摘要:
For over 30 years, the Doppler effect has been used to identify blood flow with ultrasound. Many Doppler techniques are currently available, including continuous-wave Doppler, pulsed Doppler, color Doppler, and power Doppler. Each Doppler technique has strengths and limitations. Color Doppler provides a survey of blood flow in real time, which has improved our ability to detect and characterize vascular abnormalities throughout the body. However, to use the information provided by color Doppler and to help ensure accurate diagnosis, it is necessary to understand the basic principles of Doppler image interpretation thoroughly. In addition, to maximize the information obtained, it is important to adjust technical parameters to optimize the Doppler image. This article focuses on the basic concepts necessary to produce high-quality color Doppler images and to interpret those images accurately.

摘要:
Fecal incontinence due to anal sphincter disruption is a relatively common process affecting 2% of the population. While non imaging modalities have been utilized in the past, the recent development of endoanal ultrasound to evaluate the integrity of the anal sphincter is now becoming more commonly employed. Disruption of the sphincter using endoanal ultrasound techniques is accurately diagnosed in a high percentage of patients. In many instances, endoanal ultrasound has replaced or is used as an adjunct to non imaging techniques. This article discusses the clinical and imaging characteristics of fecal incontinence due to anal sphincter disruption.

Three-dimensional (3-D) ultrasound imaging is an emerging technology that has introduced a novel way to evaluate patient anatomy and pathology. Volume data are acquired by combining data from conventional two-dimensional (2-D) ultrasound images. The data generally are displayed with both multiplanar and volume-rendered images. Volume data can be rotated into a standard anatomic orientation and viewed systematically from any arbitrary orientation. This article reviews potential clinical applications of 3-D ultrasound in gynecology and obstetrics, and provides a rationale for the advantages of using 3-D ultrasound compared with 2-D ultrasound. In gynecology, reports suggest there are benefits related to evaluating congenital anomalies of the uterus and possible infertility, screening for endometrial cancer, diagnosis of fibroids and adnexal masses, and use of intrauterine devices (IUD) and 3-D sonohysterography. In obstetrics, the benefits involve evaluating the first-trimester gestational sac and embryo and identifying congenital anomalies of the face, skull, spine, heart, abdomen, and extremities.

A definitive diagnosis of a multiple gestation can be made sonographically from 6 weeks gestation on by visualizing two or more fetuses with heartbeats. When a multiple gestation is identified, sonographic evaluation of the pregnancy is more than a series of singleton examinations. In addition to the evaluation of each fetus on its own, the fetal sizes and amniotic fluid volumes should be compared, and the pregnancy must be evaluated for placentation type and for the complications unique to multiple gestations. Invasive procedures requiring ultrasound guidance are more complicated than in singleton pregnancies because of the presence of multiple fetuses and amniotic cavities.
