Merkel cells (MC) are epithelial cells expressing cytokeratin-type intermediate filaments. They often are localized within the basal cell layer of the epidermis. Since basal cell layer keratinocytes synthetize basal membrane components, it was of interest to investigate whether or not MC could also do so. We used both double-labeling immunofluorescence and immune electron microscopy techniques with a panel of antibodies allowing the identification of MC as well as the staining of basal membrane zone components (including bullous pemphigoid antigen, laminin, type IV collagen and epidermolysis bullosa antigen). A specific loss of BP antigen expression was observed below all MC directly in contact with BMZ. This suggests that, although being an epithelial cell and in contrast to basal keratinocytes, MC does not secrete BP antigens.

A retrospective study in 9 patients with Zoon’s balanoposthitis was done to determine the immunoglobulin class distribution in the plasma cellular infiltrate. This was carried out on paraffin-embedded specimens using an unlabelled antibody peroxidase-antiperoxidase method. In all lesions IgG-producing plasma cells predominated. IgA- and IgD-positive cells were also present in about equal quantities but fewer than IgG. IgM-positive cells were either absent or present in very low numbers. Especially in erosive lesions a high number of plasma cells was noticed. The κ/λ ratio was variable with a preference for λ in 5 of 8 cases. These findings are suggestive of a nonspecific polyclonal stimulation of B cells, which might be caused by a persistent infection. The above-mentioned findings were compared with premalignant and malignant skin lesions in which a major amount of plasma cells was present in the inflammatory infiltrate. The patient described here had all the clinical features (itchy red papules and reticular pigmentation) as well as the histological findings (lichenoid tissue reactions) of prurigo pigmentosa, a little-known disorder of unknown etiology, and responded well to sulfamethoxazole treatment. In order to investigate the mode of action of this drug and thus the underlying pathogenesis of this dermatosis, the effect of sulfamethoxazole on the production of oxygen intermediates (OIs) was examined both in the cell-mediated system and in the xanthine-xanthine oxidase system. It was found that therapeutic doses of the drug significantly reduced the hydroxyl radical levels, one of the most potent oxidants that might cause tissue injury. These findings suggest the possibility that OIs produced by infiltrated cells are involved in the inflammatory process of prurigo pigmentosa and that sulfonamides exert their anti-inflammatory effects by affecting OI generation which results in protection against lichenoid tissue reaction.

Fourteen patients with necrobiosis lipoidica (NL) were carefully evaluated clinically and histologically. Ten of these patients (71%) had diabetes and most of them were treated with insulin (8 of 10 patients). Trauma was found as an immediate triggering factor in 6 patients (43%) and a possible preceding factor in 3 more cases. No immunoglobulins were found in the lesional skin of NL. Intradermal injections of histamine in healthy skin of legs led to deposition of immunoglobulins and complement (C3) in skin blood vessel walls in 4 of 12 patients studied (30%). These results suggest that immune complexes might be involved only in few cases of NL. Preceding trauma combined with metabolic and inflammatory changes could be an important triggering factor in the etiology of NL.

Two cases of penis tuberculides are described. Although this disease once used to be comparatively common in Japan, the number of people affected by the disease has recently decreased as the prevalence of tuberculosis has fallen. However, we can still find a few cases in the Japanese literature every year. Outside Japan, we can hardly find reports of this disease, except for a few cases reported under the title of ‘Papulonecrotic tuberculides on the penis’ from Northern European countries. Four patients had a reccurrence of cutaneous lesions 1 week after being stung by jellyfish. Three patients had flare-up lesions after only one exposure to jellyfish. All of the recurring lesions were vesicular erythema, and the histological findings of case 3 corresponded to that of allergic contact dermatitis.

Patients with 6 or more recurrences per year of herpes labialis were entered into a randomized, double-blind, placebo-controlled, crossover study. During the trial, they applied acyclovir cream for 16 weeks and placebo cream for 16 weeks to all previously affected areas 4 times per day, and were subsequently observed for a further 16 weeks with no treatment. Results from the 23 evaluable cases showed that although recurrences did occur whilst on acyclovir treatment, patients suffered from significantly fewer days of disease and significantly fewer attacks of cold sores when compared with placebo.

A double-blind, split-face, self-controlled pilot trial was designed for comparing the therapeutical effect of (a) combination of 2% miconazole nitrate and 5% benzoyl peroxide applied twice daily and (b) topical application of 1% isoconazole nitrate in the morning followed in the evening by 5% benzoyl peroxide. There were no clinically relevant differences between both treatment regimes. These results are interpreted in terms of bioavailability of the active substances.

Solitary pulmonary lesions were detected roentgenographically in two melanoma patients during routine follow-up examinations. Melanoma metastases were suspected and the tumors were surgically removed. One turned out to be bronchogenic carcinoma, the other was found to be a chondroma. These cases indicate that in suspected solitary pulmonary melanoma metastases a surgical approach may be reasonable.

A 57-year-old Japanese woman had a history of several years of a slowly growing, dark red plaque on the sole of her foot without any subjective symptoms. Histopathologically, the lesion was a capillary hemangioma which showed a lobular structure composed of newly formed capillaries. Each lobule was separated by thin elongated rete ridges. The lesions was definitely circumscribed in papillary and subpapillary layers and appeared to extend laterally.
