AbstractOccult gastrointestinal blood loss induced by a new buffered aspirin preparation (Ostoprin, 4.0 g per day) was compared with that from an enteric‐coated aspirin (Ecotrin, 3.9 g per day) in 40 patients with osteoarthritis. Blood loss was measured by the radiochromium method and compared with the HemoQuantTMassay of fecal heme and heme‐derived porphyrins. By radiochromium, mean daily blood loss during the first week of treatment with Ostoprin increased 1.6 ml above basal compared to 0.8 ml above basal with Ecotrin (p= 0.06). When aspirin was ceased, blood loss returned more rapidly towards normal in the Ostoprin group (p<0.01). By HemoQuant,TMmean fecal hemeporphyrin excretion rose 0.63 mg hemoglobin equivalent per g feces during the first week of treatment by Ostoprin compared with 0.40 mg/g for Ecotrin (p= 0.06). There was a significant linear relationship between the two methods (r = 0.65,p<0.001). Serum salicylate levels achieved with both preparations were almost identical and encompassed the therapeutic range. HemoQuantTMis sufficiently sensitive to detect low‐level aspirin‐induced bleeding. AbstractEndocrine abnormalities in lymphomatoid granulomatosis are rare and have never been a major determinant of the clinical course. We describe three patients who illustrate that a wide range of endocrine dysfunction (hypoadrenalism, hypercalcemia, hypothyroidism, diabetes insipidus and hypogonadism) may be encountered in this disorder. Moreover, such endocrine manifestions may be the major feature at presentation, the treatment of which may significantly influence the subsequent clinical outcome.

AbstractHuman infection with Rhodococcus equi is apparently rare with most published reports describing the development of lung abscesses in immunocompromised hosts. Of only 18 cases of infection previously recorded, four have recently occurred in patients with the acquired immune deficiency syndrome (AIDS). In Australasia, R. equi has frequently been isolated from soil and infected farm animals yet no human infections have been reported thus far. Three cases of R. equi infection have occurred in New Zealand and, collectively, they cover a wider spectrum of disease than that previously recognised. The natural history of R. equi infections, their clinical features and treatment are described in the light of our recent experience. (Aust NZ J Med 1989; 19: 103–107

AbstractThe efficacy of papaverine and phenoxybenzamine as a pharmacological treatment of impotence was compared in a double blind, crossover, placebo controlled trial. Thirty‐four impotent men received intra‐cavernosal injections of papaverine (60 mg), phenoxybenzamine (6 mg) or normal saline (10 ml) at monthly intervals. Observations by EJK up to 45 minutes after injection demonstrated that papaverine caused full erections in 35% of men and partial erections in 62%. Phenoxybenzamine injection resulted in full erections in 20% of men and 62% had partial erections. The differences were not significant. Despite 82–97% of patients attaining some degree of erection in the clinical setting only six of the patients decided to use this mode of treatment for more than three months. AbstractThis report describes the clinical features of 13 cases of periodic, paroxysmal cerebellar ataxia. A family history suggestive of an autosomal dominant disorder was present in 10 cases, the first two reported Australian families involved with this disorder. Variation in the age of onset, frequency of attacks and the presence of distinct symptom complexes within one kindred confirm the heterogeneous nature of this disorder. CT head scans were performed on nine cases and were normal in eight. EEG findings were abnormal in six of nine performed. Treatment with acetazolamide resulted in abolition of paroxysms in nine of the 10 treated cases. In addition, two children had dramatic improvement in previously impaired gross and fine motor skills.
