In a series of fibrotic and sclerotic diseases including scars keloids fibromas scleroderma and lichen sclerosus et atrophicus, we found modifications in the dendrocyte population. Dendrocytes were numerous in fibrotic diseases associated with little deposits of collagen. Conversely, they were almost absent in sclerotic diseases when the lay-down of collagen was prominent. We hypothesize that dendrocytes, by their content in factor XIIIa, may limit the accumulation of collagen in the skin.

The aim of this retrospective study is to delineate in Europe the frequency and type of cutaneous manifestations associated with Takayasu arteritis (TA). Eighty patients with TA were analyzed. Symptoms suggestive of Raynaud’s syndrome were noted in 11 patients (14%) and could be directly related to large vessel involvement. Other skin lesions were observed in 10 patients (12.5%). Five had acute tender erythematous nodules on the legs with a clinical diagnosis of erythema nodosum; 2 had subacute ulcerated nodules of the legs; 1 had pyoderma-gangrenosum-like ulcerations of the four limbs which resulted from the breakdown of subcutaneous nodules; 1 had lupus-like malar flush, and the last one had urticarial lesions with livedo reticularis. Skin samples were obtained from 4 patients. Three of them agreed that reiterated biopsies be done on recurrent lesions. A granulomatous vasculitis was observed in 2 cases involving hypodermal arterioles in one case and veins in the other. The other pathological findings were septal and lobular panniculitis which can be associated with granulomatous vasculitis. Different histological findings on reiterated biopsies were frequently found. The absence of any other etiology and chronological arguments suggested a relationship between these skin lesions and TA. Tuberculosis was probable in 1 case but apparently was not related to the skin lesion

Kidney biopsies were studied in 19 psoriatics treated with methotrexate (MTX) from 1 month to 16 years. Only 3 of the patients had received the drug for less than 1 year. Four patients had been treaed for more than 10 years. The total doses of MTX ranged from 30 to 14,340 mg. In 17 of the 19 patients the cumulative MTX dose was 500 mg or more. In 9 patients the cumulative doses were over 2 g. With the exception of 1 patient, which presented sequelae of acute renal failure unassociated with MTX treatment, all other biopsies showed renal structures which, assessed by routine judgement, were found normal. Semiquantitative evaluation of interstitial fibrosis and arteriolar hyalinosis as well as interstitial volume estimated by morphometry showed no significant changes. No patient showed clinical or laboratory signs of renal impairment.

The relationship between the in vivo irritation potential of sodium lauryl sulfate (SLS) and linear alkyl benzene sulfonate (LAS) and the ability of these two surfactants to remove lipid from the stratum corneum (SC) in vitro were investigated. Either surfactant removes detectable levels of lipids only above its critical micelle concentration (CMC). At high concentrations the surfactants removed only very small amounts of cholesterol, free fatty acid the esters of those materials, and possibly squalene. SLS and LAS have been shown, below the CMC, to bind to and irritate the SC. Thus, clinical irritation provoked by SLS or LAS is unlikely to be directly linked with extraction of SC lipid. The milder forms of irritation – dry ness tightness roughness – may involve both surfactant binding to and denaturation of keratin as well as disruption of lipid. The dermatopathologist is sometimes confronted with a single lesion biopsy showing the histopathology of a lichen planus (LP) taken from a patient having no further clinical signs of LP. This entity represents the ‘solitary lichenoid benign keratosis’ (SLBK). We report about 202 patients with 204 SLBK lesions which were diagnosed between 1981 and 1987. The mean age of these patients was 59 years, 62.4% were females. Only 2 of the 202 patients showed multiple lesions. In 30.6% the SLBK was located in the face and neck area. In the order of frequency the following clinical diagnoses were made: basal cell carcinoma, senile keratosis, Bowen’s disease or seborrheic wart among others. We were able to distinguish three clinical types: (1) an erythematous type, (2) a papulokeratotic type and (3) a plaque-like type. The histopathological difference between SLBK and LP is primarily quantitative. The SLBK shows the following pronounced criteria: parakeratosis, vacuolized keratinocytes in the basal layer, a spongiotic epidermis in the center of the lesion with exocytosis of lymphoid cells, edema in the papillary layer, elastosis and plasma cells. Regularly one can find ‘lentigo-senilis’-like lesions at the edge of an SLBK. Immunohistopathology showed no characteristic features in SLBK. The pathogenesis of this distinct entity is not

The complement system may play an important role in the inflammatory reaction of psoriasis. While the classical pathway of the complement has been shown to be activated in psoriasis, there have been few reports on the involvement of the alternative pathway in the inflammatory reactions in psoriasis. Complement fragments, C4d and Bb, are released at the time of the classical and alternative pathway activation, respectively. The presence of the C4d or Bb fragment, therefore, denotes a preceding complement activation through the classical and/or alternative pathway. In the present study, we have measured the levels of C4d and Bb in psoriatic scale extracts using enzyme immunoassay techniques. The scales of these dermatoses contained higher levels of C4d and Bb detectable by enzyme immunoassay than those in the stratum corneum of noninflammatory skin. These results suggest that the alternative as well as the classical pathway of complement are activated in psoriatic lesional skin.

The debriding potential of a recently discovered potent enzyme preparation derived from Antarctic krill (Euphausia superba) was compared to a routine non-enzymatic treatment in 31 patients with venous leg ulcers. To assure a reliable interpretation of the cleaning effect, computer image analysis was used to measure wound size and the amount of debris day by day. Krill enzyme treatment was significantly (p < 0.01) better than the non-enzymatic treatment. The necrotic area diminished by 53% while in the control group no reduction of the necrotic area was observed in 7 days. The median time required to clean the ulcer was 7 days in the krill group and 10 days in the control group. Afterwards the ulcers were grafted with autologous skin biopsies, which yielded similar results in both groups. No side effects were observed. It was concluded that krill enzymes fulfil criteria for effective debridement of venous leg ulcers.

Eighteen patients with advanced metastatic malignant melanoma (stage IV UICC1987) were entered into a prospective trial with a combination of systemic fibroblast interferon-β and recombinant interferon-γ. Treatment was performed over a 6-week period with 3 × 5 × 106 U i.v. interferon-β and 5 × 100 μg s.c. interferon-γ every week. Under this therapy 16 patients showed a progressive disease, and 2 patients had a stable disease. The median time of survival was 7.5 months. Successive immunological examinations showed no significant immunomodulating effect after the 6 weeks of interferon treatment. We conclude that the combination of interferon-β and -γ is insufficient in the treatment of advanced metastatic malignant melanoma when administered by this dose, route and

An 84-year-old male with a 4-week history of vesiculobullous eruptions was diagnosed clinically and pathologically to suffer from pemphigus foliaceus. Amazingly, direct immunofluorescence demonstrated intercellular IgA deposits in the whole epidermis. The patient did not respond to a regimen of fluocortolone and azathioprine but rapidly improved under dapsone. The few reported dermatoses with intraepidermal IgA deposits do not form a homogeneous group. In the following, their spectrum will be outlined. It is important to distinguish these dermatoses from IgG pemphigus, since they require a different therapy.
