Acute adrenal insufficiency is a rare complication of the primary antiphospholipid syndrome. We report a case of melanoderma of rapid onset revealing an addisonian crisis associated with primary antiphospholipid syndrome, which was associated with life-threatening hypercalcemia. Melanoderma does not belong to the clinical presentation of acute adrenal insufficiency. Its very rapid onset in our patient is probably due to her very high level of serum ACTH, in relation with her recent abortion.

The etiology of prurigo pigmentosa still remains unknown. We present a 16-year-old female patient with ketonemia caused by diabetes mellitus. The eruption subsided when blood glucose and total ketone levels were controlled by subcutaneous insulin injection. We propose that ketonemia caused by diabetes mellitus may play a role in the pathogenesis of prurigo pigmentosa.

A 51-year-old man with aquired immunodeficiency syndrome presented with a perianal ulcer. Histologically, there were numerous cytomegalic inclusions within endothelial cells and stromal fibroblasts. Cytomegalovirus within infected cells was demonstrated by immunohistochemistry and in situ hybridization. No cytopathic changes typical of Herpes simplex were detected. The ulcer resolved completely after a treatment with ganciclovir.

A 39-year-old HIV-infected man had manifested a typical varicella successfully treated with intravenous acyclovir. Despite oral acyclovir, he developed 10 days later a widespread eruption of pinpoint-sized erythematous papular lesions. Histologic examination and viral culture showed a persistent varicella-zoster virus (VZV) infection. Intravenous acyclovir and foscarnet were both efficient. However, each withdrawal of intravenously administered treatment resulted in a rapid relapse. Among the atypical forms of chronic varicella, this eruption appears to be unique. As in our case, chronic VZV infection appears often to be a difficult therapeutic challenge.
