To gain some idea of cervical screening in Otago and Southland (two southern provinces of New Zealand), before the implementation of a national cervical cancer prevention programme, a questionnaire was sent to all doctors identified as smear takers. Questionnaires were returned by 75% of the doctors. Only 53.8% of practice nurses were involved in taking smears. Well Women Clinics staffed by nurses were not widespread in either province. Just under half (46.4%) of doctors took cervical smears as part of a health check programme. Sixty‐three per cent (63.3%) routinely performed a pelvic examination when taking a smear. Surprisingly few doctors were familiar with the technique of placing spatula and cytobrush samples on one slide, a manoeuvre which could significantly reduce the workload of the cytology laboratories. Only a minority of doctors were aware of a new sampler which can provide samples equivalent to combined spatular‐cytobrush smears. There was confusion over the timing of a repeat smear following two normal ones. the recommended interval of 3 years was only complied with by 44.2% of doctors. However, 71.4% of respondents did remind women to have smears. the study indicates that many doctors already provide patients with a good cervical screening service, and this augers well for the national screening programme.Afin d'abtenir des informations sur le dépistage du cancer du col utérin dans les provinces d'Otago et de Southland (2 provinces du sud de la Nouvelle Zélande), préalablement au lancement d'un programme de prévention, un questionnaire a été envoyéà tous les médecins susceptibles de réaliser des frottis. Ces questionnaires ont été renvoyés par 75% d'entre eux. Seulement 53.8% des infirmiéres sont concernées par la réalisation des frottis. Les ‘Well Women Clinics’dirigées par des infirmiéres ne sont pas très répandues dans ces deux provinces. Moins de la moitié des médecins (46.4%) font des frottis dans le cadre d'un programme de bilan de santé. Une confusion a été constatée à propos de l'intervalle recommandé après 2 frottis normaux. Les médecins se sont conformés à l'intervalle recommandé de 3 ans pour 44.2% d'entre eux. Cependant 71.4% des médecins répondeurs rappellent à leurs patientes qu'elles doivent faire pratiquer des frottis. Cette étude montre que de nombreux médecins proposent une bonne utilisation du dépistage du cancer du col à leurs patientes, signe de bonne augure pour le programme national de dépistage.Um die Lage hinsichtlich eines gynäkologischen Screenings in den beiden südlichen Provinzen von Neuseeland, Otago und Southland, zu erfassen, wurden Fragebogen an alle Ärzte verschickt von welchen bekannt wer, daß sie Abstriche herstellen. 75% der Fragebogen wurden beantwortet. Nur in 53.8% der Praxen waren die Schwestern mit der Herstellung der Abstriche betraut. In beiden Provinzen gab es kaum ‘Well Women Clinics’. Etwas weniger als die Hälfte (46.4%) der Ärzte fertigen Cervikalabstrich im Rahmen eines Gesundheitscheques an, 63% (63.3%) von diesen führte routinemäßig gleichzeitig eine Beckenuntersuchung durch. Überraschenderweise waren nur wenige Ärzte mit der Technik Spatel‐und Cytobrushmaterial auf einem einzigen Objektträger aufzutragen vertraut, nur eine Minderheit kannte neue Verfahren, die dem kombinierten Spatel‐ und Cytobrushabstrich entsprechen. Es herrschte Unklarheit hinsichtlich der Kontrollzeiträume nach 2 normalen Abstrichen. Der empfohlene Zeitraum von 3 Jahren war nur 44.2% der Ärzte bekannt. The presence of foamy alveolar casts or flocculent material in Papanicolaou and Leishman‐stained smears of bronchoalveolar lavage (BAL) fluid is said to be indicative of infection withPneumocystis carinii.We have investigated the sensitivity and specificity of this method of diagnosing pneumocystis pneumonia in patients with the acquired immunodeficiency syndrome (AIDS). Patients (n= 114) with diffuse lung infiltrates were submitted to fibreoptic broncoscopy and BAL. Seventy of them were patients with AIDS. the other 44 individuals were not infected by the human immunodeficiency virus (HIV).Pneumocystis cariniiorganisms were identified on Grocott's methenamine silver (GMS)‐stained BAL smears in 30 patients with AIDS. Flocculent material was present in the Papanicolaou and Leishman‐stained smears from all of these cases. Conversely,P. cariniiwere not seen on GMS‐stained smears in the remaining 84 individuals with or without AIDS. No flocculent material was observed