摘要:
The clinical-experimental provocation of blistering and the spontaneous appearance of bullous dermatoses justify the following conclusions: (1) The subcorneal blistering is caused by (a) colloid-osmotic pressure and (b) damaging the membrane of epidermal cells by the toxic influence of enzymes and the chemotactic stimulus on blood cells. (2) The intraepidermal blistering occurs, additionally to (1), by loosening the contact of keratinocytes through irritation of desmosomes and of the plasmalemma, depending on the amount of the doses of the provocating substances, as cantharidin. The morphokinetics of pemphigus vulgaris in comparison with the effect of artificially created intraepidermal blisters (cantharidin) show similarities only in the location but not in the microcellular characteristics. (3) The junction blister, located between basal lamina and basal cells, develops through blocking of enzymes by disturbances of the general metabolism as illustrated by the blistering caused through monoiodoacetate, yellow cross or intracutaneous injection of proteolytIc enzymes. The exchange of fluid of the microcirculation together with viscosity changes within the area of the basal membrane is of importance. (4) The subepidermal blistering, which develops beneath the basal lamina, originates from (a) damage of anchoring fibrils and (b) perivascular edema. This situation leads to blistering in varying layers of the skin combined with autolysis and general liquefaction with regard to separation. (5) The disturbed function of the membrane, especially within the epidermis, leads to a diffusion of the intracellular enzymes (lysosomes), including electrolytes, towards the extracellular space. The active transport against the concentration gradient is missing. (6)The principles of the Koebner phenomena can be demonstrated by chemical vesicants. The limitation in the process of blistering to the subbasal layers only (as in dermatitis herpetiformis Duhring) can be concluded from the increased resistance of the epidermis to the blistering substance as cantharidin). (7) The appearance of spontaneous bullous dermatoses as pemphigus or pemphigoid is closely related to the presence of antibodies and aggressive C’ factors in the blood with corresponding titers. (8) From the viewpoint of therapy, drugs which stabilize the endothelial membranes of the microvessels and diminish the exchange of fluid towards the interstitial area are effective. In an uncontrolled investigation 63 selected patients with verrucae vulgares were sensitized with DNCB (1-chloro-2,4-dinitrobenzene) with subsequent regular painting of the warts with DNCB. Only 86% of these patients could be sensitized, in contrast to 96% in a normal population. 20 patients had to be excluded from the material on account of defaulting or defective sensitization. In the remainder of the patients allergic type IV inflammation developed around the painted warts and 1 patient developed an urticarial type I reaction around the warts and on the body. In the patients with multiple warts, 80% (24/30) were cured. This corresponds to the percentage cure in patients with solitary warts. Complement-binding wart virus antibodies were present in 15% of the patients prior to the treatment and in 43% after the conclusion of treatment. This investigation provides a new method of treatment of verrucae vulgares and suggests that regression in the warts is immunologically conditioned. On account of the risk of allergic side effects, the method should be carried out in special departments only.

摘要:
A mother and her son are described who both suffer from hereditary angiooedema. Both were treated with Danazol, a synthetic derivative of ethisterone. The prophylactic clinical effect seemed obvious, and also the serum concentration of the complement component C4, which had regularly been lower before treatment, became normal. However, no change of the serum concentration of the C1 esterase inhibitor could be demonstrated; this is in contrast to observations of other authors.

60 male Syrian hamsters were treated on one ear with 2.5% selenium disulfide in a surfactant base and on the other ear with the same surfactant base. After treatment 15 animals were investigated by 3H-thymidine autoradiography, 15 animals by the colcemid method and 15 animals by the in vivo double labelling technique with 3H- and 14C-thymidine. In 15 animals the porportion of labeled cells was determined which still had contact with the basal lamina in the sebaceous gland 6 days after 3H-thymidine injection. 摘要:
A patient already published as a case of Richner-Hanhart syndrome (RHS) (stabilized corneal lesions and hyperkeratotic lesions on the palms and soles) proved to be associated with tyrosinemia type II. 2 other cases (sister and brother) with only typical dermatologic features of RHS and tyrosinemia type II are described. The treatment with a low phenylalanine and tyrosine diet improves the cutaneous lesions in our 3 cases.

摘要:
The weal and flare produced by monochromatic irradiation in solar urticaria may be treated as a classical dose-response. This has been used to investigate therapy with H1 and H2 antihistamines. The conventional H1 drug proved superior. But from the practical viewpoint, solar urticaria is difficult to suppress even with a relatively efficient H1 antihistamine, chlorpheniramine; the mean protective factor in 5 patients was only 2, insufficient for satisfactory clinical management.

摘要:
We report the case of a man aged 39 who received several injections of mineral oil in his pectoral muscles 20 years ago. Supposedly, the purpose of the injections was to improve the aesthetic appearance as well as the strength of the muscles. The final result was the development of impressive symmetrical paraffinomas. A short review of the recent literature on paraffinomas is presented.
