In a series of 100 adults with tinea faciei various dermatologic manifestations were mimicked, viz.: discoid lupus erythematosus in 52 patients, lymphocytic infiltration in 15, seborrheic dermatitis in 11, rosacea in 8, contacts dermatitis in 7, polymorphous light eruption in 4 and granuloma faciale in 3. Trichophyton rubrum was isolated in 78% of the respective cases, other isolated organisms being Microsporum canis (13 cases), Trichophyton violaceum in 6, Trichophyton mentagrophytes (asteroides) in 2 and Epidermophyton floccosum in 1 case. In 85% of these patients the nails were also involved by the same agent found in the lesions of the face. On the basis of these observations it is recommended that all adult patients with tinea faciei should undergo a comprehensive mycological investigation to find the primary focus, which may be an infected nail.

A ‘Minolta Tri-Stimulus Colorimeter II’ was evaluated for obtaining objective measurements of early changes in erythema and tanning. The meter showed a subtle, continuous transition between the primary erythematous response and the delayed tanning of skin which was below the visual threshold for detection. Thereafter, the a* (redness) value of the meter showed a significant linear correlation with the dermatologist’s perception of erythema while the b* (yellow) value showed a significant correlation with the perception of tanning. This capability of the tri-stimulus colorimeter to simultaneously evaluate the hue and saturation of skin color affords an improved opportunity to quantitate the transition from erythema to tanning without subjective

The prevalence of nonmelanocytic skin cancers has been evaluated from histologic data related to the population of the Mosan region of Belgium. 2,538 neoplasms were identified over a period of 5 years. The collected data are in opposition with those provided by the national cancer registry of Belgium. Nonmelanocytic skin cancers represent at least 15 % of the total number of cancers. The relative risk to develop a given malignant cutaneous neoplasm rather than another is constant until 80 years of age. The age distribution of the rates of estimated prevalence is different between keratoacanthomas on the one hand and basal and squamous cell carcinomas on the other hand.

A retrospective study of 23 cases of nonlymphoblastic T cell lymphomas with no features of either mycosis fungoides (MF) or Sézary syndrome (SS) was carried out. 6 patients exhibited cutaneous (or mucous) lesions and are presented herein. There was no detectable visceral or node involvement except in 1 patient. 3 cases were immunologically proven to be of peripheral T cell origin. Except T cell lymphomas in Japan and in the Caribbean area, the frequency of prevalent skin involvement is reportedly very low in non-MF/SS non lymphoblastic T cell lymphomas. However, it is likely that this frequency has been underestimated in Western countries. The histologic features of our cases run the heterogeneous spectrum of recently described node-based forms of peripheral T cell lymphomas: ‘angioimmunoblastic lymphadenopathy with dysproteinemia’ -like lymphoma, large T cell lymphoma, multilobulated cell lymphoma, T zone lymphoma. The histologic pattern of skin infiltration was different from that of MF/SS. One case was initially suggestive of a pseudolymphoma and had a prolonged evolution but the other patients suffered from rapid extracutaneous involvement and fatal evolution with resistance to treatment. Cutaneous plasmocytomas were seen in 1 case. A double-blind, crossover trial with a new triprolidine derivative, acrivastine (BW 825C; 8 mg 3 times daily), cyproheptadine (4 mg 3 times daily) and placebo was carried out in 18 patients suffering from idiopathic cold urticaria. Acrivastine and cyproheptadine significantly (p < 0.01) reduced weal areas following ice cube challenge when compared to placebo. Acrivastine was found to be significantly more effective (p < 0.01) than cyproheptadine in reducing weal areas. Furthermore, cyproheptadine caused significantly more drowsiness than acrivastine (p = 0.021) or placebo (p = 0.013), which did not differ from each other. This study shows that acrivastine is an effective agent in the treatment of cold urticaria and suggests that acrivastine in the dose used lacks adverse effects, such as drowsiness, traditionally associated with antihistamine therapy.

A randomized, double-blind clinical trial was undertaken in 41 patients with dermatomycosis to compare topical 2% fenticonazole cream (group A: 21 patients) with topical 1% bifonazole cream (group B: 20 patients). Treatment was performed as a once daily application. Mycological and clinical parameters were assessed before treatment and after 7, 14, 21 and 28 days. At the control visits the clinical investigator also expressed an overall judgement on the patient’s state of disease. This parameter was based on a combined clinical and mycological assessment by the physician; laboratory screening investigations were undertaken before and at the end of treatments. All patients were checked for their state of disease 3–4 weeks after the end of treatment. All assessment criteria showed fenticonazole to be at least as efficacious as bifonazole. Several trends in favor of fenticonazole were also found: fenticonazole achieved superior results in the overall clinical evaluation, and after 3 weeks of treatment 15 patients out of 21 on fenticonazole were cured in mycological and clinical terms, whereas treatment with bifonazole resulted in complete healing of only 7 patients out of 20. This difference is statistically significant (p = 0.021) and indicates a more rapid therapeutic activity of fenticonazole. At the posttreatment rechecks no recurrent disease was registered, irrespective of whether patients had received fenticonazole or bifonazole. Laboratory screening investigations revealed no evidence of significant treatment-related changes or abnormalities in both treatments. No adverse events were noted for either treatm

62–65% reduction in the number of lesions was obtained in the treatment of 52 patients with acne vulgaris in this randomized single-blind study comparing a 5% benzoyl peroxide/2% miconazole cream (BPO-MCZ) with a 5% benzoyl peroxide gel (BPO). We report a case of Candida albicans skin abscess, which developed on the left knee of a 59-year-old male, who had been bedridden for the past 2 months because of subarachnoid haemorrhage. The route of the infection was considered to be an intravenous catheter that had been inserted into the left great saphenous vein at the medial malleolus. The prolonged systemic administration of antibiotics and corticosteroids, as well as prostration, were suspected to be the factors that led to the infection. The patient was successfully treated with intravenous miconazole and subsequent high-dose oral amphotericin B therapy.

Six cases of isolated aplasia cutis congenita of the scalp among family members over three successive generations are described suggesting an autosomal dominant inheritance. Five family members were examined in our clinic while one member is abroad. A review of the literature concerning isolated congenital aplasia cutis of the scalp in three or more generations is done.
