Data from our series of 70 patients support the hypothesis that linear IgA disease is a heterogeneous disease as regards clinical features, target antigens and immunogenetics. The clinical presentation varies as regards age of onset and severity of skin and mucosal involvement and scarring. The use of split skin and of the novel substrate cylindroma has identified 2 target antigens. The majority of patients have a target antigen associated with epidermal cells and possibly cylindroma hemidesmosomes. A minority have a dermal antigen which resembles collagen VII on cylindroma. There is an association with HLA-B8, -DR3, -Cw7 and with the linked rare tumour necrosis factor a allele, but this is not universal. These differences do not correlate with each other.

Background: In the last decade UV filters have been incorporated in a large number of cosmetic products. Objective: The purpose of our study was to evaluate the prevalence of sunscreen allergy in patients with a suspected photodermatitis. Methods: 108 patients were patch-tested with the Gruppo Italiano Ricerca Dermatiti da Contatto e Ambientali (GIRDCA) standard series and photopatch-tested with the photopatch series. Results: Allergy and/or photoallergy to sunscreening agents was diagnosed in 8 subjects (7.4%). Oxybenzone was positive in 4 patients, isoproyl dibenzoylmethane in 3 patients, p-aminobenzoic acid and 2-ethylhexyl-p-methoxycinnamate in 1 patient, respectively. Conclusion: Our results obtained during a 3-year period (January 1990 to June 1993) confirm that sunscreens are presently the commonest photoallergens.

We investigated antigen profiles of the sera from 2 cases of pemphigus vulgaris (PV) who subsequently developed pemphigus foliaceus (PF). Immunoblotting was carried out using normal human epidermal extracts as the antigen source. The sera during PV reacted exclusively with the 130-kD PV antigen and the sera during PF exclusively with the 150-kD antigen. Therefore, dermatologic, histologic and immunohistologic findings before and after this transition were clearly shown related to changes in serum reactivity with pemphigus antigens.

An acetone-soluble fraction of human dander provoked positive patch test reactions in 45 (38%) of 120 patients with atopic dermatitis. The positive patch tests were rare in normal controls and clinical controls. The positive patch test reactions histologically showed spongiosis and mononuclear cell migration in the epidermis, edema and mononuclear cell infiltrate in the upper dermis. Scratch tests with the acetone-soluble human dander fraction were positive in only 2 (2%) of the 120 patients examined. It was suggested that in a considerable number of patients with atopic dermatitis, an acetone-soluble fraction of human dander provokes a delayed-type skin reaction without inducing an immediate-type skin reaction.

Although IgA is present in the skin of patients with dermatitis herpetiformis (DH), the relationship of that IgA to the mucosal immune response is not known. The purpose of this study was to determine if IgA antibodies directed against tissue antigens, which are present in the serum of patients with DH, are also present in the gut secretions of these patients. Serum and gut secretions from 8 patients with DH and 5 control subjects were analyzed by indirect immunofluorescence for the presence of IgA antireticulin and antiendomysium antibodies. IgA antibodies against reticulin (3 of 8 patients) and endomysium (4 of 8 patients) were found in the serum of patients with DH. IgA antibodies were also present in the gut secretions of all of these patients, but not in subjects (patients with DH and controls) that did not have serum IgA antireticulin or antiendomysium antibodies. The presence of IgA antireticulin and antiendomysium in both the serum and gut secretions of DH patients suggests that these antibodies arise as a result of the mucosal immune response and that IgA of mucosal origin can persist in the serum of patients with DH.

