women during pregnancy, temporary positions should be found that remove the pregnant woman from the hazard while allowing her to continue work.

AbstractA proportionate mortality study of police and firefighters in New Jersey was conducted using the records of a comprehensive retirement system. Three reference populations were used: (1) U.S. general population, (2) New Jersey general population, and (3) police as a reference group for the firefighters. Overall neither group differed from the New Jersey male population in the cause of death. Analyses by latency showed an increase in skin cancer and cirrhosis in firefighters and cirrhosis in police. With increased time from first employment, an inverse association was found between heart disease and time of first exposure. This was reflected in statistically significant increased proportionate mortality rates (PMR) for arteriosclerotic heart disease (ASHD) (ICD 410–414) for both working police (PMR = 1.15) and firefighters (PMR = 1.2). Retired police and firefighters had PMRs of 0.96 and 0.98, respectively. Firefighters had a significant increase in nonmalignant respiratory disease (PMR = 1.98) and leukemia (PMR = 2.76) when the police were used as a reference group. AbstractA 41‐year‐old man was found to have a malignant mesothelioma of the pleura. During childhood in Corsica, he had been exposed at home to chrysotile ore from the Canari mine. Analysis of lung mineral content revealed background levels of chrysotile but an elevated level of tremolite and actinolite asbestos. The latter had a geometric mean length of 3.7 μm, a value considerably longer than we have found for tremolite and actinolite from Quebec chrysotile miners but roughly the same as the mean length of amosite and crocidolite in workers with occupational amphibole exposure. No tremolite or actinolite fibers of length greater than 8 μm microns and width less than 0.25 μm were observed. The mean aspect ratio of the tremolite and actinolite fibers was 7, a value similar to that found in chrysotile miners with mesothelioma but considerably less than the mean aspect ratio of amosite and crocidolite from those with occupational expsoure. AbstractPsychological performance was examined in relation to central and peripheral nerve conduction by means of the Wechsler Adult Intelligence Scale test, short‐latency somato‐sensory‐evoked potential (SSEP), and median nerve conduction velocity in 19 male gunmetal foundry workers exposed to lead, zinc, and copper. (Their blood lead concentrations—ie, 16–64 μg/dl with a mean of 42—and plasma zinc and copper concentrations were significantly higher than those of control subjects.) In these workers, the score of picture completion (psychological performance) was significantly low; indicators of lead absorption, but no indicators of zinc and copper absorption, were significantly correlated with this score. The score of picture completion was significantly correlated with the N11–N13 latency of SSEP (conduction time in the spinobulbar region) in the workers; their N11–N13 latency, together with the N9 and N9–N11 latencies, was significantly prolonged and was significantly correlated with indicators of lead absorption. Furthermore, their maximal motor and sensory conduction velocities of the median nerve were significantly slowed. AbstractAs recognition of occupational illness increases, the scope of health problems related to work widens. An important area of concern is the worker's family, which has been shown to be at increased risk of disease attributable to the hazards previously thought to be relevant only to the worker. Such “para‐occupational” disease occurs particularly in spouses and children through transport by the worker of hazardous materials from the worksite into the home. The most common vehicle has been contaminated work clothing brought home for cleaning. Outbreaks of severe illness caused by lead, beryllium, asbestos, and other compounds have been traced to home contamination by industrial dust. AbstractTo evaluate a suspected association between an outbreak of acute respiratory illness and eosinophilia and employment as a rubber worker, we performed a retrospective review of medical records of rubber workers employed from September 1983 to July 1984 in a plant housing a thermoinjection process. Twenty‐five workers met the case definition of a respiratory illness requiring a physician visit. The predominant respiratory illness was acute in onset with cough, chest tightness, and dyspnea. Peripheral eosinophilia, up to 40% of white blood cells in a peripheral smear, was seen in 10 of 18 (56%) cases. Twenty‐one of 25 white males with respiratory symptoms were employed in the thermoinjection process (odds ratio = 22, p<.001). Smoking and employment in this process contributed independently to an increased risk of being a case as determined by a logistic regression analysis. Return to the plant building caused recurrence of symptoms in most cases, and these workers have been transferred or left the company. AbstractA new case of rare‐earth (RE) pneumoconiosis is described. The subject had worked as a photoengraver for 13 years and had not been exposed for 17 years. Chest X‐ray showed a diffuse nodular pattern (q 2/3–ILO/1980). The patient was asymptomatic despite a restrictive spirometric impairment. The diagnosis derived from the finding, in the bronchoalveolar lavage fluid, of abnormal levels of La, Ce, Nd, Sm, Tb, Yb, and Lu. The presence of these elements was demonstrated by two methods: the neutron activation analysis and (as regards Ce alone) the X‐ray energy spectrometry of mineral particles observed with electron microscope.
