Abstract.A method for large‐scale production of a pasteurized antithrombin III (AT III) concentrate for therapeutic use has been adapted from published methods. It includes the following steps: (1) batchwise adsorption onto heparin‐Sepharose from plasma depleted of cryoprecipitate and prothrombin complex; (2) chromatographic elution at high salt concentration; (3) pasteurization for 10 h at 60°C in the presence of added citrate ion; (4) desalting on Sephadex G‐25, and (5) sterile filtration and freeze‐drying.Seven batches prepared in this manner gave a mean yield of 269 UAT III/kg plasma. Abstract.Alpha1‐proteinase inhibitor (α1‐PI) has been prepared as a concentrate in quantities large enough for clinical testing of its safety and efficacy in the treatment of emphysema and other disorders. The α1‐PI was purified from Cohn fraction IV‐—I paste by polyethylene glycol precipitation and DEAE‐Sepharose chromatography. The methods used in the purification are gentle and the resulting product behaves almost identically to the α1‐PI from plasma. The protein has been heat treated (60°C, 10 h) to lower the risk of transmission of plasma‐borne diseases. Abstract.The isotype profiles of anti‐Rh (D) antibodies were measured from 16 serum samples with a solid‐phase radioimmunoassay that employed monoclonal anti‐isotype antibodies. These antibodies had been standardized in another solid‐phase system with the aid of monoisotypic antibodies (e.g., anti‐tetanus toxoid of isotype IgG4), and this permitted calculations of the relative concentrations of different isotypes in each anti‐D population. Of the heavy‐chain isotypes, only IgG1 and IgG3 were found in anti‐Rh (D) antibodies. The share of IgG1 varied from 100% (2 sera) to 0% (1 serum) of detected units, and the mean proportion was 73.8%. Abstract.Immunoadsorption columns capable of specifically removing antibodies show promise as an alternative treatment to plasma exchange. Most columns are designed to be perfused with plasma and it is necessary to use a plasma separation device (filtration or centrifugation) for the procedure. This increases the cost and complexity of the immunoadsorption procedure. We have adapted immunoadsorption columns capable of removing anti‐A or anti‐B red cell antibody from plasma to a whole blood perfusion system by coating the silica with a thin layer of collodion and incubating with albumin (1%). Patients were given heparin, 25 units/kg intravenously, followed by a continuous infusion of 3% citrate and heparin, 10 units/ml, to achieve a ratio of 1:16–1:20 (anticoagulant to whole blood) before delivery through the columns. Abstract.One example of Rh blood group mosaicism showing a mixed cell population of CcDe and cde red blood cells was found in tests on 552 normal individuals aged 60 years or more. The mosaic person was uninformative for other chromosome 1 markers having the phenotypes Fy (a–b+), 6‐PGDAand PGM11. Abstract.364 units of fresh frozen plasma (FFP) used at the Medical College of Ohio Hospital between April and July 1982 were analyzed for their pattern of use. According to the criteria developed for the survey, 33% of the units were used for blood pressure support, 34% were used for clotting support, 14% were used for the combined reasons of blood pressure and clotting support, 11% of the units were used during therapeutic pheresis, and the remaining 7% were used for unidentified reasons. 39% of the FFP were given with red blood cells. Since FFP transfusions carry potentially more serious adverse effects than albumin/plasma protein fractions, their use as volume expanders, or to reconstitute whole blood, should be discouraged.