Background: Accumulating evidence suggests that synthetic retinoids may be capable of affecting the differentiation and growth of nervous tissue in vivo and in vitro. On the other hand, adverse reactions concomitant with brainstem involvement definitely or probably related to oral retinoid therapy have been reported in a small number of patients. Objective: The purpose of the present study was to substantiate the possible effects of oral isotretinoin on the synaptic activity and propagation of action potentials along the nerve fibers. Methods: The auditory brainstem response of 33 patients with severe nodulocystic acne before and 3 weeks after the onset of oral isotretinoin administration was investigated using auditory evoked potentials. Results: The paired analysis of the response variables before and after treatment failed to reveal any statistically significant differences. However, a marked increase in latencies and interpeak latencies and a decrease in amplitudes for both ears were found in 3 patients after therapy. Conclusions: It seems reasonable to suggest that these subclinical changes may be due to an isotretinoin-induced synaptic malfunction or to a conduction defect in the auditory nerve fibers.

We reviewed the immunoreactivity of sera binding to the epidermal side of basement membrane split skin from 13 adults and 8 children with IgA alone, 9 adults with IgA and IgG and 7 adults with IgA and ocular pemphigoid. Immunoblots were done against previously described 45-, 97-, 180- and 230-kD antigens, and reactivity was confirmed by elution of antibody from nitrocellulose and binding to the basement membrane. Ten of 13 adults and 7 of 8 children reacted with the 97-kD antigen. Sera with both IgA and IgG reacted in varying patterns and on occasion with more than 1 antigen. All 7 patients with ocular cicatricial pemphigoid reacted uniquely with a 45-kD antigen.

Background: Molluscum contagiosum is generally a self-limiting benign skin disease that affects mostly children and young adults. Objective: The purpose of this multicenter, double-blind, placebo-controlled study was to compare the clinical efficacy and tolerance of 0.3% and 0.5% podophyllotoxin in a hydrophilic cream base to cure molluscum contagiosum in Asian males. Methods: Preselected patients (n = 150), age range 10–26 years (mean: 15.4), harboring 1,125 lesions (mean: 7.5), with size ranging from 2 to 8 mm in diameter (mean spot size 3.2 mm) and biopsy-proven diagnosis of molluscum contagiosum, were randomly allocated to three parallel groups. Twenty-four (16%) patients had atopic dermatitis. Patients self-administered placebo, 0.3% or 0.5% podophyllotoxin cream, twice daily for 3 consecutive days, and if total elimination was not achieved with one trial (6 topical applications), the same treatment was extended to 3 more weeks (24 topical applications in 4 weeks). The duration of the study was 12 weeks with 6 months (on monthly basis) follow-up. By the end of the treatment 80 patients (52, 92 and 16% patients in the 0.3%, 0.5% and the placebo groups, respectively) were evaluated as completely cured. During the treatment 92 patients (61.3%) did not complain of any allergic or localized adverse symptoms. Tolerable moderate to mild frequent side effects were pruritus (20.6%) and erythema (18%), with no dropouts. No recurrence was seen after 9 months of follow-up. Response to the trial medications appeared to be directly proportional to the concentration of podophyllotoxin (p < 0.001). Self-medication was well accepted by all the patients. It was concluded that the 0.5% podophyllotoxin cream preparation was more efficacious than the 0.3% incorporation (p < 0.001) and can be considered a safe, home-based first line of therapy to cure molluscum contagiosu

We studied in vivo binding sites of pemphigus antibodies and the expression of pemphigus and desmosomal antigens by keratinocytes in various culture periods. Both pemphigus vulgaris (PV) and pemphigus foliaceus (PF) antibodies mainly bound to the desmosomal areas of the lesional skin. Desmoglein and PV antigens co-localized on the cultured normal human keratinocytes in both monolayers and stratified areas. PF antigens, frequently together with involucrin and suprabasal keratins, were expressed by stratified keratinocytes. The Western blotting study demonstrated two different desmogleins with molecular sizes of 130 and 150 kD. The 130-kD desmoglein bore PV antigenic epitopes. The 150-kD band increased in volume as cultured keratinocytes stratified. PV antigen expression on the 130-kD desmoglein precedes PF antigen formation occurring in keratinocyte differentiation.
