The purpose of this studywas to characterize, using the scanning electron microscope, the nature of root surfaces denuded by (1) root planing alone or (2) root planing plus citric acid treatment. Six teeth were extracted from three squirrel monkeys, and the coronal half of the root surface was planed to remove attached periodontal ligament fibers and cementum. Citric acid, pH 1, was topically applied to the denuded surfaces of 3 teeth. The surface of only root‐planed specimens had an irregular surface which corresponded to a smear layer. Root surfaces that had received acid treatment after root planing exhibited funnel‐shaped orifices of dentinal tubules, and intertubular zones with a fibrillar, mat‐like morphology. The implications of the different root‐surface characteristics are discussed.

This experiment was carried outin order to study the rate of progression of experimentally produced periodontal tissue breakdown in teeth which prior to the placement of cotton floss ligatures either had a normal or a permanently increased tooth mobility. Six beagle dogs were used. Trauma from occlusion of the jiggling type was produced on test teeth using a model previously described. Four months later experimental periodontal tissue breakdown was induced around both control and test teeth by placing cotton floss ligatures around the necks of the teeth. The ligatures which were exchanged once every 4 weeks during a 4month period were replaced at the level of the gingival margin. Tooth mobility measurements and radiographic examinations were performed on Days 0, 30, 60. 90, 120, 160, 240 and 300. One dog was killed on Day 120 and the remaining 5 dogs on Day 300. Biopsies of the test and control teeth were fixed in formalin, decalcified, embedded in paraffin and mesiodistal sections cut with the microtome set at 4μm. The results demonstrated that the degree of periodontal breakdown, initiated and maintained by ligature placement and plaque accumulation, was similar around teeth with a wide periodontal ligament space and in teeth with a normal width of the periodontium. In other words, progression of the plaque‐associated lesions appeared to be unrelated to the width of the periodontal ligament space, i.e., to the degree of horizontal tooth mobility.

Studies of blastogenesis of lymphocytesin culture from juvenile periodontitis (JP) patients have been inconclusive. Experiments demonstrating differences in lymphocyte blastogenesis to preparations of putative periodontopathogens in JP and Phytohaemagglutinin (PHA) were performed. Variables in the blastogenesis assay system recently reported were controlled which included using a range of cell and activator concentrations, incubation times of 3, 5 and 7 days, conical‐bottomed microtest wells and chemical inhibitors during labeling of DNA which permits accurate assessment of lymphocyte blastogenesis. Using these modified culture conditions, stimulated lymphocytes of localized (LJP) and generalized (GJP) forms of JP patients did not differ significantly from stimulated lymphocytes of healthy subjects in counts incorporated, stimulation index, incubation time, cell concentration or activator dose required for maximal blastogenesis to bacterial preparations ofActinobacillus actinomycetemcomitans(Y‐4),Bacteroides gingivalis, Capnocytophaga ochraceus, Fusobacterium nucleatum, Streptococcus sanguisandTreponema denticola.Unstimulated lymphocyte cultures reflecting the autologous mixed lymphocyte response (AMLR) were increased for LJP compared to healthy subjects although not statistically significant. Unstimulated lymphocyte cultures of GJP were decreased compared to healthy subjects and LJP (P<0.05). These observations indicate that GJP patients may have abnormalities in T‐ and B‐lymphocyte regulatory mechanisms.

Patients with juvenile periodontitis (jp)were grouped into one of the two recognized forms of the disease: a localized form affecting incisors and first molars (LJP) and a generalized form affecting more than 14 teeth (GJP). The role of the neutrophil in the etiology and pathogenesis of LJP has recently been recognized. Experiments aimed at confirming previous information related to neutrophil Chemotaxis defects in LJP were performed. Additional experiments aimed at demonstrating significant differences in phagocytosis of bacterial spores and inducing spore germination were also completed. Age‐ and sex‐matched healthy subjects were used as “internal” controls (experiments run concomitantly). Peripheral blood neutrophils from 29 LJP, 24 GJP and 24 healthy subjects were assessed for Chemotaxis. This assay was performed in a Boyden diffusion chamber using 10−8mN‐formylmethionylleucylphenylalanine as chemoattractant. A chemotactic defect was defined being 2 standard deviations below the mean of healthy subjects. Twenty‐three of 29 LJP (79%) and 14 of 25 GJP (58%) had a neutrophil defect. Peripheral blood neutrophil phagocytosis assays were performedin vitrousing radiolabeled bacterial spores [45Ca]Bacillus cereus.Eighteen of 29 LJP (62%) and 7 of 24 GJP (29%) had a phagocytic defect. Neutrophil‐induced spore germination assessedin vitroshowed that 13 of 20 LJP (65%) and 3 of 8 GJP (38%) had this defect. These observations indicate that patients with JP have abnormalities in neutrophil functions of Chemotaxis, phagocytosis and spore germination.

摘要:
The purpose of this studywas to determine patient compliance with suggested maintenance schedules. All patients eligible for maintenance therapy in a private periodontal practice were included. The study covered a period of 8 years. All patients had chronic inflammatory periodontal disease and had been eligible for at least 1 year of maintenance therapy before the close of data collection. A total of 961 patients were studied; only 16% complied with recommended maintenance schedules. Erratic compliance was found in 49% of the patients and 34% never reported for any maintenance therapy.

摘要:
Periodontal inflammation caused by excessivedoses of vitamin A was studied in a patient who had taken 200,000 international units daily for more than 6 months. Clinical manifestations of the disease disappeared when the use of vitamin A was interrupted. The role of vitamin A as a factor in periodontal inflammation is discussed.

摘要:
This article presents a shortreview of the literature describing the relationship of periodontal disease and the maxillary sinus. This is followed by two case reports which demonstrate a close physical relationship between molar roots and the sinus and suggest that a sinusitis may develop from the extension of periodontal disease into the maxillary sinus. Further investigation of this hypothesis is necessary.

摘要:
Gingiva from a 3‐year‐old iranian boysuffering from Papillon‐Lefèvre syndrome was examined by light and electron microscopy. Deep pockets associated with predominantly Gram‐negative plaque were present. The gingival lesion was dominated by plasma cells, many of which were degenerate. Russell bodies were a prominent feature. No defect of epithelium was detected.

The prophy‐jet® was evaluatedfor its effectiveness in removing plaque and stain from teeth and on its effects on the marginal gingiva. Using a randomized split‐mouth design, half the mouth was treated with the Prophy‐Jet and the contralateral side with a rubber cup and prophylaxis paste. The results indicated that the Prophy‐Jet removes tooth plaque and stain as well as the rubber cup and prophylaxis paste method, and does so in less time. It was also found that there was some immediate soft tissue trauma following both treatment modalities, with a mean Trauma Index score of 0.23 ≠ 0.16 for the rubber cup and prophylaxis paste and 0.75 ≠ 0.37 for the Prophy‐Jet. Within 6 days, the trauma index scores were not significantly different. Similar results were found for marginal gingival redness and bleeding on probing. The Prophy‐Jet is an excellent alternative instrument for removal of tooth stain and dental plaque.
