Summary:HLA antigens were determined in 169 Australian patients with insulin dependent diabetes mellitus (IDDM). HLA‐B8 (47·9% v. 23·8%) and B15 (18·9% v. 8·2%) were significantly more frequent in the IDDM patients than in 1460 controls. The relative risks for developing IDDM in people carrying these antigens were 2·94 and 2·59 respectively. Carriers of the B8/B15 genotype had a relative risk of 5·48. Summary:Serum immunoreactive gastrin was measured in 14 patients with duodenal ulcer before and during a 12‐month course of cimetidine 400 mg bd. All patients were symptomatically well during the cimetidine therapy and both basal gastrin and that in response to a protein rich meal were assessed before, at six months and at 12 months during therapy. The basal and post‐prandial gastrin were significantly higher at six and 12 months on cimetidine than before cimetidine but the six and 12 month levels were similar. Summary:In the period from March 1970 to April 1978, 176 kidneys from 137 cadaver donors were exchanged, on the basis of HLA matching, between transplant centres throughout Australia and New Zealand. The kidneys were preserved by simple ice storage or machine perfusion, and transported by air in chipped ice. Total ischaemia times ranged from 3·5 to 28·6 hours and immediate function occurred in 52% of cases. Graft survival was 62% at one year, 51% at three years and 47% at five years, the corresponding patient survival rates being 86%, 74% and 71%. An average of 1·4 antigens mismatched was achieved, and 59% of recipients received kidneys with zero or one antigen mismatched. These kidneys survived better than those with two or more antigens mismatched, the difference being 12% at one year, and rising to more than 20% at five years. The results confirm the practicability and potential value of the Organ Sharing Scheme, and indicate that effective sharing of kidneys on the basis of zero HLA mismatches could substantially improve the current success rate of clinical cadaveric renal transplantati

Summary:Immune complexes were detected in 51 sera from patients with a variety of immunological diseases: 14 systemic lupus erythematosus (SLE); 14 infectious mononucleosis (IM); 12 rheumatoid arthritis (RA) and 11 subacute bacterial endocarditis (SBE). Three methods were used to detect complexes: the fluid—phase Clq binding assay (Clq.BA); the solid—phase Clq binding assay (Clq.SP) and the Raji cell radio‐immunoassay (RIA). Modification of the Clq.SP and the Raji cell RIA by use of monospecific antisera to immunoglobulins G, A and M enabled the class of antibody in the immune complexes to be determined. Summary:Electrophysiological techniques were used to study 17 patients with chronic recurrent ventricular tachycardia. Twelve of the patients had coronary disease, while eight had pre‐existing intraventricular conduction defects. The site of origin of the tachycardia as deduced by analysis of the QRS morphology correlated well with the electrocardiographic evidence of the site of the old infarction.Ventricular tachycardia was induced by pacing techniques in eight patients while it occurred spontaneously during the study in two others. In only four of the eight patients in whom tachycardia was induced, could the arrhythmia be reinduced; the arrhythmia could not be reproduced in one, while reproducibility was not assessed in the remaining three. Tachycardia was terminated by pacing techniques in six patients and by DC cardioversion in four. Ventricular tachycardia was initiated with a run of “torsade de pointes” in four patients. Ventricular tachycardias with varying morphology were observed in three patients. Summary:The diagnostic and potential therapeutic value of rapid right atrial pacing in ventricular tachycardia and supraventricular tachycardia with aberrant intraventricular conduction, was examined. The effect of right atrial pacing at incremental rates beginning 10 bpm above the rate of the tachycardia was studied in five patients with ventricular tachycardia, and in four patients with supraventricular tachycardia with rate‐related bundle branch block aberration, the mechanism of tachycardia having been demonstrated at electrophysiology study. Atrial pacing resulted in persistent (four) or occasional (one) normalisation of the QRS complexes to that seen in sinus rhythm in those five patients with ventricular tachycardia. The intraventricular conduction pattern persisted with atrial pacing in those patients with supra‐ventricular tachycardia and aberrant intraventricular conduction. This confirms that atrial pacing is a useful and simple diagnostic test in wide QRS tachycardia, which does not require sophisticated electrophysiological facilities. Summary:Thirteen patients with active systemic lupus erythematosus (SLE) and biopsy proven nephropathy were treated with thiamphenicol for two weeks. Laboratory indices of SLE disease activity (antinuclear factor, complement activity and ESR) were improved after the course of treatment in half of the patients and did not change significantly in the rest. Thiamphenicol may have value as an alternative to currently popular immunosuppressive drugs in the management of systemic lupus erythematosus.

Summary:In 12 (eight female) consecutive patients with primary aldosteronism (1971–1978), aged 15 to 54 years, the diagnosis was unsuspected until hypokalaemia (mean 2.2 mmol/l) was revealed by routine biochemical testing. In nine patients, hypertension was severe (diastolic blood pressure>120 mmHg, seven on treatment). A firm diagnosis of primary aldosteronism was based on sequential demonstration of (1) high urine potassium excretion with concurrent unexplained hypokalaemia, (2) inappropriately suppressed plasma renin activity after application of two potent stimuli to renin secretion, namely, salt‐free diet and upright posture, (3) inappropriately high aldosterone production demonstrated by measurement of 24‐hour urine aldosterone excretion after seven days of high salt diet and fludrocortisone acetate orally.Left adrenal vein catheterisation, which is said to provide preoperative localisation of aldosterone producing adenoma to the right or left gland and to distinguish aldosterone producing adenoma from bilateral hyperplasia of the zona glomerulosa, was successful in ten of 12 patients. A left‐sided adenoma was correctly predicted in eight, a right‐sided adenoma in one and results consistent with bilateral hyperplasia were found in one patient. Right‐sided adenomas were present in the two patients in whom adrenal vein catheterisation was unsuccessful. After removal of the 11 adenomas, plasma potassium returned to normal and hypertension was cured in nine patients and markedly improved in two.These results show that the combination of seven days of salt loading and fludrocortisone acetate to firmly establish the diagnosis, followed by adrenal venous catheterisation to lateralise the tumour, leads to a high cure rate in the treatment of aldosterone produc
