Recent reports have expanded our understanding of the range of clinical presentations of the acute seroconversion syndrome and of the virologic and immunologic events that comprise acute human immunodeficiency virus (HIV) infection. A growing number of case reports have documented the highly variable, nonspecific manifestations of acute HIV infection, which may mimic a number of other illnesses. Previously, some manifestations of acute HIV infection have only been associated with established HIV disease. Diagnosis of the acute seroconversion syndrome requires not only a high index of suspicion but also persistence, as serial assays for p24 antigen and the development of HIV antibodies may be required for patients who present during the “window” period.

Fungal infections are a significant and growing cause of morbidity and mortality in patients with acquired immunodeficiency syndrome and human immunodeficiency virus. They are often difficult to recognize; treatment often fails to cure. New regimens seeking long-term suppression and prophylaxis have been explored. This article will focus on the developments in the last year involving these major mycotic infections, ie, candidiasis and other superficial mycoses, cryptococcosis, histoplasmosis, blastomycosis, coccidioidomycosis, and aspergillosis.

摘要:
The potential for drug interactions leading to adverse reactions is great in patients with the acquired immunodeficiency syndrome, since multiple drugs are commonly prescribed to these patients. In addition, patients with acquired immunodeficiency syndrome have a higher incidence of adverse reactions to drugs that are commonly used in the treatment of opportunistic infections than non-acquired immunodeficiency syndrome patients. As a result of the high rate of adverse reactions, drugs that are currently available are often limited in their use. An appreciation of the potential drug interactions and knowledge of the most frequently occurring adverse reactions can increase the chance of a successful therapeutic response.
