Autoradiographic technique was utilized to measure UV-induced DNA repair synthesis in 8 patients with epidermodysplasia verruciformis: 2 infected with human papillomavirus 3 (HPV 3), 4 with HPV 4, and 2 simultaneously with HPV 3 and HPV 4. Two cell populations were compared, i.e. epidermal cells derived from light-exposed and covered skin, and peripheral lymphocytes. A control group consisted of 10 healthy subjects. DNA repair was found to be normal both in the peripheral lymphocytes and in the epidermal cells from exposed and unexposed skin. The values for covered skin were even somewhat higher but this was not statistically significant. The type of HPV infection and malignant conversion had no apparent effect on DNA repair. A certain reduction of the UV-DNA repair synthesis was demonstrated in 2 patients infected with both types of viruses.

The authors describe a new original observation of a D-penicillamine-induced pemphigus in a patient with rheumatoid polyarthritis. It begins in two steps: the first crisis of blisters having subsided after withdrawal of D-penicillamine, there is a recurrence 7 months later. Clinical and histopathological aspects of pemphigus vulgaris are discussed. Existence of intercellular antisubstance antibody in the serum and in the skin. Evolution identical to that of a classical pemphigus requiring an important corticotherapy. Review of the literature on the previous cases of D-penicillamine-induced pemphigus and other pemphigus attributed to the action of other drugs, namely pyritinol, rifampicine and practolol. Discussion of the pathogenesis.

A case of blistering eruption developing at the donor site of a split skin graft in a 48-year-old woman is described. Conventional histology revealed a subepidermal bulla. Direct immunofluorescence was negative. The significance of these findings is discussed.

66 psoriatics, 34 normal adults, 10 normal juveniles, and smaller groups of patients afflicted either with seborrheic dermatitis-rosacea or atopic dermatitis were examined for the threshold of capillary petechia formation on clinically normal forearm skin in response to repeated tape stripping. A definitely lowered petechial threshold was found in psoriatics (mean of 9 vs. 19 tape strips) and also in persons with the seborrheicrosacea complex. Normal juveniles and patients with atopic dermatitis had an increased threshold. The possible relation of these data are pointed out to recent findings of ultrastructural abnormalities of psoriatic papillary capillaries, and an altered capillary response is suggested as a sine qua non precondition for the development of psoriasis. The petechial threshold test is recommended as a simple adjunct diagnostic tool.

2 patients with widespread porokeratosis Mibelli (PM) were treated orally with RO 10-9359. The dose was 75 mg/day for 10 days, then 50 mg/day for 3 weeks. The drug produced a good improvement of condition with no serious side effect. Ultrastructural examination of a healed lesion showed the presence of a fine granular substance in the intercellular space of the spinous layer, most likely produced by keratinocytes; ultrastructural cellular alterations of PM were still evident and the lesion recurred after suspension of treatment.

6 cases with acute febrile neutrophilic dermatosis (Sweet’s syndrome) responded rapidly and dramatically to treatment with potassium iodide. All patients became afebrile and symptom-free within 24–48 h after therapy. The cutaneous eruptions subsided completely in 3–5 days. 5 of the patients received the drug only for 2 weeks, but they have had no recurrences. A 57-year-old man with skin psoriasis and psoriatic arthropathy, in whom an IgG-K monoclonal immunoglobulin was identified, is reported. A 7-year follow-up did not disclose any evidence of malignancy. The significance of this unusual occurrence is described.
