Summary:Dermal necrosis following coumarin: Is it immunologically induced? I. G. Hislop, P. J. Zilko, M. Petersen and N. Ahmed,Aust. N.Z. J. Med., 1980,10, pp. 51–53.Two patients with dermal necrosis due to anticoagulation therapy with warfarin are reported. Both patients demonstrated some disturbance in immunological function. It appears possible that warfarin may act as a happen in the induction of hypersensitivity to the drug. It is recommended that future cases should be studied to determine whether there is a defect in immunoregulation, and whether circulating immune complexes are responsible for the typical skin lesion

Summary:Apparent insulin resistance due to abnormal enzymatic insulin degradation: A new mechanism for insulin resistance. A. McElduff, C. J. Eastman, S. P. Haynes and K. M. Bowen.Aust. N.Z. J. Med., 1980,10, pp. 56–61.A 16–year‐old girl presented with severe, prolonged insulin resistance. Insulin antibodies, initially thought to be responsible for the insulin resistance, were suppressed using monocomponent insulin and immunosuppressive therapy; however insulin resistance persisted. Insulin kinetic studies suggested abnormal metabolism of a bolus injection of125insulin and the reappearance in the circulation of radioactive products, demonstrated by chromatography to be of different molecular weight to insulin. These products were of similar molecular weight to material obtained by incubating125/insulin with protease. Trasylol significantly reduced the patient's insulin requirements and normalised the disappearance of125/ insulin from the circulation. Prolonged treatment with Trasylol resulted in a fall in insulin requirement to non “insulin resistance” levels. The insulin requirement remained static when Trasylol was ceased. Med., 1980.10, pp. 69–72.Variant angina with ST elevation indicates transmural myocardial ischaemia and is due to spasm of a large epicardial coronary artery. Spasm occurs in arteries with varying degrees of fixed obstruction, giving rise to different clinical profiles of variant angina. However, coronary angiography is required to differentiate between those with minor coronary disease, and those with significant (>70%) obstruction. In patients with minor coronary disease or normal arteries, β‐blockers are contraindicated, and treatment with calcium antagonist vasodilators should be commenced after documentation of spontaneous or ergo‐novine induced spasm. Patients with significant fixed coronary obstructions require bypass grafting if technically feasible. The role of coronary spasm is not confined to variant angina, as it causes angina at rest with ST depression, and may also cause myocardial infarction and sudden
