Topical corticosteroids are utilized in the treatment of a wide variety of skin diseases, primarily those involving an inflammatory component. Recent investigations have revealed that one of the effects of long-term usage of steroids is the depletion of skin mast cells. This led to the treatment of patients with urticaria pigmentosa with topical high potency corticosteroids for 6 weeks. At the end of treatment there was a marked reduction in tissue histamine and an absence of mast cells as well as a disappearance of pruritus and Darier’s sign. Treated areas remained clinically improved for at least 9–12 months. Recent findings in a family with X-linked recessive ichthyosis are presented. The first description of this family in the literature was given and correctly diagnosed by Csörsz in 1928. His paper can be considered one of the most widely cited proofs of the existence of X-linked ichthyosis. The extended pedigree as well as data of steroid sulfatase and arylsulfatase C determinations presented in this paper verify the diagnosis of the X-linked mode of inheritance of ichthyosis in this family. The biochemical investigations carried out on leukocytes of family members resulted not only in a confirmation of the clinico-genetic diagnosis, but they also helped to establish the heterozygous genotype of a female mentioned previously as an affected person

Chronic renal failure (CRF) interferes with the catabolism of retinol and is frequently associated with hypervitaminosis A. The effect of CRF on the plasma levels of etretinate (Tigason®, Tegison®), an aromatic retinoid, was therefore studied in 4 patients who were given the drug as part of a maintenance antipsoriatic regimen. The concentrations of the parent compound and its main metabolites (etretin and isoetretin) were monitored for 24 h after receiving a routine dose of the drug. In comparison with 4 non-CRF patients receiving similar maintenance doses of etretinate, although the CRF patients had higher peak levels of the parent compound (p < 0.01), the levels of the metabolites were similar or lower. The plasma half-lives for all compounds were approximately the same in each group, indicating that the catabolism of the drug was not affected by CRF. The phototoxic potential of tenoxicam, a new non-steroidal anti-inflammatory drug (NSAID), was investigated following oral and intradermal administration of the drug. No hypersensitivity responses to ultraviolet radiation (UVR) were observed for either orally or intradermally administered tenoxicam. In a parallel in vitro study, human polymorphonuclear leucocytes (PMNL) and mononuclear leucocytes (MNL) were exposed to UVR in the presence and absence of the NSAIDs tenoxicam, piroxicam and benoxaprofen (10–40 μg/ml). The generation of reactive oxidants by control and UVR-treated human PMNL and MNL in the presence and absence of the NSAIDs was measured according to the extent of luminol-enhanced chemiluminescence. Only benoxaprofen activated reactive oxidant generation by PMNL and MNL. The results of these in vitro and in vivo studies show that phototoxicity is unlikely to complicate chemotherapy with tenoxic

A 69-year-old man with sulfone-resistant subcorneal pustular dermatosis was successfully treated by etretinate, 1 mg/kg body weight for 5 days, followed by 0.75 mg/kg body weight for 2 months. This confirms the possible usefulness of systemic retinoids in nonpsoriatic pustulosis.

A patient suffering from generalized granuloma annulare associated with mild hypothyroidism due to an atrophic autoimmune thyroiditis is presented. Treatment with thyroxine resulted in the restauration of a euthyroid state. Subsequently disappearance of the antithyroid antibodies along with a progressive decrease in the number of the skin lesions was noticed. The association between generalized granuloma annulare and autoimmune thyroiditis has been reported once previously in the literature, but it might be of more frequent occurrence.

We present a case of junctional epidermolysis bullosa (EB) associated with urethral tract involvement. He was a 27-year-old man who had symblepharon of the right eye, hoarseness and urethral stricture in addition to generalized blister formation. Ultrastructural examination of a biopsy skin specimen demonstrated the characteristic morphology of junctional epidermolysis, a form of EB that has not been previously associated with urethral stricture.

A patient developed successively acrodermatitis chronica atrophicans (ACA) and localized scleroderma. ACA was demonstrated to be a tick-borne disease, whereas morphea is only suspected to be one. As morphea appeared after successful treatment of ACA, it suggests that localized scleroderma is not directly an infectious disease but might be rather the consequence of spirochetal infection.
