Somatomedin-C/insulin-like growth factor I (SM-C/IGF-I) binds to both cultured keratinocytes trypsinized to single-cell suspensions and fresh epidermal sheets derived from 1 M NaBr-treated 6-mm punch biopsies. A gradual increase of 1–4% in specific binding of SM-C/IGF-I to fresh epidermal sheets was observed during a 5-hour incubation period at 15°C. The human keratinocyte SM-C/IGF-I receptor was isolated by polyacrylamide gel electrophoresis after cross-linking with iodinated SM-C/IGF-I and migrated at 135,000 daltons under reducing conditions. Both a partially purified preparation of SM-C/IGF-I, as well as recombinant DNA-derived thr-59-SM-C/IGF-I (25–100 ng/ml), stimulated keratinocyte replication in a serum-free system, with an approximately equivalent effect as insulin (10 μg/ml). Since fibroblasts produce SM-C/IGF-I, our data suggest that some dermal-epidermal interactions in diseases such as psoriasis may be mediated, in part, by SM-C/

Ninety-eight patients having fixed drug eruptions were subjected to provocation tests. Cutaneous involvement was seen in 43, mucous membrane in 33 patients, while cutaneous and mucous membrane involvement was seen in 22 patients. The causative drugs were found to be trimethoprim-sulphamethoxazole (45), acetylsalicylic acid (24), hyoscine butylbromide (Buscopan®) (8), ibuprofen (Brufen®) (6), oxyphenbutazone (6), tetracycline hydrochloride (2), phenolphthalein (1) and phenobarbitone (1). A renal allograft recipient with small raised red scaly lesions on a forearm is described. The clinical and histological picture was that of similar lesions reported in some epidermodysplasia verruciformis patients. Virological investigations revealed the presence of human papilloma virus type 5; this is the second report of the occurrence of this type in allograft patients. As human papilloma virus type 5 is known to have malignant potential, clinicans should be on the lookout for these banal-looking and distinctly non-warty lesions in renal transplant recipients. Close follow-up is indicated.

The effect of nifedipine on subjective symptoms and objective dermographometer induced whealing was assessed independently in two double-blind crossover trials in which active drug or placebo were taken for 2 weeks. The lower dose study used nifedipine 5 mg 3 times daily and gave rise to symptomatic improvement as assessed by diary card and visual analogue scale in 5 of 11 patients. This was not statistically significant and was not accompanied by a reduction in dermographometer-induced wheal and flare responses except at highest pressures. In a separate study 7 patients were treated with nifedipine 10 mg 3 times daily. No change in whealing occured, 3 patients’ symptoms became worse and only 1 showed symptomatic improvement on nifedipine. Side effects were frequent at this dose. Eccrine sweat gland carcinoma is a rare malignancy of skin adnexa with potential aggressive growth and metastatic spread. We report here a case of eccrine carcinoma arising on a finger with widespread pulmonary metastasis. A brief synopsis of the pathological and clinical aspects of eccrine sweat gland carcinoma is presented and currently available therapeutic modalities are discussed.

A homosexual male, seropositive to HIV and with previously documented syphilitic infections, developed a slowly growing nodule on his left wrist (possibly a chancre) followed, after 2 months, by a few scattered, large papular lesions. Serological evidence of active syphilis was obtained and treponema were identified in the initial lesion by means of immunofluorescent staining. Treatment with both aqueous crystalline penicillin G and benzathine penicillin G healed the lesions. This represents a further case of an atypical presentation of early syphilis in an immunocompromised host.

A case of pitted keratolysis caused by Dermatophilus congolensis is reported. The organism was isolated from the lesion and identified by its morphological, cultural, and biochemical characteristics. A survey of the literature revealed that it rarely causes human infections, but is a common causative agent of disease in domesticated and wild animals. Human infections reported previously were traced to contact with infected animals or contaminated soil. We report pitted keratolysis in a 44-year-old physician with no known history of such a contact.

A 35-year-old woman developed typical relapsing polychondritis within a year following the onset of dermatitis herpetiformis. This occurred during a reduction in the dapsone dose regimen. To our knowledge, the coexistence of these two conditions in a single patient has not yet been reported in the literature. Both the parallel course and the good response to dapsone suggest a pathogenic link between these two diseases.
