Whereas hyperthermia has long been used in dermatology for the therapy of diseases as diverse as syphilis gonorrhea, psoriasis or melanoma, the understanding of the biological effects of heat shock on the skin attracts new interests to an old field. The proteins induced by heat (stress or heat shock proteins) appear to play a general role in protection from cellular injury and eventually in the natural defences from solar radiation. On the other hand these ubiquitous proteins may also be involved in the immunopathology of diseases such as systemic lupus erythematosus or leprosy.

To determine the prevalence of congenital melanonevocytic nevi (CMN), of so-called congenital nevus-like nevi (CNLN), nevi spili (NS) and café-au-lait spots (CLS) in childhood, a series of 939 children aged 8–16 years had total skin examination. CMN/CNLN were observed in a frequency of 5.9% NS in 2.1 % and CLS even in 32.7%. Except CLS (which occurred more frequently in boys) all of these lesions were equally represented in both sexes. According to the classification of Kopf and coworkers, 44/55 CMN/CNLN were small and 11/55 medium-sized. CMN/CNLN affected preferentially trunk and upper limbs while head and neck were spared. Such nevi occurred more frequently in dark types of skin complexion and showed a tendency to be more frequent in groups of patients with increased average number of acquired melanonevocytic nevi. Remarkably, CLS were also found in 23/55 (41.7%) children with CMN/CNLN and in 12/20 (60%) children with NS (in a higher frequency than such combinations have to be expected). Compared with the data from other studies both CMN/CNLN and CLS in our patients were observed in highest prevalence in the literature. In this study the clinical and laboratory data of 14 patients who experienced 16 attacks of nonstaphylococcal toxic epidermal necrolysis (TEN) are presented. All patients were treated in the Department of Dermatology of the University Hospital of Utrecht. Only 1 patient died. Attention is focused on a successful management consisting of reversed barrier nursing, painstakingly executed skin care, timely use of antibiotics and a therapy consisting of high dosages of corticosteroids. Thorough management in centers experienced in the treatment of TEN is essential for good treatment results. Further investigations concerning the use of corticosteroids in treating TEN are needed.

Ten cases of localized and generalized discoid lupus erythematosus are reported in which previously untreated patients were given hydroxychloroquine sulphate 600 mg daily for 10 days followed by 400 mg for 20 days. The purpose of the study was to evaluate the effect of this drug on the ‘lupus band’ before and after treatment, in diseased, unaffected sun-exposed, and unaffected non sun-exposed skin. A good response from both the clinical and immunopathologic (i.e. reduction or disappearance of the immune reactants) standpoint was evident in 6/10 patients; in another 3 patients a good clinical but not immunopathologic response was recorded, while in 1 case a clinical worsening corresponded to an immunofluorescence improvement. Acquired C1 esterase inhibitor deficiency is a clinical syndrome closely resembling hereditary angioedema in which most patients have an associated malignancy of B cell lineage. Sera from 33 patients with B cell neoplasms were assayed for C1 esterase inhibitor level by rocket immunoelectrophoresis. The clinical summary of a patient who met the biochemical criteria for acquired C1 esterase inhibitor deficiency (low C1 esterase inhibitor, low C4) is presented. We hypothesize that this syndrome represents an autoimmune reaction involving tumor cell rejections and of potential benefit to the patient.

Clinical significance of specific antinuclear antibodies (ANA) in Japanese patients with systemic sclerosis was studied. The patients with systemic sclerosis were classified into four groups according to ANA: (1) anticentromere antibody-positive; (2) anti-Scl-70 antibody-positive; (3) anti-nRNP antibody-positive, and (4) others. The mean score (the number of positive signs in six selected specific signs) of the patients with anti-Scl-70 antibody was significantly higher than those of the other three groups. More frequent contracture of phalanges in the patients with anti-Scl-70 antibody less diffuse pigmentation in the patients with anti-nRNP antibody and less pulmonary fibrosis in the patients with anticentromere antibody were revealed. As shown above the detection of specific ANA in systemic sclerosis is clinically important.

An immunogold technique in scanning electron microscopy was applied to the detection of Langerhans cell surface CD1a antigen in a heterogeneous human epidermal cell suspension. Scanning electron microscopy investigations performed in the secondary electron imaging mode and in the backscattered electron imaging mode allowed to define Langerhans cell surface morphology and the labeling degree, respectively. In particular, the backscattered electron imaging mode of the scanning electron microscope appeared very useful in identifying colloidal gold particles and, as a result, made it possible to make total counts of the labeled surface antigenic sites. Finally, the combination of secondary electron and backscattered electron signals generated mixed images with the Langerhans cell surface morphology well recognizable together with distinct gold particles. The immunogold labeling in scanning electron microscopy here described represents a highly sensitive method and yields a new improvement in the study of Langerhans cells immunophenotype.

We report a case of a 23-year-old woman who was afflicted with disseminated skin erythema multiforme-like eruptions that started at the menarche, relapsed at the premenstrual periods, dramatically spread during two pregnancies and cleared after abortion; the skin lesions responded dramatically to thalidomide treatment. A high-affinity binding factor to 17α-hydroxyprogesterone (17-OHP) was found in the serum of this patient. Her lymphocytes did not proliferate in vitro after exposure to exogenous 17-OHP but showed significant chromatin activation. There was a decreased expression of HLA antigens at the surface of the patient’s blood lymphocytes. Anticardiolipin antibodies (ACA) were determined by an ELISA assay in 142 VDRL-positive patients who were classified in two groups: group I consisted of 77 patients with syphilis and group II of 65 patients with biological false-positive reaction in the standard test for syphilis. In group I 10% had ACA versus 52% in group II (p < 0.0001). Isotype distribution in group II showed a predominance of IgG. These results suggest that antiphospholipid antibodies in VDRL and ACA tests have different specificities.

Three cases of solar urticaria in the visible spectrum successfully treated with astemizole, a H1 antihistamine, are reported. The administration of 10 mg a day of the drug increased the minimal urticarial dose from 2 to 12 times.
