The leucocyte-migration-inhibition test was performed to examine 10 patients with progressive systemic sclerosis and 6 patients with linear scleroderma for evidence of cell-mediated immunity to RNA, DNA, human muscle antigen and collagen human type I. An inhibition of cell migration was detected in 6 patients with progressive systemic sclerosis (PSS) and in 3 patients with linear scleroderma in presence of RNA. No cellular reactivity in presence of DNA was observed. Seven cases with PSS, and all 6 cases with linear scleroderma showed a migration inhibition when human muscle antigen was tested. Cell-mediated immunity to collagen human type I was discovered 7 times in PSS and 2 times in linear scleroderma.

A case report is presented of an individual who carried vegetative cells of Bacillus firmus on multiple sites of his body for over a year. With the aim of investigating the survival mechanisms of Bacillus species on human skin, vegetative cells or spores of this microorganism were applied to the forearms of volunteers. Whereas vegetative cells were rapidly eliminated, the bacillus was recovered up to 2 weeks following an inoculum of spores. Persistence was not passive since germination, growth, and possible sporulation were demonstrated. We observed strong individual differences in the carriage of bacilli.

25 cases of atypical sweat duct proliferation in association with cutaneous tumors are described. 20 of these were keratoacanthomas. The compression of the duct either in its dermal or intraepidermal portion is considered to be the etiology. Similar changes are seen with decubitus ulcer and stasis dermatitis. Atypical sweat duct hyperplasia may be useful as an extra aid in the differentiation of keratoacanthoma from well-differentiated squamous cell carcinoma.

From 1973 to 1976, 21 patients with staphylococcal toxic epidermal necrolysis were admitted to the Jikei University Hospital in Tokyo. Of all of them were isolated coagulase-positive Staphylococcus aureus from various sites. In Japan, unlike in Europe and America, phage group II cocci were found in few cases. On the other hand, S. aureus belonging to other phage groups than group II (I, III mixed group and III group) were very commonly isolated. All the isolates produced exfoliatin which was capable of producing exfoliation in newborn mice. The type (A or B) of exfoliatin which was detected by the antiserum agar method did not correlate to the severity of this disease.

A short summary of normolipemic xanthomatoses is exposed. A case of xanthoma disseminatum is reported. The typical clinical picture is described and numerous investigations are recorded. No association with any systemic disorder neither diabetes insipidus nor rheumatoid arthritis could be found. Histologically the granulomas consist of many multinuclear giant cells embedded in an infiltration of histiocytic cells and lymphocytes. Fat staining demonstrates lipid accumulation as a secondary phenomenon. Systemic corticosteroid therapy was effective in preventing the regrowth of the granulomas after their surgical removal.

The effect of a new non-steroid anti-inflammatory substance (bufexamac) in a special constituent was compared with that of 0.1% triamcinolone acetonide, 1% hydrocortisone cream, and placebo during a double-blind multicentre trial. The clinical effect of these four creams was studied in 193 patients receiving treatment for the following skin disorders: atopic dermatitis, allergic contact dermatitis, and non-allergic contact dermatitis, as well as dermatitis seborrheica. After 2 and 4 weeks’ treatment, when 193 and 157 patients, respectively, were re-examined, the effect of triamcinolone acetonide and hydrocortisone cream was significantly better, than that obtained with bufexamac in the cream basis employed. 16 patients with cutaneous sarcoidosis were treated intermittently over a period of 6 months with levamisole in an open study. The skin lesions cleared in 2 of 13 patients completing the course of treatment. Aggravation of the sarcoidosis was noted in 2 other patients after a few weeks of treatment. Tuberculin sensitivity did not increase during the treatment period. Dinitrochlorobenzene sensitivity increased in 5 patients but the lesions did not clear in any of these patients. It is concluded that levamisole is not useful in the treatment of cutaneous sarcoidosis.
