Objective: The in vitro antifungal activity of lithium succinate has been evaluated against 46 different Malassezia furfur strains, isolated from patients suffering from dandruff, seborrheic dermatitis, and pityriasis versicolor. Methods: Minimum inhibitory concentrations of lithium succinate were measured by the agar dilution technique. Results: Lithium succinate was found to be able to inhibit growth of all investigated M. furfur isolates in vitro but at much higher concentrations than those which are normally used for susceptibility testing. Minimum inhibitory concentrations between 1,250 and 10,000 μg/ml were determined. When compared to the 8% lithium succinate ointment, the investigated concentrations are even far below this 8% value. Conclusions: It is suggested that the effect of lithium succinate ointment in seborrheic dermatitis therapy in vivo may be at least partly due to an antifungal activity of the agent

Background: The most frequent side effect of a long-term topical corticosteroid therapy is skin atrophy. To avoid or to reduce atrophy often an initial continuous application is followed by an intermittent maintenance therapy. Objective: In this study we measured the skin thickness before and after the two-phase application schedule with the superpotent topical steroid clobetasol propionate (CP). For 16 days CP samples were applied twice daily on the test areas of 12 volunteers (phase 1). Then CP was applied to the same skin areas in accordance with the following timing: every 5th day, every 7th day, every 10th day, every 14th day (phase 2). Phase 2 lasted for 45 days. During the entire period of the study the skin thickness was measured regularly by the skin compression and thickness method as described previously. Results: It could be seen that in phase 1 the skin became about 15% thinner. In phase 2 the steroid-induced skin thinning was approximately the same when CP was applied every 5th or 7th day. The skin thickness reached a more or less normal level when CP was applied every 10th day. After the 14th day a completely normal level was found. By measuring the skin thickness every day it was further shown that the skin thinning process lasted for 3 days when CP was applied once only. Conclusion: These results demonstrate that skin thinning must be expected by an intermittent maintenance therapy applied at relatively short intervals. The longer the intervals, the weaker the skin thinning. Moreover, these investigations showed that the skin thinning effect after a single CP application persists for nearly 3 days.

Pachydermodactyly is a rare form of superficial digital fibromatosis characterized by progressive asymptomatic thickening of the back and sides of the proximal interphalangeal joints of the fingers. Atrophia maculosa varioliformis cutis is an acquired dermal atrophy, localized on the cheeks. Only a few cases of each pathology have been published. We find it interesting to report the case of a patient with both conditions as these two connective-tissue diseases are very rare. The association is probably fortuitous.

A wide variety of cutaneous manifestations have been described in association with Crohn’s disease (CD). We describe a patient with a 2-year history of CD who developed both lichen planus (LP) and lichen nitidus (LN) in addition to erythema nodosum (EN) lesions. The clinical course of EN reflected the activity of the bowel disease, whereas LP and LN appeared to persist independently of the ongoing disease activity. Immunohistochemical studies of the infiltrates in these cutaneous and intestinal lesions showed that the majority of the infiltrates were T cells expressing T cell receptor (TCR)-αβ. Analyses of TCR V gene expression in these infiltrating T cells demonstrated a different pattern of Vβ expression that provides an explanation for the difference in the clinical courses of these lesions. A case of subcorneal pustular dermatosis (SPD) is reported in a 48-year-old man with a 2-year history of hyperthyroidism (Graves’ disease). SPD has been reported in association with IgA monoclonal gammopathy, but to our knowledge it has not been associated with hyperthyroidism. A 50-year-old woman presented with a skin tumor on her right calf. The tumor had been noticed 20 years previously and grew to more than 60 mm in diameter. The histological findings were characterized by numerous bland xanthomatous histiocytes and a few atypical giant cells with pyknotic nuclei, although mitotic figures were few. A 73-year-old woman with a malignant hidroacanthoma simplex on her right lower limb is reported. The lesion was immunohistochemically studied. Carcinoembryonic antigen and epithelial membrane antigen were found in the luminal border of ductal structures within the aggregations of neoplastic cells. We review the literature on this subject.

Anaplastic large-cell cutaneous lymphomas (ALCL) represent a heterogeneous group. Primitive cutaneous lymphomas are exceptional, sometimes with a deceptive clinical aspect, but must be considered as a distinct clinicopathologic entity. We report a case of a cutaneous ALCL, appearing 7 years after a homolateral Hodgkin ganglionic lymphoma and occurring on a chronic lymphoedema of the arm. A Stewart-Treves angiosarcoma was suggested owing to its location, vascular aspect and low histologic differentiation. Immunohistochemistry was helpful in making the diagnosis of ALCL. Southern blotting analysis revealed a rearrangement of the β-chain of the T-receptor gene, thus suggesting the T-cell origin of this lymphoma
