Sonographic artifacts are inherent to the modality and occur to some extent in every image. This is despite optimal technique and scanning parameters. Artifacts occur in both gray-scale and color Doppler imaging, and though many are readily recognizable as such, occasionally, an artifact may obscure the diagnosis or even create false pathology. This article illustrates the common and some of the more unusual sonographic artifacts that may be encountered, reviews the factors leading to their formation, and discusses strategies to recognize and minimize these misleading echoes.

Whereas acute appendicitis is the most common cause of right lower quadrant (RLQ) pain, numerous other conditions may cause signs and symptoms that mimic acute appendicitis. These include other appendiceal diseases, inflammatory bowel diseases, nonbowel gastrointestinal conditions, urinary diseases, and, in females, gynecologic diseases and conditions associated with pregnancy. The important role of ultrasonography in the diagnosis of not only acute appendicitis but also each of the other conditions that cause RLQ pain is described. The ultrasound criteria for the positive, negative, and indeterminate appendix ultrasound examination and the sensitivity, specificity, and positive and negative predictive values of this modality are discussed. The limitations of ultrasound in such diagnoses are also discussed, and an algorithm for the management of patients with RLQ pain is suggested.

Postcatheterization pseudoaneurysms are an increasingly common complication of endovascular procedures. Ultrasound (US) is essential in diagnosis and in playing the key role in the noninvasive treatment of such pseudoaneurysms. The past decade has seen a shift from surgical management to US-guided therapy initially using manual compression. Recently, the minimally invasive technique of percutaneous thrombin injection has been described as an alternative to US-guided compression. This review article discusses the cause and natural history of pseudoaneurysms and describes the technique of US-guided thrombin injection.

Six pregnant women with singleton fetuses were referred to our Prenatal Diagnostic Centers because of an initial diagnosis of intrauterine growth restriction (n = 4) or short femoral length (n = 2). We first examined the patients with two-dimensional (2D) ultrasonography (US), and then with three-dimensional (3D) US, and arrived at a definitive diagnosis of thanatophoric dysplasia. Although in experienced hands a diagnosis of thanatophoric dysplasia can be made with 2D US before the 24th week of gestation, our experience suggests that 3D US is a valuable complement to 2D US for the evaluation of fetuses with skeletal dysplasias.

Intraoperative ultrasound is a useful method of identifying renal masses during partial nephrectomy. It can be used to locate primary lesions, identify satellite lesions, assess for peritumoral vascularity and venous invasion. As nephron-sparing surgery becomes more widespread, intraoperative ultrasound of the kidneys will become more commonplace.

The objective of the study discussed was to develop an intrauterine growth retardation (IUGR) index to detect fetuses with IUGR. The study was conducted in Australia and was based on 219 pregnant women at Wollongong Hospital in the Illawarra region in New South Wales, Australia. Overall, 21 variables, including ultrasonographic variables and risk factors for IUGR, related to IUGR were tested in the sample. The results of discriminant analysis showed that hypertension, amniotic fluid index, abdominal circumference, head circumference, head circumference-to-abdominal circumference ratio, and low maternal weight gain in pregnancy have the highest F values. That means that these parameters were the most important predictors of IUGR in fetuses. An IUGR score was developed using these parameters. The developed index in this study has a reasonably high sensitivity (89.1%) and specificity (71.8%) in detecting IUGR in fetuses, compared with the developed indices to date. The IUGR score includes both ultrasonographic and maternal risk factor variables. This combination increases the accuracy of identification of IUGR in fetuses.

The article describes the development of ultrasound tables for biparietal diameter (BPD), femur length (FL), abdominal circumference (AC), and head circumference (HC) for fetuses in Iran. It was attempted to determine if racial differences exist between Iranian and Australian fetuses in ultrasound measurements of BPD, HC, AC, and FL. The sample for this study consisted of 70 Iranian and 93 Australian pregnant women. The Iranian women received prenatal care and delivered at Fatemieh Hospital in Shahrood, Iran during October 1998 through February 1999. All these women had an ultrasound scan for anomalies in the second trimester. We selected only those pregnancies in which the estimated date of delivery by last menstrual period was within ±14 days of the estimated date of delivery determined by the initial ultrasound examination. The data from Australian fetuses were obtained from the ultrasound section at the Wollongong Hospital in Australia. Birth weight differences were observed, with Australian infants being heavier than Iranian infants. The results also showed Australian fetuses have longer FL and larger HC, AC, and BPD than Iranian fetuses in the third trimester. Overall, this study shows that race-specific standards for estimating gestation age (GA) may be an important consideration in improving the accuracy of ultrasound estimation of GA. Because the evaluation of retardation growth retardation is so dependent on GA, assessment using standards specific to the Iranian race may more accurately classify infants and improve the prenatal diagnosis of fetal growth disturbances. Using European or Australian standards may cause overestimation of the rate of growth-retarded fetuses in Iranian pregnancies.
