A case of localized bullous pemphigoid in a 79-year-old woman is reported. The symptoms developed 5 months following radiation treatment of breast cancer. Direct immunofluorescence investigations showed IgG and C3 deposition in the basement membrane zone (BMZ) of lesional skin. Serum BMZ antibodies (IgG, IgM) and C3 deposition were demonstrated. Light microscopic investigation revealed subepidermal blister formation. A cellular infiltrate consisted predominantly of lymphocytes and eosinophils in the upper dermis. Junctional blister formation, located within the lamina Iucida of the basement membrane, was confirmed by electron microscopy. Differential diagnoses and various possible etiologies are discussed.

Glycosaminoglycan (GAG) fractions isolated from the urine of scleroderma patients were compared with GAGs from control subjects for their ability to induce fibrotic changes in the skin of mice after intraperitoneal injection. Contrary to previous reports, however, no fibrotic changes were detectable, either visibly or in terms of skin tissue thickness. The ability to obtain consistently authentic biologically active factors from this source is therefore questioned.

8 Korean children exhibited well-defined depressed lesions of the skin accompanied by slight redness of the surrounding area. First these lesions appeared in the inguinal area and then spread peripherally to the navel region and the ipsilateral vulval area. In these 8 cases, 7 occurred in girls and 1 in a boy. The age of onset ranged from 2 to 9 years. Biopsy specimens taken from the well-depressed area showed a decrease in or absence of, subcutaneous fat and a mild inflammatory infiltrate of lymphocytes and histiocytes in the lower dermis and subcutis. These 8 cases are the first, to our knowledge, to be reported outside Japan and considering the age of onset of the dermatosis it is not confined to infants. Therefore, we would like to describe this entity under the title of lipodystrophia centrifugalis abdominalis juvenilis instead of lipodystrophia centrifugalis abdominalis infantilis.

A patient with moderately extensive psoriasis developed acute gouty arthritis while receiving topical corticosteroid and tar therapy. The serum uric acid levels were normal before, during, and after the acute gouty episode. Patients predisposed to gout by virtue of an appropriate associated condition should be evaluated for gout even in the presence of normal serum uric acid.

22 adult females with therapy-resistant acne vulgaris were treated for 12 months with Diane®1, a drug containing cyproterone acetate and ethinylestradiol. Treatment was withdrawn in 7 patients because of side-effects or lack of effect. In the remaining 15 patients, the treatment had extremely promising results, from 70 to 90% improvement of the acne. In a remarkably high number of patients, the androgen production, measured by the urinary excretion of fractional 17-ketosteroids, was elevated. None of these patients had signs of endocrinological diseases, in particular no cases of hirsutism of Stein-Leventhal syndrome were found. The current concept of the course of acne is that the conversion in the skin of testosterone to dehydrotestosterone is increased. The finding of an elevated urinary excretion of androgenic substances in this group of acne patients indicates that the pathogenesis is far more complicated
