Persistent ashen-gray maculae 5–7 mm in diameter and freckles developed after prolonged PUVA therapy of psoriasis. Histopathology of maculae showed extensive collections of melanophages in the upper dermis. The melanocytes were strongly stimulated at the site of the freckles. The number of melanosomes, mitochondria, endoplasmic reticulum and dendrites were increased. A study on carbohydrate metabolism (glycaemia and insulinaemia curves during the oral and the intravenous glucose tolerance tests) in 20 porphyria cutanea tarda patients revealed diabetes mellitus in 2 and impaired glucose tolerance in 1 patient, but the carbohydrate tolerance did not differ greatly from that of matched controls. The porphyria cutanea tarda patients, instead, had a significantly exaggerated insulin response, which was more marked in patients with liver disorders. It is proposed that the hyperinsulinism is secondary to the liver damage.

The difficulty of differentiating an epithelial skin tumour with partial adenoid histological features from a metastasis of an internal adenocarcinoma is exemplified by a case report of an 83-year-old woman. Histological, histochemical and ultrastructural results led us to suppose that two different carcinomas with metastases were present: an adenocarcinoma of the colon and a ‘unicellular muciparous squamous cell carcinoma of the skin’. Carprofen is a new non-steroidal anti-inflammatory drug and a phenylpropionic acid derivative. It seldom causes pruritus, erythema or urticaria. Since 1 year, we have observed 8 photosensitive eruptions associated with Carprofen, a surprising side effect for this group of derivatives. The skin eruption started from 4 days to 3 weeks after the initiation of the treatment and was always limited to the exposed areas of the skin. The eruption was erythematopapular, but also vesiculous or bullous. 2 cases required systemic corticosteroids. Photo patch tests were positive, suggesting a photoallergic process. Phototherapy should be avoided in patients treated with carprofen.

3 cases of reactive perforating collagenosis in siblings are reported which were characterized clinically by typical umbilicated papules at sites exposed to traumatism. The histopathological feature is characterized by a cup-shaped depression of the epidermis together with extrusion of collagen fibers at its base.

RePUVA treatment proved to be more efficient than simple oral photochemotherapy in a patient suffering from actinic reticuloid. This combined therapy may solve some problems of long-term treatment in actinic reticuloid.

59 patients with pityriasis amiantacea (PA) were followed up after 8 years on average. 15% had suffered from psoriasis and 19% were aware of psoriasis in their relatives. The incidence of psoriasis was significantly higher (p < 0.01) than in the Scandinavian population, and the predisposition to psoriasis was significantly higher than in the population (p < 0.001), but significantly lower (p < 0.01) than in psoriatic patients. PA seems to be a manifestation of psoriasis in many cases.

A female patient with porphyria cutanea tarda (PCT) clinically cured by a low-dose chloroquine treatment showed neither exacerbation of the disease nor an increased excretion of urinary porphyrins during pregnancy. The role of estrogens and gestagens as clinical manifestation factors of PCT is discussed. According to our observation the porphyrogenic effect of the estrogens seems to have been overestimated.

Only little is known about the involvement of the epithelium of epidermoid cysts in dermatoses with keratinocytic changes because these cysts are excised only rarely. Histological findings on an epidermoid cyst, excised from a patient with wide-spread lichen planus are reported. These changes exactly agree with the epidermal histological features of lichen planus and cannot be interpreted as a foreign body reaction.
