Three wall intrabony defectswere produced in 11 dogs using a round bur followed by curettes and hoes. A copper band was fixed to the tooth with stainless steel ligature wire. Six weeks later, the copper band was removed and the defect was treated with an enriched collagen solution (ECS) prepared from acid‐extracted dog skin collagen. Thirty‐three defects were treated with ECS and 33 defects were controls. Healing was assessed histologically 4 and 6 weeks after treatment for the presence of new cementum, periodontal ligament and alveolar bone, as well as arrested epithelial downgrowth along the dental root. Unlike the controls, treatment with ECS resulted in restoration of periodontal attachment after 4 weeks. This included formation of new cementum, new alveolar bone and dense connective tissue fibers running between bone and cementum. Control sections showed epithelial migration along the root, separating it from the adjacent connective tissue and thus preventing new attachment.

Interproximal gingival tissueswere compared histologically relative to the presence or absence of bleeding after stimulation with a wooden interdental cleaner. Fifteen bleeding and 15 nonbleeding interproximal gingival biopsy specimens were obtained from 30 patients and processed for light microscopic evaluation. Morphometric analysis of tissue components revealed that bleeding areas had a significantly greater per cent of inflamed connective tissue. The inflammatory lesion in the bleeding specimens was primarily in the midinterproximal area. From a diagnostic standpoint, this information provides a rationale for the use of bleeding to detect inflammatory lesions in the midinterproximal region.

Seven patients who participatedin a clinical study comparing the response of periodontal osseous defects to either grafting with Durapatite ceramic or debridement alone were followed under an active maintenance program for 3 years. Comparison of mean values showed that gingival recession, attachment levels and probeable pocket depths remained essentially stable for Durapatite sites over the 3 years. Attachment levels for debridement sites also remained constant, but these areas showed significant coronal migration of the free gingival margin and increases in probeable pocket depths. These latter changes became apparent and significant only after 3 years. Intrapatient comparisons showed that recession and attachment gain were most frequently greater and pocket depths most frequently less for Durapatite sites. These findings suggest that the Durapatite‐treated sites were stable for 3 years following surgery, while areas treated with debridement alone regressed.

摘要:
An unusual gingivitis notedas a discrete lesion involving the facial attached gingiva in a focal and confined area has been described in two adolescent females. A unique histopathologic appearance includes a striking intraepithelial edema associated with a focal area of acute and chronic inflammation.Inconclusive immunologic study of one patient indicated an epithelial‐connective tissue immune complex deposition suggestive of but not typical of that seen in pemphigoid.

The uptake ofCa2+by endotoxin‐challenged 3T6 fibroblasts,in vitro, was studied. In recent years, the role of calcium in cell injury ultimately leading to cell death has attracted a fair amount of interest. The purpose of the study was to determine whether the direct toxic action of endotoxin is related to a disturbance in Ca2+homeostasis. Increased calcium uptake in endotoxin‐challenged cells was found to be directly related to the bacterial source and method of extraction of endotoxin, the cell density of the culture and the pH of the medium. The effect of endotoxin on calcium uptake was completely reversed by polymyxin B which is known to neutralize the endotoxicity of lipopolysaccharides. These results imply that the increased calcium uptake may be one of the mechanisms by which endotoxin causes direct tissue damage. The potential significance of these data to periodontal disease is discussed.

Effects of protein‐energy malnutritionwere studied in newborn rats and their dams. Upon delivery, dams received 6%, 12% or 20% protein diets. At Day 15 pups received14C(U)‐proline. The posterior tongue, hard palatal mucosa, soft palatal mucosa and skin were analyzed for collagen and counts incorporated (collagen synthesis, that is, the rate of14C‐proline converted to14C‐hydroxy‐proline into gingival collagen). These regions of the dams were also removed to study collagen content. Although soft palatal mucosal collagen of newborns in the 6% and 12% protein groups was decreased, that of skin in the 6% protein group was increased. No such differences were observed in tongue and hard palatal mucosa. Counts incorporated was decreased in the tongue, soft palatal mucosa and skin, but not in hard palatal mucosa. Collagen contents of tongue and hard palatal mucosa and skin of the dams showed no differences, whereas that of soft palatal mucosa in the 6% protein groups was increased. Effects of protein‐energy malnutrition on oral tissues and skin were therefore different between newborns and lactating dams. Furthermore, one part of oral mucosa is affected differently from other parts of mucosa and in both, degree of malnutrition has different influences.

Blood loss in periodontal flapsurgery using lidocaine 2% with either 1:50,000 or 1:100,000 epinephrine was compared. Ten patients from the graduate periodontal clinic were selected for the study. After the initial therapy which included oral hygiene instruction, scaling and root planing and occlusal adjustment, 20 surgical procedures were performed. In any one patient if 1:50,000 epinephrine was used on the posterior sextant of an arch, then 1:100,000 epinephrine was used on the contralateral side, with similar volumes being injected by the same method. Blood loss was determined by the cyanmethemoglobin comparison technique. Procedures using lidocaine 2% with 1:100,000 epinephrine generally had more than twice as much blood loss as those using lidocaine 2% with 1:50,000 epinephrine.

Organisms representative of soil, skinand fecal flora and of supragingival and subgingival flora were tested for inhibition of growth and killing by various salts (NaHCO3, NaCl, MgSO4). The antimicrobial activities of KHCO3, NaF, sodium lauryl sulfate (SLS) and chloramine T were also compared with that of NaHCO3, and the rate at which NaHCO3exerts its bactericidal effect was studied. Suspected periodontal pathogens were more susceptible to salts than were control non‐oral bacteria. Supragingival plaque organisms showed intermediate susceptibility. Periodontal pathogens were more susceptible to NaHCO3than to NaCl; NaHCO3and KHCO3showed similar activity against all strains tested. Accordingly, the antibacterial activity of NaHCO3is not simply an osmotic effect and is due to the bicarbonate ion.NaF, SLS and chloramine T had greater antimicrobial activity than NaHCO3. Supragingival bacteria required at least 6‐hour exposure to 1.0 m NaHCO3to produce 99% lethality (decrease colony‐forming units by 2 log10), whereas selected periodontal pathogens were killed more rapidly (30–120 minutes). The higher the concentration of bicarbonate, the faster the lethality. Morphologic examination by transmission electron microscopy of organisms exposed to bactericidal salt concentrations revealed marked fibrillar condensations within the cytoplasm and shrinkage of the cytoplasm from the outer membrane.For NaHCO3to be clinically effective, a high concentration must be introduced into the periodontal pocket and maintained there long enough to kill periodontal pathogens. Furthermore, NaHCO3must be reapplied often enough to prevent recolonization by these pathogens. An advantage of NaHCO3over NaF, SLS and other antimicrobial agents is its safety, availability and low cost.

Two extensive peridontalrecessive lesions in bilateral maxillary cuspid areas were treated surgically with free autogenous masticatory mucosal grafts. No attempt was made initially to achieve total root surface coverage. Evaluation of the sites during a 4‐year postoperative period demonstrated unusual degrees of “creeping attachment.” The maxillary left cuspid graft completely covered the root with 9 mm of gingival‐equivalent tissue while the maxillary right cuspid area displayed 90% (7 mm) root surface coverage.
