SUMMARYA case of spontaneous rupture of an apparently normal kidney is presented. A search of the literature revealed only three reports of a similar case. The probability that there are nevertheless logical explanations for these occurrences is discussed.

SUMMARY1One new case of nephrocolic fistula has been reported and represents the now commonest calculous, non–tuberculous, pyonephrotic type.2The four astiological factors responsible are presented.3The clinical presentation may be confusing, particularly after antibiotic administration, and radiological evidence in the form of a retrograde pyelogram or chromatological evidence using indigo–carmine provides the best diagnostic criteria.4The fistula is usually small and after separation closure is a simple matter.5The previously quoted mortality rate of 30 per cent, for this condition is undoubtedly much lower today because of earlier diagnosis and the advent of chemotherapy.I am indebted to Mr W. SUMMARYEstimation of blood levels after administration of 120 mg. colistin sulphomethate sodium revealed concentrations lower than those previously reported in this country. However, the blood levels obtained inhibited the growth of many strains ofPs. pyocyaneaandEsch. coli.Colistin S.M. is mainly excreted in the urine where very high concentrations were found.Twenty–one patients with urinary infection due toPs. pyocyaneawere treated with colistin S.M. and eradication was obtained in thirteen (62 per cent.) compared with five out of seventeen patients (29 per cent.) who were given other antibiotics.Following the administration of colistin S.M. to volunteers, a fall in creatinine clearance was observed in every case, and a transient rise in blood urea was also noted. This effect was not observed in patients with urinary tract infection and it was thought likely that any decrease in renal efficiency due to the colistin S.M. was balanced by the increase in efficiency resulting from control of the infection.Colistin S.M. is of value, therefore, in treatment ofPs. pyocyaneainfections andEsch. coliinfections resistant to the commoner antibiotics, but the nephrotoxic effect must be borne in mind especially where impaired renal function is suspected. The changes in renal function which we observed to follow the administration of colistin S.M. appear to be reversible.We wish to thank Mr J. de Louvois for his skilled technical assistance and Drs J. SUMMARYA case is reported of an adult in whom a multicystic kidney was associated with a contralateral polycystic kidney.The diagnosis and the significance of the association are discussed. SUMMARYThe results of the Boari flap operation for replacing the lower ureter following injury are described in twelve patients. Reflux was present in nine patients, but, in spite of the presence of a urinary tract infection in four, no deterioration in renal function has occurred over a two–to–twelve–year follow–up. Two patients have no ureteric reflux, one of whom had a submucosal reimplantation of the ureter into the flap; the other has a partial stenosis at the anastomosis. The remaining patient has pyelonephritis on the operated side and hypertension, but it is not known whether she has ureteric

SUMMARYA microdissection study of two post epididymo–vasostomy epididymides is reported. One of them, Case 8, had originally been operated upon in order to cure a post epididymo–orchitis azoospermia. The other, Case. 10, had a congenitally underdeveloped organ which had been operated upon for an azoospermia of unknown origin. In both cases the operation had been successful in that it had given rise to regenerative activity, and anastomotic connecting ducts had been formed between some of the vasa efferentia and the lumen of the implanted vas deferens. The regenerative process appeared to have been carried out entirely by vas efferens tissue as all the connecting ducts were lined by an epithelium of basic vas efferens type up to the point where they fused with the vas deferens. In Case 10 the regenerative process had been particularly active as the tissue in the region of the anastomosis contained large numbers of small, blind–ending tubules in addition to the main anastomotic ducts.
