摘要:

So-called biologically active detergents may produce direct toxic skin changes by causing the release of pharmacologically active substances (kinins). They also give rise to true contact eczema by their protease content. The significance of positive patch tests to these detergents in eczematous subjects who had not been exposed previously to enzyme containing detergents is discussed.

Ferrous Sulphate combines with Potassium Dichromate in cement to form an insoluble compound. The addition of 0.3% Ferrous Sulphate to patch tests with cement reduced in sensitive individuals the intensity of patch test reactions and abolished it in some altogether. Patients with a normal skin alkaline resistance test benefitted most. These observations suggest possibilities of preventing cement-induced Chromate sensitivity.

A 5% 5-Fluoro-Uracil ointment was found during an extensive trial to be a useful treatment of basal cell carcinoma and premalignant lesions like senile or actinic keratoses. The results remained satisfactory during a 2 year, in some cases even longer, follow-up period.

A 0.1% Dexamethasone ointment was found in a double blind controlled trial to be therapeutically superior to Locacorten (Ciba) in the treatment of eczema. It was not effective against psoriasis even under occlusive dressings. Corticosteroid applications produce local suppressions of the immune mechanism giving rise to viral infections (molluscum contagiosum, viral warts), but in particular to fungal infections; this is common with prolonged treatment of foot eczema. Subsequent anti-fungal therapy on its own, cleared all skin lesions.

1. All patients with so-called Penicillin resistant gonorrhoea respond to the intravenous infusion of 20 mega-units of Penicillin given over one 2 hour period without admission to hospital. 2. 75% of patients with contact eczema due to Chromate can be treated successfully with a local application which effects an ion-exchange and renders a previously positive patch with Dichromate negative. 3. Allergic contact eczema in hairdressers can be controlled by periodic injections of Synacthen-Depot (Ciba).
