The chemotactic response of peripheral blood polymorphonuclear leukocytes (PMNs) to N-formyl-methionyl-leucyl-phenylalanine was determined in four groups of persons (I) 19 patients with pustulosis palmoplantaris (PPP) and skeletal disorder (pustulotic arthro-osteitis); (II) 15 patients with a similar anterior chest wall involvement, but no PPP; (III) 9 patients with PPP, but without skeletal involvement, and (IV) 69 healthy adults (controls). The chemotactic activity of PMNs was found to be significantly increased in groups I–III, and patients with a similar osteoarthropathy, but no PPP compared with the controls. Furthermore the patients with pustulotic arthrosteitis had enhanced chemotactic activity compared with the patients with PPP only. This indicates that both skeletal involvement and PPP are somehow related to the function of PMNs. Skin biopsies from 49 patients with 21 different dermatoses and biopsies of normal skin were investigated with regard to the reactivity of keratinocytes with the monoclonal antimonocytic antibody anti-Leu M2 using the avidin-biotin-peroxidase complex method. In all excisates, a cytoplasmic reaction of keratinocytes was seen which was regularly weaker in the basal layers of the epidermis than in the upper layers. Marked differences in the labelling intensity were shown between the individual dermatoses. Keratinocytes have now been shown to react with several monoclonal antibodies directed against monocytic antigens; therefore the functional significance of this should be discussed.

The presence of HLA-associated genetic factors in patients with multiple skin carcinomas was investigated. 43 patients affected by multiple basal and squamous cell carcinomas and 220 healthy age-matched controls were typed for 72 HLA-A, B, C, DR antigens. A negative association of B-17 (mostly B-58) and a positive association of Cw-3 and DR-1 was observed in patients with multiple basal cell carcinomas; a similar negative association with B-58 and positive association with DR-1 was seen in patients with squamous cell carcinomas. The results of this study suggest the presence of resistance factors to the development of skin cancers associated with B-17 (B-58) antigen.

Acitretin (Ro 10-1670, etretin) is the free acid derivative of etretinate. This compound possesses a much shorter half-life of elimination than etretinate and was therefore proposed for clinical development. A total of 635 patients with various dermatoses were studied in 12 main clinical trials performed in Europe and in the United States. It was shown that acitretin is effective in the treatment of psoriasis and other diseases of keratinization. Doses between 25 and 35 mg per day are recommended to initiate treatment since marked improvement or clearing was obtained in the majority of patients within this range. Hypervitaminosis A signs and symptoms have been observed in patients treated with acitretin. It is concluded that the efficacy and safety profile of acitretin resembles that of etretinate.

Ro 13-7410 was given to 29 patients (a total of 38 treatment courses) for 7.5 weeks (range 4–23). This compound is one of the most potent retinoids ever synthetized and has the highest affinity to human skin cellular retinoic-acid-binding protein. Here we report on a 60-year-old female patient suffering from multiple erosive inflammatory lesions of the vagina for 2 years. Subepidermal blister in the histological examination, deposits of IgG and C3 in the direct immunofluorescence studies and the basement membrane zone antibody titers between 80 and 320 have confirmed the diagnosis of bullous pemphigoid. The lesions responded to treatment with prednisolone and azathioprine fairly well. There have been no signs of scarring till now.

We report a case of familial porphyria cutanea tarda (FPCT) in a 7-year-old girl. The condition was exacerbated by low dose of hydroxychloroquin, but controled by venesection. The favorable prognosis of FPCT should be further established by long-term prospective studies.

Hidrotic ectodermal dysplasia (HED) was observed in a 49-year-old Japanese man. His clinical signs included alopecia, dystrophic nails and palmoplantar keratoderma, but his teeth, facial appearance and sweating were normal. In his family, 9 members in 5 generations were affected, suggesting that the disorder has an autosomal dominant mode of inheritance. Biopsy specimens of the hyperkeratotic lesions of the palm and sole were studied with an electron microscope. The most prominent feature was the increase of the number of desmosomal discs in the thickened stratum corneum, suggesting that hyperkeratosis observed in HED is due to the delayed desquamation of the stratum corneum.

Neisseria gonorrhoeae was cultured from the cervix, pharynx and blood stream of an 18-year-old woman who had a disseminated gonococcal infection. Two different phenotypes of N.gonorrhoeae, distinguished on the basis of their protein 1 serovar, whole-cell protein profile, lectin-type and antibiotic susceptibility, were isolated from both the cervix and pharynx, whereas only one of these phenotypes was isolated from the blood. This finding stresses the importance of blood cultures as a prelude to determining appropriate antimicrobial therapy in cases of disseminated gonococcal infection.
