Four human teeth and their facialgingival attachment were removeden block3 months after periodontal flap therapy and citric acid root conditioning and then prepared for histologic evaluation. At the time of periodontal surgery, and prior to citric acid application, the facial root surface was grooved at midline and citric acid was appliedonlyto one‐half of each root surface. Tissues were decalcified and histologically prepared as horizontally oriented, step serial sections from the level of the gingival margin to the level of the facial crest. The gingival margins demonstrated reformation of a crevice. The corium at this level showed a mild inflammatory infiltrate. Apical to this level, junctional epithelium adhered to both sides of the root surface and to the central groove. Further apically, supracrestal fiber groups were encountered. No new cementum was noted in these areas. Fiber attachment was present apical to this level. Since no new cementum was seen at this area of attachment, it may represent collagen attachment present prior to periodontal surgery. In the four specimens examined, no differences were seen in the soft tissue closures between the rootplaned citric acid‐treated root surfaces and root surfaces which received root planing alone.

摘要:
Assessment of bleeding upon probingis an integral part of a periodontal examination. Recent investigations have indicated that bleeding reflects histological, clinical and bacteriological alterations associated with periodontal disease. Furthermore, clinical and histologic data suggest that bleeding is an earlier sign of gingivitis than visual signs of inflammation (redness and swelling). The current literature referring to the role of bleeding upon probing in the diagnosis of periodontal disease is reviewed, and its limitations as a clinical parameter are discussed.

摘要:
Acomprehensive review of periodontaldressings is presented. The rationale for the application of dressings, their advantages and disadvantages are described. Tissue reactions to dressings and the therapeutic and adverse effects of antimicrobial agents used in dressings are discussed. The present status and value of a surgical dressing is critically assessed in view of recent studies which indicate that the routine use of dressings in postsurgical care may be either unnecessary or undesirable.

The primary purpose of the studywas to determine if, with an open flap approach, ultrasonic or hand instrumentation could remove all calculus from previously untreated teeth with moderate to severe loss of attachment. To be included in the study, teeth had to have at least 5 mm of attachment loss, be scheduled for extraction and be graded 2 or 3 on the Calculus Index of the Periodontal Disease Index system. Under local anesthesia, fullthickness, envelope‐type flaps were elevated apical to the crest of bone to allow access to the root surfaces which were then treated with hand or ultrasonic instrumentation until the roots felt hard and smooth to a Hartzell explorer. The teeth were then removed, rinsed, and lightly scrubbed to remove debris and a No. 14 wheel bur was used to place a groove along the coronal extent of the connective tissue attachment. A stereomicroscope at magnification × 4.6 was used to quantitate the percentage of residual calculus on 25 teeth treated by each method. Overall, hand‐scaled root surfaces demonstrated less residual calculus (5.78%) than ultrasonically treated surfaces (6.17%). Twenty teeth treated by each method were then prepared for histologic evaluation and evaluated under the light microscope at magnification × 100 for residual calculus and relative smoothness. Residual calculus was found on four ultrasonically and 12 hand‐treated teeth and was almost evenly distributed between anterior and posterior teeth for both methods. The finding of considerably less calculus on histologic than on stereomicroscopic examination may have been due to the loosening of deposits by instrumentation, especially ultrasonic vibration, and their subsequent release during histologic preparation.Among the hand‐scaled teeth, of 244 surfaces evaluated 138 (56.6%) were considered smooth, and 106 (43.4%) were graded rough. Among the ultrasonically scaled teeth, of 256 surfaces evaluated, 48 (18.8%) were smooth and 208 (81.2%) were rough.

摘要:
An unusual case of the ossifying fibromaof a 54‐year‐old woman is reported. Clinically the lesion appeared as an exophytic mass on the hard palate and apparently originated outside of bone. Histologically the resected tumor is well encapsulated and made up of a richly cellular fibroblastic stroma containing foci of mature bone with a predominantly lamellar structure. While the possibility exists that the lesion is reactive, it appears to be a true neoplastic growth and a purely soft tissue process arising from the periosteum of the palate. Thus, we proposed the termperiosteal ossifying fibromafor this distinct lesion in order to distinguish it from the common peripheral ossifying fibroma.

Cyclosporine is a relatively newimmunosuppressive agent which has been used successfully to prevent organ transplant rejection, to treat type‐I diabetes mellitus as well as several other autoimmune disorders. It is anticipated that the therapeutic use of cyclosporine will progressively increase in the future to include the treatment of numerous other disorders. As its clinical use broadens, more health professionals will need to become familiar with the beneficial effects as well as some of the undesirable side‐effects of cyclosporine therapy. Two side‐effects of the drug which are of interest to the periodontist are gingival hyperplasia and transient perioral hyperaesthesia. It is anticipated that the dental profession, particularly periodontists, will be called upon to help prevent, control or treat these side effects. The purpose of this paper is to familiarize the periodontist with cyclosporine and to discuss its anticipated impact on the practice of periodontics.

This paper explores, by means of clinical observation, toothbrush misuse and its relationship to gingival recession/clefting. The morphology of the lesions created is correlated with the vigor, duration, frequency and direction of the toothbrush abuse. In addition, histologic observations of biopsied tissue from different types of clefts and recessions are used as a basis for discussion of the possible pathogenesis of this type of lesion.

It has been reportedthat sensitive dentin is present in no less than one out of seven normal patients with a higher incidence among postperiodontal therapy patients. Numerous agents have been developed to relieve this sensitivity without any being universally acceptable. Two new dentifrices, Protect and Protect with fluoride were compared clinically with Sensodyne to determine the relative ability of these three products to relieve both thermal and tactile sensitivity in sensitive dentin. Double blind analyses were completed and results compared using appropriate statistical tests for each hypothesis tested. The three dentifrices relieved both tactile and thermal sensitivity, but there was no statistical difference at 8 weeks between the three agents. There was a difference in the time it took the dentifrices to achieve a statistically significant reduction in dentin sensitivity.