in Papanicolaou or Leishman‐stained smears in these 84 patients. We concluded that the presence of flocculent material in Papanicolaou or Leishman‐stained smears of BAL fluid is indicative ofP. cariniipneumonia in patients with AIDS.La présence de cylindres alvéolaires spumeux ou de matériel floculé dans les étalements de liquide de lavage bronchoalvéolaire (LBA) colorés selon Papanicolaou ou Leishman est considérée comme symptomatique d'une infection parPneumocystis carinii.Nous avons étudié la sensibilité et la spécificité de cette méthode de diagnostic de l'infection par Pneumocystis carinii chez des patients atteints de syndrome de déficience immunitaire acquise (SIDA). Cent quatorze malades avec des infiltrats pulmonaires diffus ont subi une fibroscopie bronchique et un lavage broncho‐alvéolaire. Soixante dix d'entre eux edtaient atteints de SIDA, 44 n'étaient pas infectés par le Virus de l'Immunodéficience Humaine (VIH). LePneumocystis cariniia été identifiié par la coloration de Grocott chez 30 patients atteints de SIDA. Chez ces patients, la présence d'un matériel floculé est constante sur les étalements colorés au Papanicolaou et au Leishman. A l'inverse,Pneumocystis cariniin'a pas été retrouvé chez les 84 autres malades, atteints ou non du SIDA et les étalements de LBA ne contenaient pas de matériel floculé. The effect of filtration through two layers of cotton gauze on the cellular composition of bronchoalveolar lavage (BAL) fluid was studied in 25 patients with pulmonary disease. There was no significant difference between median total cell count in unfiltered and filtered BAL (P= 0.73) or in the distribution of neutrophils or eosinophils. the percentage of bronchial epithelial cells was significantly higher in unfiltered than in filtered BAL (P= 0.02). Furthermore, differential cell counts showed a significantly lower percentage of alveolar macrophages (P= 0.04), and a significantly higher percentage of lymphocytes (P=0.04) in unfiltered compared with filtered BAL. Thus, gauze filtration results in a loss of bronchial epithelial cells and lymphocytes, and is not recommended in the routine analysis of cellular components in BAL fluid.L'effet de la filtration à travers deux couches de gaze de coton sur la composition cellulaire du liquide de lavage broncho‐alvéolaire (LBA) a étéétudié chez 25 patients atteints d'une pathologie pulmonaire. Aucune différence significative entre les échantillons filtrés et non filtrés n'a été trouvée en ce qui concerne la numération cellulaire totale moyenne (P=0.73) et la distribution des polynucléaires neutrophiles et eosinophiles. Le pourcentage de cellules épithéliales bronchiques est significativement plus élevé dans les échantillons non filtrés que dans ceux qui ont été filtrés (P= 0.02). Par ailleurs, le comptage differentiel montre un pourcentage plus faible de macrophages alveolaires (P=0.04) et un pourcentage plus élevé de lymphocytes (P= 0.04) dans les LBA non filtrés. La filtration à travers la gaze a pour résultat une perte des cellules épithéliales bronchiques et des lymphocytes et n'est donc pas à recommander pour l'analyse de routine des composants cellulaires du liquide de lavage broncho‐alvéolaire.Die Auswirkung einer Filtration von BAL‐Material durch doppelte Baumwollgaze wurde am Material 25 Patienten untersucht. The epidermal growth factor receptor (EGFr) status and the vimentin (V) status of malignant cells in pleural fluids from patients with breast cancer were determined using an immunoperoxidase labelling technique. the results were correlated with the oestrogen receptor (ER) and the progesterone receptor (PR) status of the primary tumour and with disease‐free survival time of the patient. A negative correlation between EGFr and V status and hormone receptor status was found. the longest mean survival time occurred in patients with negative EGFr and V status and positive hormone receptor (ER and PR) status. the shortest mean survival time occurred in patients with positive EGFr and V and negative ER and PR status.Le récepteur du facteur de croissance de l'epiderme (EGFr) et la vimentine (V) ont étéétudiées par une technique d'immunopéroxydase, au niveau des cellules malignes des épanchements pleuraux de malades traités pour cancer du sein. Les résultats ont été corrélés avec les récepteurs d'oestrogènes (ER) et de progestérone (PR) et avec la durée de survie sans récidive. Une corrélation négative est trouvée entre le récepteur de