Squaric acid dibutylester (SADBE), a potent contact allergen, was tested for mutagenicity in the bacterial plate incorporation assay (Ames test), in the presence and absence of mammalian microsomes. In contrast to dinitrochlorobenzene which is mutagenic in this test, SADBE was found not to be mutagenic. In 53 patients with extensive or total alopecia areata, SADBE dissolved in acetone was applied weekly to one side of the head, the other side serving as control. In 46 patients (87%), hair regrew either exclusively on the treated side, or considerably faster and denser on this side. In some patients, continuous treatment failed to maintain the response. Persistent response was observed in 37 patients (70%). These results are essentially the same as those obtained with DNCB. Therefore, contact allergy is proposed as a therapeutic concept for alopecia areata.

41 patients with severe alopecia areata were treated with either local or oral 8-methoxypsoralen and UVA irradiations. In 26 cases (73%) a good to excellent result was obtained. The number of irradiations required for regrowth varied but 20–40 treatments were sufficient in most cases with a positive response. Patients with circumscribed or ophiasic alopecia responded better than patients with total or universal alopecia, atopy or a family history of alopecia did not seem to influence the outcome of the treatment. The age of onset of the disease seemed to be of importance as well as the duration of the treated attack. Patients over 20 years at the first attack of alopecia showed a better response than patients with an earlier onset of the disease. 8 children with lamellar ichthyosis, 1 with epidermolytic hyperkeratosis and 5 with symmetrical progressive erythrokeratoderma were treated with a new aromatic retinoid (Ro 10-9359). Clinical improvement was dramatic. The children acquired an appearance never obtained before with other managements. The treatment had to be maintained to prevent recurrence. The tolerance to the drug was good. The side-effects were minimal and tended to disappear after several months of treatment. Our results suggest that because of its efficacy, good tolerance and easy administration, the oral retinoid Ro 10-9359 is at present the treatment of choice for the great ichthyotic disorders of children.

40 leg ulcers were treated for 6 weeks by application twice a day of a 20% benzoyl peroxide lotion. Each ulcer was regularly measured and its bacterial flora was determined. Therapeutic results were very satisfactory: the average healing rate of the ulcers was 62.8% of the initial wound area after 4 weeks of treatment and 76.5% after 6 weeks of treatment. The bacterial flora of the ulcers changed during treatment, streptococci and staphylococci disappeared. Pseudomonas aeruginosa became abundant.

A 3-year-old girl in poor general condition was admitted with a diagnosis of cryptococcosis and treated with 5-fluorocytosine. This case is characterized by its occurrence at a young age with no underlying disease, with involvement of the central nervous system, lung, skin, liver, spleen and lymph nodes, with typical cutaneous papules and nodules with ulceration and crusting, gelatinous appearance on biopsy of the skin, severe peripheral eosinophilia, and a remarkable response to 5-fluorocytosine therapy monitored by follow-up skin biopsies.

A case of anetoderma is described in which progressive protrusion of the anterior teeth was a concomitant finding. A possible interrelationship between these two conditions is suggested, based upon the anatomy and physiology of the oxytalan fibers of the periodontal ligament.

2 patients with typical clinical and histological pictures of hyperkeratosis lenticularis perstans were described. Both patients showed abnormalities of the mucosal surface. The ultrastructural findings, both in the cutaneous and in the mucosal lesions, provide strong evidence that the mucosal abnormalities are a real component of hyperkeratosis lenticularis perstans.

Unilateral dermatomal cavernous hemangiomatosis (UDCH) is a nosologic entity characterized by (1) multiple hemangiomata; (2) location in lower dermis and subcutaneous tissue; (3) unilateral dermatomal distribution; (4) lack of gastrointestinal lesions; (5) lack of enchondromata; (6) benign course; (7) absence of family history; (8) onset either acquired or congenital, and (9) changes in size possibly hormonal in nature. UDCH must be distinguished from other disorders with multiple hemangiomata which convey adverse prognoses.

The association between acanthosis nigricans (malignant type) and internal malignant tumour is well established. In the majority of cases the malignant tumour is localized to the gastrointestinal tract or the thoracic cavity. The malignant tumour is believed to be an adenocarcinoma in most cases. A case of malignant acanthosis nigricans associated with a follicular adenocarcinoma of the thyroid gland in a 31-year-old white male with congenital hypothyroidism and eruptive xanthomas due to hyperlipoproteinemia of type V (Frederickson) is reported.
