摘要:
Cancer chemotherapy has become one of the significant modalities of treatment for the cancer patient. Many of the major advances in cancer treatment have been made in the area of chemotherapy. Nurses are involved in most aspects of care for the patient receiving chemotherapy. While we must be highly skilled technicians to ascertain safe administration of these agents, there is likewise a need to be knowledgeable about the agents we administer to avoid becoming purely technicians. Most importantly, the nurse has a duty to remain ever aware of the human needs of the patient she cares for. This means instilling confidence in her abilities, imparting her knowledge to the patient, and taking time to understand the fear and stress that become as routine for the cancer patient as the blood counts and venipunctures.

摘要:
Blood transfusion reactions encompass a wide spectrum of manifestations that may result in benign, catastrophic, or fatal patient outcomes. Immunologically mediated transfusion reactions are classified as hemolytic or nonhemolytic. Hemolytic reactions are further subclassified as being either acute or delayed. The differences and associated complications of the above are outlined in this discussion. Although hemolytic and nonhemolytic reactions may be differentiated by their symptoms, a definitive diagnosis depends upon laboratory testing. The laboratory tests and results used for these diagnoses are outlined. The similarities of emergency supportive care and differences in long-term management of hemolytic vs. nonhemolytic reactions are also discussed. Nursing responsibilities pertinent to monitoring and assessing the patient while blood/blood products are being administered are reviewed. These nursing skills coupled with timely and appropriate medical management minimize the physiologic consequences relative to hemolytic/nonhemolytic transfusion reactions.

摘要:
I.V. nitroglycerin is proving to be an important modality of treatment. The effects of I.V. nitroglycerin are decreased preload, relaxation of spastic arterioles, and decreased peripheral vascular resistance. I.V. nitroglycerin is being utilized for the following clinical problems: 1) refactory angina pectoris, 2) C.H.F. accompanying an acute MI, 3) control of blood pressure in perioperative hypertension, and 4) production of controlled hypertension during surgical procedures. It is well documented and clinically established that I.V. nitroglycerin should be administered from glass containers. Polyethylene tubing which is nonabsorbent to nitroglycerin should be employed to ensure accurate administration of the prescribed dose. An infusion instrument that has been tested should be used. Specific guidelines and policy should be established by each medical facility for the administration of I.V. nitroglycerin.

摘要:
With emphasis being placed on quality patient care and cost containment, home I.V. therapy appears to be the method of choice for some patients. One means of accomplishing this therapy is the utilization of silastic catheters. Even though these catheters may be used for a variety of procedures, the basic fundamentals of catheter care are essentially the same. Criteria for patient selection should encompass the need for prolonged therapy as well as the emotional and intellectual ability to provide self-care. After the patient is selected, the teaching process should begin immediately. With selection of suitable candidates and proper teaching techniques, patients can safely and successfully return to their home environment.

Central venous catheterization by the subclavicular approach is an invasive procedure that historically has been reserved for the physician. With the advent of the peripherally inserted central catheter, specially trained nurses can, with a great margin of safety, catheterize a central vein. The major obstacle is changing the point of view of physicians and administrators.
