The purpose ofthe present study was to investigate on a longitudinal basis, the effectiveness of a specific preventive dental program for outpatients who were taking phenytoin for seizure control. Plasma, mixed and parotid saliva were also monitored in relation to each other and gingival enlargement. The results confirmed that a preventive dental program consisting of frequent prophylaxis and oral hygiene reinforcement was effective in minimizing gingival enlargement associated with phenytoin therapy. However, a small but statistically significant increase in anterior gingival enlargement occurred within the first 6 months of phenytoin therapy. No further enlargement developed during the subsequent 9 months. Control subjects not receiving phenytoin demonstrated no change in the coronal position of the free gingival margin about anterior segments of the dentition. In the posterior segments, the control group exhibited statistically significant gingival recession which became evident at 12 months. Individuals taking phenytoin did not demonstrate any change in the coronal position of the free gingival margin within the posterior segments of the dentition. Combined with the dental prophylaxis, improvement in plaque control appeared to be consistent with the prevention of clinically significant gingival enlargement. Parotid and mixed salivary levels of phenytoin were approximately 10% that of plasma values. Both salivary levels rose when plasma levels increased as reflected by the relatively high coefficient of correlation between these parameters. Furthermore, neither plasma, mixed or parotid levels of phenytoin were correlated to the minimal degree of gingival enlargement which occurred in the anterior segments of individuals taking phenytoin.

Standard woundsof simple incision and partial excision of the dentogingival junction were made in miniature swine. The healing from the 1st to the 14th day were described clinically and histologically, and the standardization of the wounds permitted quantitative and statistical analysis. In wounds that are closely adapted to the tooth, the epithelium of the new dentogingival junction contributes primarily to early wound strength recovery. This functional achievement probably would not occur with thick clots or hematomas, or with any displacement of the gingiva away from the tooth.

Arandom double blindcrossover study of patients on the effects of tetracycline therapy over a 3‐month period revealed that there were no significant differences between the placebo group and tetracycltne‐treated groups in relation to (1) Gingival Index, (2) Debris Index and (3) Papillary Bleeding. A marked improvement in the Gingival Index occurred after 3 months of treatment in each group resulting from curettage and home care. Papillary bleeding was significantly reduced after 3 months of treatment in the tetracycline group and similar trends were observed in the placebo group. The Debris Index in both experimental and placebo groups showed no significant change after treatment for 3 months. The data suggest that tetracycline therapy does not appreciably alter either the Gingival Index, Debris Index, or the Papillary Bleeding Index over a 3‐month period.

摘要:
The effect ofLevamisole on gingivitis development was studied in three beagle dogs. Plaque accumulation was first allowed on one side of the jaws whereas the other side was subjected to careful tooth cleaning procedures. After 2 weeks' experimental gingivitis the dogs were given Levamisole and plaque accumulation was permitted on the other side of the jaws for another two weeks. The following parameters were investigated in the two gingivitis experiments: Plaque and Gingival Indices, Gingival Exudate measurements and histologic measurements. The biopsies were prepared both for light and electron microscopy. The clinical parameters did not show any differences between the two gingivitis experiments. The biopsies, however, demonstrated a larger area of infiltrated connective tissue and an increased number of leukocytes in the junctional epithelium in Levamisole‐treated compared to normal animals.

摘要:
The purpose ofthis report is to provide further information regarding oral changes in patients with chronic renal disease who are undergoing hemodialysis. Quantitative changes in salivary flow rate and alterations in the composition of stimulated and unstimulated saliva were studied along with the rate of formation of supragingival calculus. Results revealed a lower salivary flow rate and increased salivary urea concentration in the dialysis group. Dialysis patients also formed more heavy calculus.

Two hundred andthirty‐eight Caucasians were studied for ABO blood typing and for HLA antigens. The subjects were divided into normal, necrotizing ulcerative gingivitis, chronic gingivitis, periodontosis and Periodontitis groups. Results showed the chronic gingivitis group was significantly different in ABO grouping than the control group with the gingivitis subjects having a larger percentage of AB types and a smaller percentage of O types. The periodontosis group showed a trend toward more A and B blood groups and a smaller percentage of O groups than the controls. Compared to the normal group there was a significant reduction in HL‐A2 antigen frequency in the Periodontitis groups and a trend in reduced frequency in the periodontosis group.

This study indicatesthat a significantly greater percentage of periodontosis patients had high serum IgM and IgA levels compared to the control subjects plus a trend in the same direction for IgG. Paradoxically, 41% of the periodontosis subjects had no elevation of IgG, IgA or IgM.

Plaque formationand gingival inflammation were found in ODU plaque‐susceptible rats. A study was performed to determine the relationship between the presence of bradykinin in gingival tissue and the inflammatory capacity of rat plaque. Bradykinin content in gingival tissue and the bradykinin inactivating activity of dental plaque of 12‐month‐old ODU rats were measured. The presence of bradykinin was observed in the gingival tissue of the ODU rat and a bradykinin inactivating factor was found in the rat plaque. The factor was destroyed at 60°C for 30 minutes, at pH 1.5 or by treatment with perchloric acid. The optimum pH of the factor was approximately 8.0. Chelate reagents, such as O‐phenanthroline and EDTA4Na, strongly inhibited the bradykinin inactivating factor. The protease inhibitor, Trasylol, also inhibited this factor. The SH reagent,l‐cysteine, inhibited weakly and Zn++inhibited less. The investigation suggested that the rat plaque possesses the ability to inactivate bradykinin and plays an important role in modifying the inflammatory response caused by bradykinin in gingival tissue.
