Dermatophytes are classified according to two systems. The first, or parasitic classification of Sabouraud, rests on the morphology of the dermatophytes on the hair in vivo (Achorion, Trichophyton endothrix or ectothrix microides and megaspores, Microsporum) or on the absence of growth on the hair in vivo (Epidermophyton). In the second type they are classified according to their saprophytic morphology. There are many classifications of this type based on the asexual state of the dermatophytes, e.g. the classification of Emmons (Trichophyton, Microsporum, Epidermophyton) or the classification of Vanbreuseghem and DeVroey (Microides, Trichophyton, Microsporum, Epidermophyton, Keratinomyces). As the existence of a sexual state of the dermatophytes has been demonstrated, the dermatophytes are also classified in two perfect genera: Arthroderma and Nannizzia. In this paper it is assumed that both classifications have to be used because they are not correlated.

18 of 49 pediatric renal transplant recipients, all of whom were on continual steroid and immunosuppressive treatment, were found to have warts. Although all 18 patients received treatment for this problem, only 5 had the warts successfully eradicated. The frequency of warts among transplant patients is three times that of the general childhood population and twice that of a group of nephrotic pediatric patients who received immunosuppressive therapy. We propose that drug-induced suppression of cell-mediated, and to a lesser extent, humoral immunity, may predispose people to develop warts.

Quantitative techniques of sensitization to 2,4-dinitrochlorobenzene (DNCB) was used to determine in psoriasis the intensity and frequency of allergic reactions to DNCB following primary challenge with 2,000 μg allergen and secondary challenge with decreasing doses of DNCB. 56 patients with psoriasis and 23 healthy volunteers were examined. Frequency of positive reactions to DNCB was similar in both groups, since all normal controls were sensitized, whereas only 8 of 56 psoriasis cases failed to develop delayed hypersensitivity to DNCB. However, the intensity of acquired contact allergy was significantly diminished in psoriasis in comparison with controls. The patients with stationary skin lesions resembled the normal population in the intensity of reaction to DNCB. Decreased intensity of DNCB sensitization seemed to be related to the activity of the disease, but not correlated with the extent of the lesions. A relationship was found between reduced reactivity to DNCB and decrease in E rosette-forming lymphocytes. The data suggest that the impaired function of T lymphocytes in active psoriasis could be responsible for both, defective recognition of contact antigens, such as DNCB, and the alteration of secondary response to DNCB

Intradermal injections of triamcinolone acetonide have been used in the treatment of psoriatic onychodystrophies in seven patients. The therapeutic response initially is good but relapses appear inevitable. One patient developed rather persistent periungual hypopigmentation, thereby limiting the routine recommendation of this form of treatment.

Total skin irradiation with fast electrons (4 MeV, 3,500 rad) was applied in 10 cases of histologically proven mycosis fungoides without any signs of internal dissemination. The technique is described. Complete disappearance of both lesions and symptoms occurred in all patients after treatment. However, half of the patients relapsed after the initial irradiation and were subsequently treated with a booster dose of 1,000 rad 4 MeV electrons or with topical nitrogen mustard. Irradiation with fast electrons seems to be of great importance in the treatment of mycosis fungoides.

Regarding the rare phenomenon of diffuse melanosis of the skin secondary to widespread melanoma, the possibility of a circumscribed melanosis in melanoma patients is hypothetically discussed on the basis of a personal observation. Microscopically, such a circumscribed melanosis appears as clusters of melanin-laden macrophages.

Four patients with ashy dermatosis are described. Their ages varied from 8 to 12 years. Three had typical widespread macular eruptions and one had a linear lesion not described before. The follow-up investigation revealed that the eruption disappeared within 2 years in three of the patients. One patient could not be followed. The duration of the disease and the linearity of one lesion resembled lichen ruber planus.

Two Nigerian patients with dermatofibrosarcoma protuberans of the face are described. Both patients developed histologically typical tumours of unusual size. The condition is discussed in relation to previous reports of this unusual location and the frequency of dermatofibrosarcoma protuberans in Africa.
