The list of cytokines detected in meningococcal disease includes tumour necrosis factor, IL-1, IL-6, IL-8, IL-10, soluble tumour necrosis factor receptor p55, soluble tumour necrosis factor receptor p75, IL-1 receptor antagonist and soluble IL-6 receptor. There is a uniform pattern of appearance with highest levels in meningococcal septic shock and, except for soluble IL-6 receptor, highest levels on admission to hospital. In recent years particular focus has been placed on the anti-inflammatory cytokines in meningococcal disease.

Chlamydiae are important human pathogens responsible for a wide range of infections and diseases. Chronic infections can be particularly devastating, as shown by diseases caused by Chlamydia trachomatis, such as trachoma and post-infectious tubal infertility, or possibly associated with Chlamydia pneumoniae, such as atherosclerosis. Cytokines are likely to be involved for the early resolution of chlamydial infections. However, recent data suggest a role played by cytokines in establishing chlamydial persistence, thus possibly contributing to chronic disease through immunopathological responses. Recent information from in-vitro cell culture systems and experimental animal models is discussed. Moreover, some new evidence about the effector functions of other immune mediators is presented.

Epidemiological and virological studies have established an association between some cases of insulin-dependent diabetes mellitus and certain viruses. The process leading to insulin-dependent diabetes mellitus is multifactorial and is dependent on the genetic make-up both of the virus and of the host. The best understood mechanism currently involves the coxsackieviruses for which a molecular mimicry between coxsackievirus P2-C protein and a protein on the beta cell membrane can lead to an autoimmune response that eventually destroys the beta cells.

The effectiveness of antibiotics has largely been attributed to their antibacterial properties. In the past few years, a number of published reports have described experiments that clearly demonstrate the ability of antibiotics to enhance, or in some cases attenuate, the immune response. These studies are reviewed, and the potential therapeutic applications of this newly recognized attribute of antibiotics is discussed.

Child care settings have been associated with the increased transmission of many infectious agents. Respiratory tract and enteric pathogens are the most frequent cause of infections in child care. Streptococcus pyogenes and Streptococcus pneumoniae have recently gained attention, especially in association with invasive disease and antibiotic resistance. Current strategies for the prevention and treatment of infections in child care facilities include personal and environmental hygiene, immunization, exclusion or cohorting of ill children, and the judicious use of antibiotic therapy.

摘要:
Systemic infection continues to be an important cause of morbidity and mortality in newborn infants. The incidence is particularly high in very low birthweight neonates who require intensive care treatment. Respiratory diseases, total parenteral nutrition, prolonged intravascular cannulization, broad spectrum antibiotics and H2blocker therapy are factors associated with an increased risk of septicaemia. The very different and distinct pattern of early versus late bacterial infection, and congenital versus postnatally acquired systemic fungal infection, is emphasized. A new syndrome of invasive cutaneous fungal dermatitis is described in the smallest and the most immature babies. Viral infection is increasingly recognized as an important entity, and potential preventive and treatment strategies are highlighted.