l'EGF, la vimentine et le taux des récepteur hormonaux. La survie moyenne la plus longue est observée chez des patientes négatives pour le récepteur de l'EGF et pour la vimentine et positives pour les récepteurs hormonaux (ER et PR). La plus courte survie est associée a la positivité du récepteur de l'EGF et de la vimentine et à la négativité de ER et PR.In Pleuraergüssen von Patientinnen mit Mammakarzinom wurden der Rezeptor für den epidermalen Wachstums‐faktor (EGF) sowie Vimentin mit der Immunperoxydase‐ Technik untersucht. Die Ergebnisse wurden mit dem Vorliegen von Östrogen‐ und Progesteronrezeptoren im Tumor sowie dem rezidivfreien Intervall der Patientinnen korreliert. Eine negative Korrelation bestand zwischen EGF‐Rezeptor und Vimentin. Die längste durchschnittliche Überlebenszeit wurde dagegen bei Patientinnen mit negativem Ergebnis für den EGF‐Rezeptor und Vimentin jedoch positiven Hormonrezeptoren gefunden. The cytological features of 18 myxoid liposarcomas from 12 patients were analysed. the most consistent cytological features were: abundant myxoid matrix, several small tissue fragments with uniform small, round or stellate cells and a network of branching thin‐walled capillaries. the presence of univacuolated or multivacuolated lipoblasts was noted frequently, but not in all aspirates. the diagnostic importance of vacuolated lipoblasts and the main problems of differential cytodiagnosis are discussed.Une série de 18 cytoponctions provenant de 12 patients chez qui un diagnostic de liposarcome myxoïde a été porté cytologiquement et histologiquement est analysée. Les caractéristiques cytologiques les plus constantes sont la présence d'une matrice myxoïde abondante, de petits fragments tissulaires constitués de cellules rondes ou étoilées assez monomorphes et d'un réseau branché de capillaires à paroi fine. La présence de lipoblastes mono ou pluri‐vacuolisés est fréquente mais non constante. L'importance diagnostique de ces lipoblastes vacuolisés et les problèmes majeurs de cytodiagnostic différentiel sont discutés.Feinnadelpunktate wurden von 18 histologisch gesicherten myxoiden Liposarkomen von 12 Patienten hergestellt. To investigate interobserver variation of fine needle aspiration (FNA) cytological diagnosis with respect to distinguishing between carcinoma, sarcoma and lymphoma, a set of 80 randomly sampled slides was randomized twice and read ‘blindly’by five cytopathologists. In the first round the slides were read without any information, and in the second round clinical information was provided. Histology was used as a reference standard. In the first round, the positive predictive values for the cytological diagnosis of carcinoma, sarcoma and lymphoma were 93%, 94% and 86% respectively. In the second round the positive predictive values for the cytological diagnoses of carcinoma, sarcoma and lymphoma were 95%, 99% and 99%, respectively. Interobserver variability, tested with weighted kappa scores (range 0.73–0.92) between histological and cytological diagnosis, was low. the most accurate FNA cytologic classification was obtained when the clinical context was known.L'étude de la variabilité interobservateur du diagnostic différentiel entre carcinome, sarcome et lymphome par cytoponction à l'aiguille fine a été conduite sur 80 cas pris au hasard. Deux lectures successives ont été faites ‘en aveugle’par 5 cytopathologistes. Les renseignements cliniques n'one été fournis que lors de la deuxième lecture. L'histopathologie est le standard de référence. Après la première lecture, la valeur prédictive positive moyenne du diagnostic de carcinome, de sarcome et de lymphome est respectivement de 93%, 94% et 86%. Après la seconde lecture, ces valeurs sont respectivement de 95%, 99% et 99%. La variabilité interobservateur, testée par le score kappa pondéré (intervalle de 0.73 à 0.92) entre les diagnostics histologiquest et cytologiques est faible. Le meilleur résultat est obtenu lorsque le contexte clinique est connu.Zur Überprüfung der Übereinstimmung zwischen verschiedenen Zytopathologen wurden 80 Präparate von Karzinomen, Sarkomen und Lymphomen zweimal randomisiert und von 5 Zytopathologen blind ausgewertet. In der 1. Runde erfolgte dies ohne Zusatzinformation, in der 2. mit klinischen Informationen. Bezugsstandard war die Histologie. In der 1. Runde waren die positiven praediktiven Werte für Karzinom, Sarkom und Lymphom 93%, 94% bzw. 86%. In der 2. Runde betrugen die Werte 95%, 99% und 99%.
