A study has been undertaken of the relationship between the severity of psoriasis and alcohol consumption in 100 patients with chronic plaque psoriasis. In male patients, heavy drinking, at a level liable to be detrimental to health, was found significantly more commonly in those with severe psoriasis, and alcohol-related medical or social problems were frequent. By contrast alcohol excess and alcohol-related problems were significantly less common in women irrespective of the severity of their psoriasis.

We have evaluated the normal variations in the sebum excretion rate from follicle to follicle on the forehead by using the noninvasive techniques of the Lipometre® and Sebutape®. For a given overall amount of sebum excreted to the surface of the skin during a limited period of time, both the number of active sebaceous follicles and the amount of lipids excreted by them may vary. Significant intraindividual and interindividual differences may be found for these parameters. Such a noninvasive approach to the biology of sebaceous glands represents a new tool allowing a precise evaluation of diseases of the sebum excretion and of their treatment

Two sibs with palmo-plantar keratosis and dendritic corneal opacities, previously described as suffering from Richner-Hanhart syndrome by other authors, about 25 years ago, showed increased plasma and urine tyrosine levels. Their neurological and mental features were within normal limits. A comprehensive review of the literature showed a total of 47 cases of fully documented tyrosinemia type II; 8 more patients also had the clinical features of the disease, but aminoacidemia had never been observed. The importance of early diagnosis is stressed, since a low tyrosine-phenylalanine diet dramatically improves the symptoms and may prevent mental retardation.

In a patient suffering from benign lesions of epidermodysplasia verruciformis due to human papillomavirus type 5 little clinical improvement could be observed after 6 months of oral etretinate. The drug was given 75 mg/day for 1 month before dosage was reduced to 50 mg/day. Control biopsy after 3 months of treatment still revealed large quantities of virions and typical cytopathic effects in light and electron microscopic examination.

The expert system approach to computer diagnosis uses a non-algorithmic method to represent and manipulate an expert’s knowledge and reasoning. This information, which may be provided by a dermatologist, is represented by rules in a logic-based computer language in order to provide interactive and explanatory features. The major advantages of using expert system techniques for computer-aided diagnosis in dermatology are that (1) knowledge is made explicit, (2) the heuristic nature of an expert’s knowledge can be more easily captured and (3) the more easily readable programs make modification easier. In the example described, the differential diagnosis of nine facial skin tumours was considered. The program, using the language ‘micro-PROLOG’ in an expert system shell ‘APES’, consists of ‘rules’ and ‘facts’ which define a relationship between patient and symptom, symptom and disease or disease and therapy. Acute febrile neutrophilic dermatosis (Sweet’s syndrome) was diagnosed on the basis of typical clinical and histological features in a 45-year-old woman. Gastrointestinal symptoms had preceded the dermatosis, and an intestinal infection with Yersinia enterocolitica could be proven by culture and serology. Kasabach-Merritt syndrome with consumption coagulopathy occurred in a 5-week-old female infant with a hemangioma on her back. Disseminated intravascular coagulopathy was improved by an exchange transfusion. Administration of steroid and radiation therapy was attempted as a treatment for the hemangioma and thrombocytopenia; however, it was unsuccessful. Complete recovery was obtained by surgical excision. Enhanced computerized tomography was advantageous to determine the size of the hemangioma prior to operation.

Multiple malignant melanocytic lesions developed on a localized area of the thigh in an 80-year-old woman 3 years after excision of a primary malignant melanoma on the left second toe and dissection of left inguinal lymph nodes. Biopsy specimens obtained from 2 small pigmented macules revealed the histologic features of new primary lesions, simulating that of the primary lesion on the toe; no tumor cells were found within vessel lumina. A brown-black papule neighboring one of the macules histologically resembled compound type of melanocytic nevus, but the tumor cells compressed thinned epidermis with focal epidermotropism. Other papules, whose color ranged from light brown to reddish brown, showed metastatic tumor cells only in the dermis. In 3 of them, individual tumor cells were lined up along the collagen bundles, showing an ‘Indian-file’ arrangement. Such a combination of various histologic features in a case of intransit metastases of acral type of melanoma has not been reported in the literat
