In this study, the in situ immunological typing of cell populations in lichen planus was attempted. T lymphocytes and suppressor/cytotoxic subsets, B lymphocytes, macrophages, immunocytes and Langerhans’ cells were studied by one or more technical parameters and semiquantitative assessment of T cell populations were carried out. The resistance to traction of the dermo-epidermal junction and of the superficial vascular plexus is altered by oral photochemotherapy. The dermo-epidermal junction is weakened when the resistance of vascular walls increases. These modifications are related to the histological alterations occurring during photosclerosis induced by PUVA.

Macroscopically papillomatosis confluens et reticularis (PCR) is a typical dermatosis with three different clinical forms. Histologically the characteristics are orthokeratosis, papillomatosis and hyperpigmentation. A case report of PCR and diabetes mellitus reveals further histological factors, these being interstitial and intracellular edema in the epidermis and a homogenization of collagen and rarefication of elastic fibres. Ultrastmcturally intracytoplasmic lipid vacuoles in the basal and suprabasal squamous cell layer were a prominent feature. These findings could be demonstrated in normal and PCR skin, which until now has not been described in the skin of diabetes mellitus patients. The question arises if PCR can be interpreted as a symptom of a fat metabolism disease in which unknown additional factors may play a role.

Plasma concentrations of 8-MOP were studied in 6 subjects at three dose levels and the minimal phototoxic dose determined after 60, 120 and 180 min. Although there is an inverse correlation between these two values, none of the eight mathematical expressions examined gave a perfect fit in all cases.

10 subjects were assessed using the vasoconstrictor assay technique in a double-blind study in order to evaluate the relative potencies of several diluted and undiluted proprietary corticosteroid preparations. Dermovate ointment achieved a significantly higher score than any other preparation but there was no significant difference between the scores for Betnovate ointment (betamethasone valerate 0.1%), Propaderm Forte cream (beclomethasone dipropionate 0.5%), Propaderm ointment (beclomethasone dipropionate 0.025%), Nerisone Forte ointment (diflucortolone valerate 0.3%), and Nerisone ointment (diflucortolone valerate 0.1%). Furthermore, no significant difference in scores could be demonstrated between Adcortyl ointment (triamcinolone acetonide 0.1%), Ledercort ointment (triamcinolone acetonide 0.1%) and extemporaneous dilutions of these ointments 1 part in 4 in their recommended diluents (triamcinolone acetonide 0.025%). The relevance of these findings to clinical practice is discussed.

An 80-year-old woman with eosinophilic fasciitis (EF) is presented. Besides symptoms indicative of EF, the patient expressed features of other autoimmune disease with elevated ANA, anti-DNA, serum-aldolase and thyroid autoantibodies. Glucocorticoid treatment resulted in rapid improvement of laboratory parameters and after 6 months a clinical improvement was also observed.

A 33-year-old woman developed pruritic lesions on both legs. There were brownish-red keratotic papules, whose biopsy specimen showed acantholysis and benign dyskeratosis.
