Abstract:The aim of the study was to investigate the specificity of psychological profiles in female Irritable Bowel Syndrome (IBS) patients by a comparison with a gastrointestinal patient control group and with a normal population control group. The results showed that the IBS patients scored significantly higher than the normal population sample on three test scales: Anxiety, Phobic and Somatisation. Our results thus support the conclusion of earlier authors that a moderate degree of psychoneurotic disorder exists amongst IBS patients. However, the patient control group also scored significantly higher than the normal control group on two factors (Phobic and Somatisation), and the comparison between the IBS group and thepatientcontrol group did not reveal any significant differences. It is suggested that attention to the psychological needs of both IBS and other gastrointestinal disease patients should form an important part of the management strategy, particularly in the former — where a consistently clear benefit for any particular therapy has yet to be show

Abstract:An immunochemical test for fecal occult blood has been evaluated. It has been found to be specific for human hemoglobin and to be reproducible, accurate and four times more sensitive than chemical occult blood tests. Storage of prepared slides at ‐20°C prevented reduction in sensitivity. To determine the effect of blood from the upper gastrointestinal tract, six volunteers ingested 100 ml of their own blood. Positive chemical, but no positive immunochemical tests were produced. In 20 healthy subjects, challenge with red meat and vegetables with high peroxidase content increased the positivity rate of chemical tests but had no effect on the positivity rate of the immunochemical test.The immunochemical method for fecal occult blood has advantages over chemical testing in that it IS specific for human blood and for lower gut bleeding. Its increased sensitivity should result in a high detection rate of colorectal neoplastic lesions. Abstract:Twenty‐two hospital patients without gastrointestinal symptoms and fourteen patients with clinically suspected fat malabsorption were studied using the14C‐Triolein Breath Test and three day fecal fat estimation. The14C‐Triolein Breath Test was 100% sensitive and 86% specific for fat malabsorption. Three day fecal fat estimations were 86% sensitive and 91% specific. These data confirm the high sensitivity and specificity obtained using the14C‐Triolein Breath Test. The speed and convenience of the14C‐Triolein Breath Test plus acceptable sensitivity and specificity make this a practical screening test to detect fat mala

Abstract:Random and homosexual populations were tested for delayed type hypersensitivity (DTH) (Multitest) reactivity and T cell subsets. In the random population DTH reactivity was stronger in males irrespective of age. Reactivity in the homosexuals was generally even stronger especially with respect to tuberculin. As a group, male homosexuals had low OKT 4/8 ratios when compared with sex and age matched controls. Analysis of DTH and T cell subset results in the random group showed an association between anergy and low OKT 4/8 ratios. It is concluded that homosexuals demonstrate some alteration of cell mediated immunity and therefore may be at risk of various infections.

Abstract:Circulating low molecular weight (LMW) IgM was demonstrated in five of 38 patients with B cell lymphoproliferative disorders. These five patients all had malignant disease and could be subdivided into two groups. In the first group were three patients, each with an associated serum IgM paraprotein; two had Waldenstrom's macroglobulinemia. and one lymphocytic lymphoma. The two patients of the second group did not have IgM paraproteins; one had lymphocytic lymphoma and one chronic lymphocytic leukemia. Both these patients also had acquired C1 esterase inhibitor deficiency, a previously recognised association with circulating LMW IgM. None of the 16 patients with benign IgM macroglobulinemia had circulating LMW IgM. In those positive sera with LMW IgM this moiety contributed between 10.5% and 37.5% of the total IgM. There was no apparent association between LMW IgM and total IgM levels, kappallambda typing or the presence of Bence Jones proteinuria. but rheumatoid factor, immune complexes and cryoglobulins occurred in many of the sera which contained LMW IgM. Pokeweed mitogen stimulated peripheral blood mononuclear cells from two patients with circulating LMW IgM secreted considerable quantities of this moietyin vitrobut this did not occur in two patients with benign IgM macroglobulinemia.We conclude that LMW IgM is found in the malignant but riot the benign forms of B cell lymphoproliferative disorders and is frequently associated with other serological abnormalities. The basic abnormality causing defective IgM polymerisation in these disorders is obscure.

Abstract:Sixteen cases of subacute sclerosing panencephalitis (SSPE) were diagnosed at the Royal Children's and Mater Misericordiae Children's Hospitals in Brisbane from September 1974 to June 1983. Although the incidence of the disease with regard to age at onset, previous exposure to measles virus, and longevity was similar to that reported in the United States and Europe, the frequency of the illness was substantially higher in the present study. The high frequency of the disease in Queensland appears to reflect a relatively low level of immunity to measles virus in the population at risk.

Abstract:The incidence of acute epiglottitis treated in Melbourne, Australia at the Royal Children's Hospital (RCH) has increased from an average of 14 cases per year during 1975–79 to 49 cases per year during 1980–81. Less marked increases have occurred forH. influenzae meningitis and bacteremia andH. influenzae(untyped) isolation from respiratory tract cultures. A review of 171 cases of acute epiglottitis showed no significant differences between the 1975–79 and 1980–81 patients with respect to sex, age, seasonal incidence, prodromal length, prodromal symptoms, geographical location, polymorpho‐nuclear cell count, or disease severity. Patients treated in 1980–81 had fewer complications (12% versus 22%,p<0.02), and a shorter hospital stay (3.0versus3.3 days,p<0.003).
