In patients, autografts of normal skin (total skin including hair follicles) were performed in vitiliginous patches, then followed by photochemotherapy. This association induced pigmentation from the edges of the achromic areas and/or from the grafts. This procedure is proposed for the treatment of recalcitrant and small plaques of vitiligo.

The condition wherein multiple satellite angiomatous lesions develop around the scar of a recently treated pyogenic granuloma is a rare occurrence. To date 30 such cases have been reported. One additional case is described and ultrastructural examination is carried out. The pathogenesis of this entity is considered in relationship to an angiogenic factor elaborated by the primary lesion.

Biopsy material of 172 patients with malignant cutaneous lymphomas and pseudolymphomas was examined using the immunoperoxidase technique. It can be stated that this technique appears to be a valuable aid for the classification and differential diagnosis of cutaneous lymphoproliferative and histiocytic disorders. In addition, the method provides a better understanding of the etiopathology of certain entities.

2 middle-aged men are described with slowly progressive papular and nodular lesions, limited to the skin of the back. Clinically and histologically both resembled Crosti’s reticulosis (‘réticulo-histiocytome du dos de I’adulte’). It was determined by electron microscopic, enzyme cytochemical and immunological studies that the ‘histiocyte’ component, which was derived from T lymphocytes, consisted of blast-like cells with characteristic multilobated nuclei with nucleoli in marginal position. For this type of lymphoma, which may have been described previously as lymphocytoma cutis or reticulum cell sarcoma, the term ‘multilobated cutaneous T cell lymphoma’ is proposed. The bioavailabilities and activities of three amcinonide preparations and Betnovate cream were assessed using three multiple-dosage regimen vasoconstrictor assays in 10 volunteers. Applications were made once daily, twice daily and every alternate day with an initial three times daily loading dose applied on the first day only. Blanching responses first increased and then decreased due to tachyphylaxis. It is proposed that clinically the most advantageous dosage regimen is a once daily application with no loading dose.

Follow-up examinations of 84 psoriatics after 4 or 5 years of PUVA therapy revealed that keratoses or skin tumours developed in 8 cases. 6 of these patients showed typical arsenic keratoses and 5 of them had arsenic intake decades ago. PUVA therapy does not modify the clinical behaviour of arsenic carcinogenesis.

An adult Black South African man with forme fruste pachydermoperiostosis is described. The patient had cutis verticis gyrata, hyperhidrosis, sebaceous plugging and an acneform rash. Basal and dynamic endocrine studies revealed that the hypothalamic, pituitary, thyroid, adrenal, gonadal axes and growth hormone secretion were all normal, but urinary excretion of 17-hydroxy- and corticosteroids, and the prolactin response to thyrotropin-releasing hormone were depressed. Urinary levels of asparagine were markedly elevated and serum levels were subnormal indicating a possible renal leak of this amino acid; glutamic acid and ornithine were present in smaller quantities in the urine. All other tests of proximal and distal renal tubular function were normal. There is no evidence from our studies to suggest a hormonal basis for pachydermoperiostosis. The amino aciduria may be a chance association of, or an (additional) occasional manifestation of the syndrome.

A 25-year-old man with solar urticaria is described. The action spectrum ranged from 400 to 500 nm. An inhibition spectrum was found to be in the visible light range above 660 nm. In 192 patients with psoriasis an increased frequency of the antigens HLA-B13 (19.79%, X2 = 32.79, p < 0.001), HLA-B17 (24.47%, X2 = 64,84, p < 0.001), and HLA-B27 (19.27%, X2= 11.07, p < 0.001) in comparison with the control group of 1,085 clinically healthy persons, was established. After 4 weeks of thalassotherapy 3 groups of patients could be distinguished: (1) those with full clinical remission – 50 (26.8%), (2) those with great or moderate improvement – 116 (60.4%), and (3) those with no improvement – 26 patients (13.6%). In the first two groups there is a statistically significant association with the HLA antigens B13 and B17. In the third group (no improvement) there is no such association with HLA-B13 (p < 0.01). Consequently, patients in whom psoriasis is associated with HLA-B13 were better influenced by the thalassotherapy. 50 patients with drug-resistant and/or extensive superficial mycoses and positive cultures for dermatophytes, were randomly assigned to treatment either with 200 mg ketoconazole (26 patients, mean age 31 years) or 500 mg griseofulvin (24 patients, mean age 32.5 years) daily, administered in identical capsules. Patients were evaluated before and during treatment, clinically, by direct microscopy and cultures in double-blind conditions. The maximum duration of treatment was 6 weeks. 26/26 patients in the ketoconazole group and 22/24 patients in the griseofulvin group had negative cultures after 2.5 and 3 weeks (median values) respectively. Clinical symptoms responded rapidly and completely to both treatments. Therapeutic results were statistically significant in both groups. 1 patient in each group relapsed during the next 3 months after the end of the treatment. No unwanted effects were reported. Although both treatments were effective, therapeutic results were slightly better and appeared earlier with ketoconazole.
