摘要:
Normal human skin grafted onto the nude mouse can be maintained without any signs of rejection throughout the life-span of the animal. Indeed, the nude mouse model is a powerful tool for understanding the pathological process of the skin. Until now many skin diseases such as psoriasis, cutaneous lupus, pemphigus and vitiligo have been looked at using the nude mouse model, which has helped to clarify the role of the various factors involved.

Modulation of Ro/SS-A autoantigens in epidermal keratinocytes has been implicated in the pathogenesis of photosensitive forms of Ro/SS-A-antibody-associated cutaneous lupus erythematosus (LE) such as subacute cutaneous LE and neonatal LE. Since Ro/SS-A ribonucleoprotein particles have recently been shown to be very complex molecular structures, we have performed studies to determine whether the expression of three of the Ro/SS-A antigenic polypeptides might be differentially regulated in transformed human epidermal keratinocytes following UVB radiation. Our findings indicate that both total cellular and cell surface levels of calreticulin are upregulated by UVB exposure more so than are either the 52- or 60-kD Ro/SS-A antigens. These results suggest that calreticulin could be a critical component of the Ro/SS-A ribonucleoprotein complex involved in the pathogenesis of Ro/SS-A-antibody-associated LE skin lesions.

The expression of some adhesion molecules of the integrin family and their ligands was investigated in skin biopsies from a patient with lymphomatoid granulomatosis (LG), who at onset presented cutaneous lesions followed by lung and brain involvement. The angiocentric and angiodestructive skin infiltrate consisted mainly of T helper ‘memory’ cells (CD2+, CD3+, CD4+, CD5+, CD7+, CD45RO+) with a variable expression of activation antigens (CD25––, CD38+, CD71+, HLA-DR+) and 20% Ki67+ (nearly all atypical cells). Sixteen anti-Ro/SS-A-positive patients with recurrent annular erythema, which has recently been recognized as a unique cutaneous manifestation of Sjögren’s syndrome, were studied. Eight of the 16 patients met the American Rheumatism Association criteria for systemic lupus erythematosus. Fifteen patients had anti-La/SS-B antibodies. Antibodies against U1RNP were detected in 2 patients with systemic lupus erythematosus, 1 with anti-Sm antibodies. Patterns of autoimmune response to Ro/SS-A antigens were variable by immunoblot analysis. HLA typing by the standard complement-dependent microcytotoxicity assay revealed that all the 16 patients were positive for HLA-DRw52 antigens but negative for either HLA-B8 or HLA-DR3 which is reportedly associated with the autoimmune response to Ro/SS-A antigens in white and black patien

Clinical and immunopathological analysis was performed on 24 cases of Sjögren syndrome with annular erythema (AESjS). AESjS predominantly appears on the cheek of the face where skin temperature is relatively low in comparison with other sites. VCAM-1 and ICAM-1 were strongly expressed on endothelial cells of AESjS, while epidermal expression of ICAM-1 was focal and weak. VCAM-1 mRNA expression was also much more intense compared to systemic lupus erythematosus. The lymphocyte response to staphylococcal enterotoxin B was higher in AESjS than that of controls, and cells positive for T cell receptor Vβ6,9,12 were expanded after the culture. The ability to image the subsurface morphology of intact three-dimensional matrices such as skin epidermal tissue, using conventional optical microscopy, requires the tissue to undergo an extensive preparative protocol. This protocol includes fixing, embedding and physically sectioning the specimen. The process is slow and subject to sample perturbation, which can result in image misinterpretation. In this study, confocal laser scanning microscopy is used successfully for the imaging of the surface and underlying morphology of unlabelled and fluorescently labelled intact skin epidermal tissue, from various origins (that is human, pig and cultured epidermis). Unlike conventional optical microscopy, the technique requires little or no pretreatment of the tissue prior to imaging. Hence the technique is rapid, and the tissue experiences minimal physical perturbation and/or damage. Confocal images of skin epidermis in the reflected light (unlabelled) and fluorescent modes were obtained. In the reflected-light mode the detailed morphology of the various layers of the intact epidermis (that is the strata corneum, granulosum, spinosum and basale) from a variety of sources are clearly imaged. In the fluorescent mode, environment-specific information is obtained. For example, topical application of the fluorescent probe acridine orange readily allows the confocal imaging of the nuclear material (nuclei and nucleoli), from cells of the different strata of intact epidermis. In the case of the fluorophore Nile red, however, visualisation of the lipid material of the epidermis, including the intercellular lipid of the stratum corneum and the lipid granules of the stratum granulosum, is possible. The reflectance and fluorescence confocal images are self-consistent and agree with images obtained by conventional, physical sectioning protocols. The confocal microscope provides a rapid and non-destructive method for imaging complex three-dimensional matrices.

In this study, we investigated UVB-light-induced cytotoxicity and the binding of antibodies to extractable nuclear antigens on cultured keratinocytes from patients with cutaneous lupus erythematosus (LE). Keratinocytes from cutaneous LE patients showed a higher susceptibility to single-dose UVB light irradiation compared to keratinocytes from normal controls. The binding of antibodies to U1RNP and Ro/SS-A antigens on cultured keratinocytes was induced by UVB light and more up-regulated when cultured keratinocytes were reacted with autologous sera. Antibody-dependent cellular cytotoxicity (ADCC) was induced when cultured keratinocytes irradiated with UVB light were combined with autologous sera, using peripheral mononuclear cells of normal controls. Immunohistochemical studies of skin biopsy specimens from patients with systemic LE revealed an increased number of epidermal Langerhans cells at the peripheral sites of skin lesions and a relative dominance of infiltrative CD8-positive lymphocytes in the central area of skin lesions. Based on these findings we suggested that ADCC mechanisms were involved in the development of skin lesions, and the distribution of Langerhans cells and infiltrated CD8 cells were responsible for the expansion and persistence of lesions.

The importance of early detection of scleroderma spectrum disorders (SSD) has been emphasized. We determined the clinical distribution of anticentromere antibody (ACA) and anticentriole antibody in the following four groups: (1) 264 patients with SSD, including 193 with systemic sclerosis, 29 with mixed connective tissue disease and 42 with suspected secondary Raynaud’s phenomenon (RP); (2) 26 patients with primary RP; (3) 248 patients with other connective tissue diseases, and (4) 139 patients with other skin diseases. The frequency of ACA was significantly higher in SSD (78/264, 30%) than in the other groups. In patients with SSD, the incidence of ACA in suspected secondary RP (28/42, 67%) was similar to that in type I systemic sclerosis (24/36, 67%). Anticentriole antibody was detected in only 1 patient with suspected secondary RP (0.4%) out of the 264 SSD patients. These data indicate that anticentriole antibody is very rare and that the antibodies against mitosis-related antigens such as centromere and centriole are associated with early SS
