28 serum and 10 blister fluid specimens obtained from 28 pemphigus vulgaris (PV) patients were assayed for immune complexes using the polyethylene glycol (PEG) assay. 11% of sera and 30% of the blister fluids had elevated levels of immune complexes. Anti-intercellular cement substance (ICS) antibody could not be detected in PEG precipitates, but was present in the supernatants from the serum. However, anti-ICS antibody was found in 70% of the precipitated complexes from the blister fluid.

In 49 healthy test persons we measured various endocrine parameters (ACTH, cortisol, prolactin, triiodothyronine, thyroxine, TSH) prior to and after whole-body irradiation with UV light of varying quality and quantity. 60 min after sunlamp and SUP® exposure a marked decrease in the average cortisol plasma level appears which is less than the daily circadian variation. After irradiation with a lamp that mainly emits UVA rays these reactions fail to appear. After sunlamp and SUP irradiation also the concentrations of the thyroid hormones triiodothyronine and thyroxine show a decrease of 22.1 and 13.7%, respectively, which is, however, statistically significant for triiodothyronine only. ACTH and TSH values remain unchanged. According to these results we can conclude that depending on the quality of rays defined endocrinological reactions are caused. Cultured guinea pig epidermal cells were treated with vitamin A acid (VA; 2, 5, 10 μg/ml) and controlled by autoradiography and histochemical methods. Cells attached to the bottom of the plates were counted and their number correlated with the number of cells sloughed off into the medium (A/D ratio). VA reduced the size of multilayered cell colonies, favoured cell spreading and accelerated confluence. It altered the mode of shedding since the cells sloughed off into the medium as isolated cells rather than cell complexes, as in untreated controls. It increased the A/D ratio in early cell cultures and decreased the A/D ratio in late cell cultures. The higher the concentration of VA, the earlier and more prominent was the decrease of the A/D ratio. Low concentration of VA (2 μg/ml) given once at seeding favoured the formation of keratohyalin and cornifíed envelopes whereas high concentrations (5, 10μg/ml) given continuously prevented their formation and caused mucoid vacuolar degeneration of single cells. VA inhibited complete keratinization in all cases, but appeared to allow pre-keratinization, particularly at low concentrations. The results indicate that VA controls epidermal homeostasis in vitro by its influence on both proliferation and shedding. 35 patients attending a hospital dialysis unit were questioned for the presence and severity of pruritus. Attention was given to skin anomalies and loss of hair. In all patients the serum vitamin A level was determined. Although all of them showed an increased serum vitamin A level, no correlation could be found between the vitamin A content and the presence of pruritus.

A case of multiple trichilemmal cysts is presented. After trauma, a large tumor developed on one of the cysts. The histologic features of this tumor were similar to those of proliferating trichilemmal cyst, but some of the tumor cells showed definite nuclear atypicality. The malignant potential of this tumor was confirmed by later development of metastases to the regional lymph nodes.

A 57-year-old woman who had sensorimotor polyneuropathy with dysgammaglobulinemia showed pea-sized tense blisters on the fingers. Histopathological features were intraepidermal multilocular bullae and degeneration of capillaries of the papillary dermis with decreased nerve components of the dermis. Clinicopathological findings indicate this case to be trophoneurotic bullae.

A total of 40 patients with palmoplantar pustulosis (PPP) were initially treated with oral etretinate (Tigason) in an open trial with a maximum treatment period of 16 weeks. Remission, with only slight residual changes in some cases, was achieved in 26 patients (65%) who were randomized to either a low dose of Tigason or placebo. In the Tigason group, 7 of 11 patients were still in remission after 6 months while in the placebo group, remission persisted in 4 of the 10 patients who stayed in the study throughout the whole 6 months’ period. Alopecia led to stopping the treatment in 6 patients and desquamation of the healthy skin in 2 patients. Other side-effects were only mild. As a conclusion, Tigason shows a beneficial effect in the majority of patients with PPP and is better than placebo in preventing relapse of the disease but intolerable side-effects restrict its use in many patient
