A patient developed an Addison-like melanoderma over the past decade. An abnormal neuropeptide related to, but bigger than pro-opiomelanocortin was found in large amount in the serum. It has most likely a stimulative action upon melanogenesis with formation of macromelanosomes.

We have previously found an important increase of cellular retinoic acid-binding protein (CRABP) in psoriatic plaques whereas the cellular retinol-binding protein (CRBP) was not elevated compared to normal human skin and nonlesional psoriatic skin. In the present study we analyzed CRABP and CRBP levels in a panel of dermatoses in order to address several questions raised by the above findings. Three observations were made: (1) CRBP showed little or no variations whereas CRABP was either normal (seborrheic keratosis, lichenification, nonlesional psoriatic and nonlesional Darier disease skin) or elevated (psoriatic plaques, lamellar ichthyosis, lesional Darier disease, pityriasis rubra pilaris, keratosis pilaris); (2) high levels of CRABP might indicate a greater sensitivity of the lesions to systemic synthetic retinoids with a carboxyl group in the C15 position, and (3) systemic administration of etretin increased the levels of CRABP but not CRBP. These observations suggest that CRABP might be the receptor for synthetic retinoids in the skin and that its analysis might be useful in monitoring retinoid therapy.

A 75-year-old man developed three lesions of Paget’s disease which involved the pubic area and both axillae. Histologically, there was no invasion into the dermis in any of the three lesions. Various kinds of histochemical and immunohistochemical studies including those for carcinoembryonic antigen showed that both genital and axillary lesions were identical in nature. Three patients with acute generalized exanthematous pustular dermatitis (AGEPD) are presented. This new clinical entity comprises generalized erythema, erythema multiforme, amicrobial pustules, fever and spontaneous healing over a 10-day period. Histologically, there are a mixed neutrophilic and eosinophilic vasculitis and an eosinophilic subcorneal and spongiform pustule. All the patients experienced pharyngitis 1 week before the onset of the rash. Two of them had high-titered IgM-neutralizing antibodies to enteroviruses and 1 had an enterovirus in stool and urine. AGEPD may be related to a viral infection and may occur in patients with a genetic predisposition to psoriasis. The diagnosis of AGEPD should be made early enough so that the patients are not given the often aggressive therapy of pustular psoriasis.

We report the case of a 66-year-old man who developed a symmetrical peripheral gangrene (SPG) on both feet during disseminated miliary tuberculosis. Mycobacterium tuberculosis was not only isolated from sputum, gastric juice, and urine, but also from a blood culture. Since a disseminated intravascular coagulopathy was very likely to be excluded, it is possible that the SPG was due to embolization of arterioles by the tubercle bacilli. The literature of SPG is reviewed and the different possible pathogenic mechanisms of SPG, such as hypoxemia, vasoconstriction, primary endothelial damage, and vascular obstruction, are discussed. SPG is another cutaneous manifestation of disseminated tuberculosis.

A 23-year-old woman suffering since 4 years from Prurigo pigmentosa is presented. Histology from an early lesion shows neutrophilic infiltration of epidermis and dermis. Treatment with Dapsone and potassium iodide was effective but failed to prevent recurrences.
