In 8 patients with systemic lupus erythematosus (LE) and 23 with chronic LE, the kidneys were examined by electron microscopy after percutaneous punction. In all cases with systemic LE and LE chronicus cum exacerbatione subacuta and in two thirds of patients with localized or disseminated variants of chronic LE we found inclusions of microtubuli in the endothelial cells of glomerular, interstitial and peritubular capillaries, which partly were interpreted as (paramyxo)-virus-like particles, partly as reaction products of the cell induced by viruses. The microtubuli were observed independently of the degree of the clinical and histological manifestation of the disease of the kidney, also of the occurrence of antinuclear antibodies and of the performed pretreatment, also in clinically and histologically normal findings. The microtubuli are not specific for LE, but a diagnostic aid in chronic LE too. Its occurrence possibly explains a common pathogenesis of systemic and chronic LE, which may be differentiated by the genetically determined pattern of the reaction of the immune system and which allows transformations from one to the other variant of LE.

The incidence of contact-allergic reactions to some 10 substances out of 75 tested in 100 selected leg-ulcer patients is about 5 times as high as in eczematous patients. These substances are p-phenylenediamine, p-aminoazobenzene, Pellidol®, benzocaine, diaminodiphenylmethane, neomycin, coal tar, balsam of Peru, wool alcohols. Sesame oil was the tenth substance. It is used to clean the skin from rests of ointments. In Holland it figures as a constituent of the often-used, so-called pasta zinci oleosa, ung. diachylon, ung. simplex, and of emulgide cream. In our opinion, not only topical medicaments should be regarded as frequent contact sensitizers in leg-ulcer patients. A second class of important sensitizers in this group of patients appears to be the para-substituted substances such as p-phenylenediamine (and azo dyes) as inducers considered to be responsible for the cross-reaction of drugs such as benzocaine and sulfanilamide, and of the rubber additive, diaminodiphenylmethane. Stress is laid upon five preventive measures concerning contactants in leg-ulcer patients (stockings, bandages, topical medications) and upon the desirability of having non-sensitizing preservatives and stabilizers at one’s disposal–especially for topical medications in leg-ulcer patients who apparently are sensitized with greater ease than patients with other skin ailm

10 of 16 (62.5%) patients with granuloma annulare presented microangiopathic changes in the skin. The incidence in control groups was 100% in diabetic patients, 40% in old-age patients not known as diabetics and 0 in young victims of road accidents. The frequency of the vascular findings does not seem to be shared equally by the two types of granuloma annulare, the generalized and the localized type. In the first, as in the diabetic group, microangiopathy was observed in all patients examined. Whereas, in the second it was detected in only 2 of the 8 patients. It is concluded, therefore, that from the point of view of their relation to a diabetic state, the two forms of the disorder differ from each other.

A second patient with erythropoietic protoporphyria (EPP) from India is reported. Unusual features in this patient were high degree of photosensitivity and marked hypertrichosis. The probable cause of hypertrichosis and its relationship to sunlight is discussed.

Fox-Fordyce disease is a relatively uncommon dermatosis classically occurring in females at the time of puberty and very resistant to treatment. We are reporting the cases of two women who developed this disease in the post-menopausal period and were treated with electrocoagulation of the lesions. Both these patients have been symptom-free for the last year. This method is simple and is therefore recommended for further trial.

The normal ranges of vitamin A (retinol) and carotene were investigated in 31 healthy subjects. In 3 patients suffering from hyperkeratosis follicularis et parafollicularis (Kyrle’s disease), pityriasis rubra pilaris, and keratosis follicularis (Darier’s disease), the serum level of vitamin A was found to be either beneath or in the lower part of the normal range. Striking improvement has been noted when these patients were treated systemically with retinoic acid (vitamin A acid) which was associated with elevation of serum levels of vitamin A and carotene. In ten cases of angioma senile and in three cases of nevi nevicellulares, the authors found an encirclement of the tumor tissue by epithelial cells arising mainly from the follicular walls. The proliferating epithelial cells led to complete rejection of the tumor in six cases of angioma and in three cases of cellular nevi. The process of rejection of the tissue is considered to be of ideological nature.
