Clinicopathologic findings are reported of a woman with plane xanthomatosis, multiple myeloma (IgGκ) and normolipemia. Plasma lipoproteins were bound to the monoclonal immunoglobulin. The complex was separated by ultracentrifugation, then the proteins were measured by radial immunodiffusion and laser immunonephelometry. Monoclonal IgGκ interact with the low-density lipoproteins. The literature about the association between normolipemic or hyperlipemic xanthomatosis and myeloma was reviewed and the physiopathology of this association discussed. Several hypotheses are suggested but, at present, it is shown that lipoprotein-paraprotein complexing in some patients may be due to autoantibody activity of the myeloma protein against serum lipoprotein; immune complexes interfere with normal lipoprotein catabolism resulting in xanthomas and normolipemia or hyperlipemi

Sebum excretion rate (SER) was determined by thin-layer chromatography and based on densitometric data in 9 male and 5 female patients suffering from acne during treatment with 10% benzoylperoxide (BPO). BPO was applied to the forehead (treatment area), whereas gel only was applied to the chest (control area) for 6 weeks. SER decreased in 10/14 subjects, no change was found in 1 subject and 3 patients exhibited an increase in SER. Suppression varied between 12 and 86%. Concomitant SER reduction in control and treatment areas was noted in 6 patients. When treatment and control areas of individual patients were considered for data evaluation, SER decrease was noted in only 4 out of 14 subjects. Similar results were obtained when free fatty acids, triglycerides and squalene data were analyzed. Combined, these data suggest that BPO may affect SER in some patients. The present experiments indicate the importance of adequate controls in such studies.

The most relevant literature concerning Paget’s disease of the breast is reviewed. A clinical and pathological study of female patients with Paget’s disease of the breast observed during 30 years at the Lisbon ‘Institute Português de Oncologia’ is carried out and its results are discussed. The low incidence of the disease in this series, when compared with those described in the available literature, is pointed out. The absence of a relation between the type of the cutaneous lesions and the histological type of the underlying tumor is emphasized. The similarity of the microscopical aspects, whichever the histological type of the underlying tumor, is considered. The differences in the clinical behavior of the tumors, related to their histological types, and the differences in respect to age, menarch, menopause, breast feeding, primary lesion and percentage of adenopathies observed between patients with and without a palpable mass, are enumerated. The authors conclude that the cure rate after 5 years is lower in patients with palpable tumors and adenopathies and in the cases with onset before menopause. Atopic cataract was observed in 19 (12.4%) of 153 Japanese adolescent and young adult patients with atopic dermatitis. Of the 19 patients, 4 had gross cataract which produced impairment of vision, and 15 had incipient cataract which was not accompanied by visual impairment. Cataracts occurred most frequently in patients with severe atopic dermatitis. Cataract development had no relationship to the distribution of skin lesions, clinical course of dermatitis, personal history of respiratory atopy, serum IgE levels, or concomitant ichthyosis vulgaris.

A comparative clinical study on therapy of nodulocystic acne with 13-cis-retinoic acid was performed in 32 patients. 20 patients were treated with a dose of 1 mg/kg body weight daily, and 12 with a combined therapy: 0.25 mg/kg/day 13-cis-retinoic acid and 0.5 g of tetracycline daily. In both groups the therapy was carried out for 16 weeks. The effect of the combined therapy within 4 months was favourable in 75% of the cases versus 100% in the group treated with larger doses of 13-cis-retinoic acid. Thus, the time needed for clearing was longer in some patients (up to 6 months). However, the incidence of side effects was considerably lower in the group of combined therapy. In acne fulminans 13-cis-retinoic acid alone is not effective and should be combined with corticosteroids.

Five corticosteroid ointments and placebo were compared in 17 volunteers with regard to their influence on normal skin under occlusive conditions. Each volunteer had six simultaneous applications on the forearms and six on the back. The trial was double-blind and lasted 4 weeks. The ointments were placed in randomized order. The treatments were 0.1 and 0.03% domoprednate, 0.1% hydrocortisone butyrate, 0.1% betamethasone valerate, 0.05% clobetasole propionate and placebo. Skin thickness was measured on days 0, 7, 14, 21 and 28, transepidermal water loss on days 0, 14 and 28, while blood flow and telangiectasias were evaluated only on day 28 at termination of the trial. The skin thickness became significantly reduced on all corticosteroids, but not on placebo; 0.03% domoprednate, however, tended to have an intermediate position between placebo and the other ointments. The transepidermal water loss did not change. Rating of telangiectasia under stereomicroscope showed a significantly lower score after 0.03% domoprednate and placebo as compared to the other ointments. Assessment of telangiectasia by laser-Doppler flowmetry showed a similar tendency. It is concluded that 0.1% domoprednate is comparable to other topical corticosteroids with respect to atrophogeneity and formation of telangiectasia, but the 0.03% concentration seems to result in fewer side effects.

In 85 patients with psoriasis vulgaris the efficacy and tolerance of domoprednate and betamethasone valerate were studied in a double-blind, randomized trial lasting 4 weeks. Complete or satisfactory remission was obtained in 36% in the domoprednate group and in 53% in the betamethasone group. This difference is not statistically significant (p > 0.10). 3 patients on each treatment experienced some local irritation. No laboratory abnormalities were found during the study.

Specimens of 4 different cervical cancers, 1 flat condyloma of the cervix, 1 condyloma acuminatum, 1 morbus Bowen of the skin and 1 skin wart were subjected to in situ hybridization using human papilloma virus type 16 (HPV-16) DNA nick translated with 3H-TTP as a probe. Within each cervical cancer biopsy we found a certain number of tumor cell clusters with clearly labelled nuclei, while sections of the other skin lesions did not reveal any accumulation of grains within their nuclei. Southern blot hybridization of the DNA extracted from the 4 cervical carcinomas with 32p-labelled HPV-16 DNA gave a positive reaction in 3 tumors. Non-reactivity in the 4th biopsy might be due to a low concentration of HPV-genomes within the total extracted DNA of this tissue part.

Eccrine porocarcinoma is a rare malignant skin tumor arising from the epidermal sweat duct unit (acrosyringium). The report of 4 cases illustrates the clinical and histological characteristics. The prognosis of this carcinoma remains difficult to assess because of the lack of a catamnestic study.
