The purpose ofthis study was to compare the long term effectiveness of scaling and root planing alone to scaling and root planing followed by periodontal surgery. Seventeen subjects with moderate to advanced periodontitis receivedthoroughscaling and root planing as well as oral hygiene instruction. A modified Widman flap was then randomly performed for one‐half of each subject's dentition. Recall prophylaxis and oral hygiene reinforcement were administered for 4 years after completion of therapy. Shallow crevices (1–3 mm) subjected to either procedure tended to increase slightly in depth and exhibit a slight loss of attachment when compared to pretreatment measurements. Moderately deep pockets (4–6 mm) treated by either procedure were reduced and demonstrated a sustained gain or maintenance of attachment level. Pockets initially ≥7 mm exhibited the greatest reduction in depth and attachment gain. Gingivitis was reduced following either procedure for moderate and deep pockets. No difference in supragingival plaque retention was noted and both procedures reduced calculus. The results indicate that both procedures were effective in treating moderate to advanced periodontitis. However, the additional flap procedure tended to result in greater pocket reduction and attachment gain for deeper pockets.

The purpose ofthe present study was to determine the effect of intrasulcular toothbrushing on permeability of the sulcular epithelium. Twenty‐four dental students were divided into two groups of 12 each. On day 0, subjects in Group I began having the buccal aspects of the maxillary right first and second molars brushed for 30 seconds daily for 49 days by an examiner using an intrasulcular technique while subjects in Group II had the same teeth brushed with an extrasulcular technique. On day 49, clinical evaluation of inflammation was performed and biopsies were taken to evaluate gingival inflammation, sulcular epithelial keratinization and permeability of the sulcular epitheliumin vitrousing a microperfusion technique. Results indicate that subjects in both groups had attained equally high levels of gingival health. The intrasulcular group demonstrated a significantly higher degree of sulcular epithelial keratinization. However, no relationship was found between the degree of sulcular epithelial keratinization and sulcular epithelial permeability. Thus, the benefits derived from intrasulcular brushing and increasing sulcular epithelial keratinization are questionable.

An hypothesisis proposed relating the possible role of vitamin C deficiency as an etiologic factor contributing to periodontal disease in diabetes. The hypothesis is based upon the following: (1) transport of ascorbate across cell membranes may be impaired by glucose, but facilitated by insulin; (2) glucose utilization is significantly accelerated by sublethal concentrations of endotoxin; (3) endotoxin‐induced histamine sensitivity of tissue is enhanced by ascorbic deficiency; and (4) ascorbic acid deficiency alters mucosal barrier function. The interrelationship of these factors is discussed.

Individuals with gingival hyperplasia are most commonly seen by the periodontist and correction of their nutrient deficiency should be an integral aspect of treatment.

Recent studies havesupported an emphasis on gingival bleeding indices for detection of inflammation. The purposes of the present study were: (1) to evaluate the relationships between the time necessary for stimulated gingival bleeding to occur and both gingival fluid flow and Löe's gingival index; and (2) to develop a clinical index based on gingival bleeding time. In the first part of the study, the elapsed time for gingival bleeding to first occur after sulcular stimulation was correlated with the gingival index and fluid flow on 150 gingival units. A high correlation was found between bleeding time with both the gingival index and exudate flow. A bleeding time index based on a scattergram of the data obtained in part I of the study was then formulated and evaluated in a similar fashion on an additional 172 gingival units. Results indicate a high correlation between the bleeding time index and the other indices. It is concluded that the bleeding time index may be used by itself as an accurate and objective clinical tool to evaluate gingival inflammation.

Several studieshave demonstrated that nocturnal bruxism and related symptoms can be relieved through nocturnal electromyogram (EMG) feedback. The effects, however, are not always long lasting and the mechanism of suppression is not understood. It was the object of this study to examine more closely the manner in which nocturnal feedback works in suppressing bruxism. Chart recordings were made of nocturnal masseter EMG activity in five bruxist subjects. Baseline recordings were made in the subject's home followed by 10 or more nights of feedback treatment. Treatment involved sounding a 300 mW tone when EMG activity exceeded about 20 μV for more than 1 second. All subjects showed a decrease in the duration of bruxism. The decrease in bruxism was due to a reduction in the duration of bruxism episodes rather than a change in the number of episodes, i.e. rather than reducing the probability of an event starting, nocturnal feedback appears to simply suppress the activity once it is initiated. This provides little evidence of learning.

Presented is acase report of unusual painful granulomatous inflammation of the gingiva. The etiologic possibilities were explored without a definitive diagnosis being confirmed. Antibiotic therapy was unsuccessful. Empirical long term steroid therapy was successful in eliminating the disease and preventing recurrence. Diagnostic possibilities are discussed as well as treatment approaches to this type of patient.

摘要:
Acase is reportedof “desquamative gingivitis” in a 13‐year‐old male patient. On the basis of the clinical distribution of lesions, the histopathology, and direct and indirect immunofluorescent tests, the lesions were definitively diagnosed as cicatricial pemphigoid. This case is unusual because of the age of the patient, and further illustrates the value of immunologic tests in establishing a specific diagnosis of desquamative lesions.

摘要:
The cellularcomposition of crevicular washings from acute lymphoblastic leukemia (ALL) patients and normal children were examined microscopically. The crevicular washings of patients with ALL yielded a significantly lower viable cell count than the normal controls. The ALL patients had a preponderance of lymphoblasts in their oral differential cell count (78.6%) while polymorphonuclear leukocytes were the prevailing cell type (84%) in the normal controls. In both the ALL and normal patients the differential oral cell counts reflected the differential peripheral blood counts.
