A case of annular pustular psoriasis is described. The annular zone of the lesion provides a model for studying the dynamics of intraepidermal accumulation of polymorphonuclear leukocytes in psoriasis. It could be shown that the unstimulated accumulation of polymorphonuclear leukocytes was limited to the clinically involved skin. The appearance of polymorphonuclear leukocytes proved to be associated with acanthosis. The micropustule formation following topical application of leukotriene B4 was markedly decreased in lesional, prelesional and postlesional skin, compared to the response in the clinically uninvolved skin of patients with chronic plaque psoriasis. Although leukotriene B4 might have an initiating role in the intraepidermal accumulation of polymorphonuclear leukocytes other factors are a conditio sine qua non for the peripheral spreading of the lesion in annular pustular psoriasis.

A case of cutaneous malakoplakia in a 44-year-old woman under immunosuppressive therapy after renal transplantation for nephrosclerosis is reported. A nodular lesion with inflammatory signs appeared on her right buttock. Histopathology showed a dermal infiltrate with histiocytes containing Michaelis-Gutmann bodies. Electron microscopy showed Michaelis-Gutmann bodies with concentric laminar structure (‘target’ appearance). Tissue culture revealed Escherichia coli growth. Erythema nodosum and IgA nephropathy are nosologic entities with an immunologic background. IgA nephropathy is a specific disease of the kidneys, while erythema nodosum may be associated with other illnesses. We report a unique case in which intermittent hematuria due to IgA nephropathy was superimposed on recurrent attacks of erythema nodosum and arthritis. A common pathophysiology is suggested.

A 42-year-old woman was observed during 3 bouts of eosinophilic cellulitis over a 6-year-period. Skin biopsies were taken at each relapse and processed for histological, immunofluorescent and ultrastructural studies. Histologically the eosinophilic infiltrate extended to the deep dermis and the subcutaneous fat. High levels of circulating immune complexes, and complement and IgG deposits around the vessels were detected for as long as the cutaneous lesions lasted. Under the electron microscope eosinophils were numerous, half of them degranulated and some granules had a double cristal core. No injury to the vessel walls was observed. The 3 recurrences occurred respectively after lincomycin, nesdonal, acetyl salicylic acid and pholcodin ingestion and responded to sulfone and steroid therapy.

We report the 2nd case of titanium balanitis reported in the world literature. Clinically the patient presented with phimosis: histologically the titanium was surrounded by a mixed granulomatous inflammatory reaction. Topical titanium preparations may lead to phimosis and therefore we thought it important to bring this potential iatrogenic complication to further notice.
