Skin lesions from 6 patients with lichen planus were studied for the presence of T cells and T cell subsets using monoclonal antibodies and indirect immunoperoxidase technique. Small numbers of Leu-2a-reactive suppressor-cytotoxic cells were present early in the basal cell layer in 2 patients with recent lesions. The analysis of T cell subsets revealed the predominance of anti-pan-T-cell (Leu-1)- and Leu-3a-reactive helper-inducer cells in 4 patients with older active lichen planus lesions. Significant numbers of suppressor-cytotoxic cells were observed in the papillary dermis and within the epidermis associated with hydropic degeneration of the basal cell layer. Activated T lymphocytes with focal acid phosphatase activity, together with activated histiocytes-macrophages with strong diffuse activity of acid phosphatase and non-specific esterase, were identified in the dermis and within the epidermis. These findings suggest that a cytotoxic immune process directed against the basal cell layer of the epidermis is the dominant pathogenic event in lichen planus.

Frozen skin biopsies from 3 acute and 3 chronic lesions of atopic dermatitis were examined by means of various ortho-monoclonal antibodies (OKT3,4 ] = C4, OKT6, OKT8; OKIa and OKM1) in an indirect immunofluorescence technique. The results show that both T helper (T4) and T suppressor cytotoxic (T8) lymphocytes are present in the inflammatory infiltration which predominantly contained la mononuclear cells. Despite the absence of double staining procedure, the observations suggest that T4 cells predominate over T8 cells in the acute lesions whereas the reverse tendency is noted in the chronic lesions. T8 cells are found mainly in the superficial dermis or even in the epidermis; by contrast, T4 cells tend to be more deeply located in the dermis. The lesions are characterized by a dramatic increase in the number of Langerhans cells (OKT6, OKIa+) that are found not only in the epidermis but also in the dermis.

The presence of autoantibodies to intercellular substance of squamous epithelium was studied in the sera of 46 patients suffering from pemphigus vulgaris. This study suggests that in the majority of patients suffering from the severe form of the disease the auto-antibody titer can be used as a guide to prednisone therapy. However, in patients with a mild or moderate form, no such correlation is established.

Alkaline phosphatase (AP) activity on cryostat sections of the skin of 38 cases of non-Hodgkin’s lymphomas of the B cell type and of 10 cases of lymphadenosis benigna cutis have been studied. Membrane-bound AP activity has been found in 8 out of 28 cases of low-grade malignant lymphomas of the B cell type and in 2 out of 10 cases of lymphadenosis benigna cutis. This is a known phenomenon in the latter. In the litterature of non-Hodgkin’s lymphoma AP activity is related to the intermediate type of lymphocytic lymphoma. Further studies are needed to demonstrate that the AP activity correlates with a specific cell type in the B lymphoid cell differentiat

The antiallergic drug oxatomide was evaluated in a double-blind placebo-controlled study in 35 patients with pruritus senilis. The trial was run in the wintertime, and the patients were orally given either 30 mg oxatomide b.i.d. (n = 19) or a placebo (n = 16) for 2 months. Complete suppression or marked improvement of the complaints was experienced by 79% of the patients given oxatomide and by 31% of the control patients. Oxatomide was superior to the placebo in reducing both the duration and the severity of itching. The need of additional topical medication was higher in the placebo group. Somnolence and cramps were each reported by 1 oxatomide-treated patient.

A 2-year-old girl having a bald nodule on the scalp is reported. The tumor was circular, hemispherically elevated and had a dull reddish color. Roentgenologic studies revealed a defect of the underlying skull bone, and spina bifída occulta on the first and second sacral vertebra were found. Craniotomy was carried out. The tumor extended into the falx cerebri and reached to the tentorium cerebelli, and was not connected with the cerebrum. Histological examination showed insular brain tissue containing glial cells in the lower dermis. Around this tissue, there seemed to be vascular leptomeninges with numerous melanocytes and thickened fibrous elements, probably dura. From these findings, this case was diagnosed as a variant of encephalomeningocele, and as melanocytes physiologically exist on the leptomeninges, melanocytes in an encephalomeningocele may diagnostically identify the leptomeninges

Congenital fistula of the lower lip was observed in 3 members of a family. There was no evidence of associated congenital malformations. In the father, we performed fistulography with radiopaque substance, showing that the fistular tract extended beyond the orbicularis oris muscle, a feature with no previous report in the literature.

A papillomatous tumor developed on the skin from the inguinal region to the middle of the right thigh. The histological findings showed tumor cells with papillary formation surrounded with fibrous stroma, and a diagnosis of metastatic adenocarcinoma of the lower gastrointestinal tract was ascertained. Autopsy findings revealed that the cutaneous metastasis was of urinary bladder origin. This paper reports the extreme rarity of metastasis from adenocarcinoma of the urinary bladder to the skin and the peculiar appearance of the cutaneous metastatic lesion.
