Identification of a developmental process in phlebitis was made through observations of five clinical indicators known to be associated with phlebitis and the general measurement of body temperature. Subjects were inpatients admitted for medical problems in a medical center setting, ranged in age from 18 to 75 years, were about evenly distributed as to sex, and received peripheral I.V. therapy. Clinical indicators were skin temperature, size of induration, size of the area of skin discoloration, presence of a palpable cord vein, and tenderness at the site. Skin temperatures were taken at the venipuncture site and at four additional sites up to 4 cm from the venipuncture site by a thermistor; size of the area of skin discoloration was measured in centimeters by use of a template; palpable cord vein was noted as to its presence or absence; and the presence or absence of tenderness around the venipuncture site was verified by subjects. A general indicator of infection, body temperature, was measured by a digital thermometer. Using predetermined criteria, 23 patients on I.V. therapy were selected to participate in the study. Observations and recordings of the clinical indicators began during the first 8 hours of therapy and continued during each subsequent 8-hour interval. Data were collected over a total of seven 8-hour intervals, at which time either phlebitis had developed in subjects or I.V. therapy was discontinued. Findings showed that 60% of subjects developed clinical indicators of phlebitis. Of this number, approximately 40% had clinical indicators of phlebitis that became evident between the 8th and 16th hour of therapy. The developmental process for phlebitis was as follows: within 8 hours, redness developed around the veinpuncture site and tenderness was reported by subjects; within 16 hours, elevation of skin surface temperature 4 cm from the veinpuncture site occurred with a rise in general body temperature; within 24 hours, measurable evidence of combinations of clinical indicators of phlebitis were evident. Palpable cord vein and areas of induration were present in some subjects after 8 hours of therapy and later in others so that no particular sequence of emergence could be identified. The diameters of induration and discoloration increased the longer therapy was continued. No association was found between age and sex of subjects and emergence of phlebitis.
