Morphology and embryology in abnormal human development were well established by the 19th century, and early in the 20th century many causation factors were discovered and preventive measures introduced. Causation factors include genetics, disease, drugs, the environment and nutrition, and careful attention to all of them will always be required for the prevention of human fetal abnormalities.

There is evidence for a vociferous minority of the public in some countries against the application of recombinant DNA (rDNA) technology. This has in part been fuelled by the planned, and in some cases actual, release of genetically engineered organisms into the environment. Scientists are agreed that some control of such releases is required but that the guidelines for such control should be internationally agreed. The Organisation for Economic Cooperation and Development (OECD) is the best body to do this. Scientists should co-operate with their own politicians and regulating bodies, be scrupulous in adhering to the agreed procedures and inform people in all walks of life about their work and the concept of risk assessment. These measures should help to avert the danger that the benefits of rDNA are lost to medicine and agriculture.

One hundred and seventeen consecutive patients (mean age 55 years) with Q wave acute inferior myocardial infarction (MI) were studied. Of these, 62 developed atrioventricular (AV) conduction disorders (group I) and 55 did not (group II). The AV block occurred early (within 24 h) in 38 (Ia) and later in 24 patients (Ib). We report the in-hospital morbidity and mortality of these acute MI patients. There were no significant differences between group Ia and Ib patients with respect to coronary artery disease risk factors. Right ventricular MI was diagnosed in 14 (36%) and 7 (29%) group Ia and Ib patients, respectively (p = NS), and in 5 (9%) of group II patients. Mean peak serum creatine kinase was highest in group Ia patients (2,403 IU/1) compared to 1,860 IU/1 in group Ib and 1,369 IU/1 in group II. There was cardiogenic shock in 11 patients with 10 deaths in group Ia, while in group II, 2 patients had cardiogenic shock and 3 died (p < 0.01). Only mortality was significantly higher in group Ia compared to group Ib (p < 0.05). Cardiogenic shock was twice the rate in group Ib (n = 4) compared to group II (n = 2; p < 0.05), but there was no difference in mortality. A complete AV block was present in 32 patients (84 %), with 25 (66 %) requiring a pacemaker in group Ia, compared to 9 (37%), with 8 (33%) requiring a pacemaker, in group Ib (p 24 h) remains similar to those without this complication. The treatment of β-thalassaemia major by the new therapeutic modality of bone marrow transplantation has recently been carried out in Kuwait. A report on the first 2 children is given, illustrating the factors affecting the outcome of marrow transplantation for this disease. One child has remained in excellent health with normal haematological indices for more than 45 months after bone marrow transplantation. The advisability of adopting marrow transplantation in lieu of the conventional therapy for thalassaemia major is discussed

The angiogenic activity of normal and neoplastic animal tissue was studied in the chorioallantoic membrane preparation (CAM). Reticulated chick erythrocytes within the tumor implant were used as a criterion for angiogenesis. Normal adult rodent tissue from the thyroid, liver, spleen, adrenal, heart muscle, skeletal muscle, small bowel, salivary gland and pituitary did not induce angiogenesis. Fourteen syngeneic mouse tumors of various tissue origin all induced angiogenesis, but to a varying extent. Solid sarcomas and carcinomas and one melanoma had a high incidence of angiogenesis in the CAM test. Ascites tumors in solid form from subcutaneous transplants, with one exception, had lower activities. There was an inverse correlation between angiogenesis and the transplantation dose of the tumors. This indicates a relationship between malignancy-related cell characteristics and tumor vascularization. There was no statistical correlation, however, between angiogenesis and growth speed, or propensity to form spontaneous metastases.

This is a retrospective study of 44 surgical cases of hepatic hydatid cysts. The most common procedure was omentoplasty in 26 patients (55.3%), 10 with omentoplasty alone and 16 combined with partial cystectomy and tube drainage. The second most common procedure was external tube drainage in 15 (32.0%), where complication rates were higher (73.3%) than omentoplasty (15.4%). The most controversial aspect of hydatid surgery is the management of the cyst cavity. The major advantage of omentoplasty is that it is easy to perform and it is the best way to obliterate the remaining cavity. Hospitalization was shortest for omentoplasty. There was no mortality.

Twenty-two patients who underwent carotid endarterectomy for transient ischaemic attacks in our institution were evaluated and followed up for 6 months to 2 years postoperatively. While there was no hospital mortality, 1 patient had a transient stroke, and a second patient had a temporary visual loss during the early postoperative phase. A third patient also developed an infected pseudoaneurysm 1 month postoperatively, which was successfully managed by resection and antibiotic treatment but resulted in an asymptomatic occlusion of the carotid artery. Another patient developed a permanent stroke shortly postoperatively and died 2 months later from cardiac failure. Indications for surgical intervention and per- and postoperative management are discussed in relation to a recently presented classification of patients with cerebrovascular disease, which has been found very helpful for the standard identification and registration of patients, thus allowing a direct comparison of different modalities of treatment with other studies.

Eye trauma is a major cause of blindness, especially in children. A substantial proportion of trauma patients develop serious complications. This study includes 319 Arab patients with ocular trauma ranging in age from 4 to 60 years (median 17.6). In 180 cases (56%), the cause was blunt trauma which precipitated hyphaema. A further 114 cases (36%) were caused by perforated ocular wounds, and there were 15 (8 %) intraocular foreign bodies. The complication rate was 65.6%: cataract in 160 (50%), vitreous haemorrhage in 23 (7%), retinal detachment in 21 (6.6%), traumatic maculophathy in 7 (2%), endophthalmitis in 2 and glaucoma in 5. Visual results included 25 monocular blindness (7.8%), 58 (18%) who could only count fingers or perceive light (monocular), 18 with vision between 6/36 and 6/60, 27 (8.2%) between 6/18 and 6/24, and 191 (60%) with 6/12 or better.

A modified Arabic chart for assessing near and far vision is presented here. In addition to overcoming the wide-step grading in current charts, simple letters replace sentences for easier recognition by the average person in Arab populations. The near-vision chart reflects the actual power needed for correction.
