Plasma and cutaneous zinc were studied in 22 psoriatic patients and 14 controls. The psoriatics were divided into three groups: 10 extensive psoriasis vulgaris, 5 palmoplantar psoriasis and 7 extensive pustular psoriasis. Low plasma zinc and decreased in vitro chemotaxis of polymorphonuclear leukocytes were found only in extensive pustular psoriasis, whereas zinc was not decreased but rather elevated in skin. This explains why conflicting results on plasma zinc have been previously reported in psoriasis and confirms that cutaneous zinc depletion is not a feature of psoriasis.

In order to examine the presence of intercellular substance antibodies (ICS Ab) which will fix complement in vitro, in the sera of patients with Brazilian pemphigus foliaceus (BPF), 7 BPF sera were tested using immunofluorescence (IF) and complement IF (CIF). All 7 sera possessed IgG ICS Ab at titers varying from 1/320 to 1/2,560 (normal human skin as substrates). Using CIF, 2 out of 7 sera proved to have a capacity to fix complement (C3) in vitro at a titer of 1/40. Although the titer of complement-fixing ICS Ab detected by CIF was lower than IgG ICS Ab detected by IF, these results with the evidence of frequent C3 deposition in the lesional skin of BPF suggest the necessity of further studies on the role of complement system in BPF.

A case of pyoderma gangrenosum is reported in a patient with a past history of ulcerative colitis. Serial C-reactive protein (CRP) concentrations were measured and were found to be markedly elevated at presentation. Levels were also raised during two subsequent relapses. CRP levels proved to be a useful objective indicator of disease activity and response to therapy, and reflected changes in disease activity before the erythrocyte sedimentation rate.

Thirty-three patients with frequently recurring genital herpes completed a randomized double-blind, crossover trial with oral acyclovir 200 mg 4 times a day and placebo for periods of 12 weeks. Five patients (15%) had full recurrence during acyclovir treatment and 31 (94%) while receiving placebo. The median time to first recurrence was 20 days for placebo and more than 84 days for acyclovir. It was concluded that acyclovir was well tolerated and an effective treatment to suppress the disease in selected cases of severe and frequently recurring genital herpes. However, the relapses seem to occur with the same rate as before, when the suppressive acyclovir treatment is stopped.

A 25-year-old woman developed urticarial lesions after visible light irradiation. Orally administered cimetidine significantly inhibited the induction of urticaria, but H1 antihistamines did not suppress the weal formation. Our case suggests that a trial with H2 antihistamine should be considered for the treatment of solar urticaria.

Long-term results of initial ketoconazole treatment were evaluated in 13 patients with chronic mucocutaneous candidiasis followed up for 3 years. Six of them shared 11 endocrine or autoimmune disorders. All patients responded well to treatment but 1 dropped out owing to anxiety. Clinical and mycological clearing was achieved in 1–2 months for the mouth, 2 months for the skin, 3–4 months for paronychiae and 4–14 months for the nails. Bacterial infections persisted in 2 cases. After withdrawal of ketoconazole, 2 patients have remained in remission. Continuous therapy was required in 2 cases and the others were treated intermittently owing to recurrences of Candida stomatitis. Ketoconazole was well tolerated. An asymptomatic reversible increase of serum hepatic enzymes occurred in 1 case. Cefotetan is a newly developed cephamycin especially resistant to bacterial beta-lactamase. Therefore both its in vitro activity against Neisseria gonorrhoeae and its clinical efficacy in uncomplicated gonorrhea are investigated. The minimal inhibitory concentrations (MIC) lie in the range of 0.016–2 μg/ml, 90% of the strains being inhibited by 0.5 μg/ml. Of 52 finally evaluable patients who were treated by a single intramuscular injection of 1 g, 48 (92.3%) were cured bacteriologically. We report a case of a bullous lichenoid eruption due to the intake of captopril. Clinical, histological, direct immunofluorescence and ultrastructural features were consistent with the diagnosis of lichen planus pemphigoides. In addition, the in vivo immunological study also revealed an intercellular fluorescence, similar to that seen in pemphigus. Complex drug-induced cutaneous reactions have been previously reported with other drugs, especially with D-penicillamine, which bears chemical similarities with captopril. However, such a drug-induced mixed pattern of lichen planus pemphigoides with pemphigus-like features has never been reported.
